Development of children with early language delay.

Four children with early language delays (ELD) were compared to a control group of 12 children with respect to their preschool language abilities from age 2 1/2 to 5 years and their verbal skills at the end of Grade 2. The language-delayed children each initially showed severe and broad impairments in syntactic, phonological, and lexical production. Over time, their deficits became milder and more selective, such that normal or nearly normal speech and language proficiency was exhibited by age 60 months. Nevertheless, when followed up 3 years later, three of the four cases were severely readin…

Defining dyslexia as a developmental language disorder.

Despite recent developments in research and theory, investigators and practitioners continue to rely on rather traditional definitions of dyslexia. This paper discusses some of the problems with traditional definitions and reviews a rapidly growing body of research that suggests a more comprehensive definition. According to this definition, dyslexia is a developmental language disorder that involves a deficit(s) in phonological processing. This disorder manifests itself in various phonological difficulties as well as a specific reading disability.

Evaluation of the child with delayed speech or language.

Because of the relative frequency of speech/language delay, all infants and preschool children should undergo routine language screening as part of health care maintenance. Diagnostic evaluation of the child with speech or language delay should answer the following questions: What is the child's descriptive diagnosis (eg, hearing impaired, mentally retarded, DLD, etc.)? What is the child's etiologic diagnosis (eg, congenital viral infection, single gene disorder, birth asphyxia, etc.)? What is the appropriate intervention strategy (amplification, orally based speech therapy, total communicatio…

Apraxia and aphasia for a visual-gestural language.

Since signed languages utilize visual-gestural channels, their study allows a unique opportunity for insight into the ways language and gesture may be represented in the brain. The separability of apraxia and aphasia for sign language was examined in four deaf signers who had unilateral brain damage, three to the left hemisphere and one to the right hemisphere. These patients were administered various tests for apraxia and a test of pantomime recognition. The patient with damage to the right hemisphere was not apraxic as we would expect. For the patients with damage to the left hemisphere, all…

Language and cultural influences in the description of pain.

Several lines of argument have been pursued in the attempt to study the meanings of pain descriptions in English. First, the component terms … fers implicit in English pain descriptions realize one important semantic component of pain in this language. The second way of analyzing semantic components of English pain involved a grammatical analysis of paradigmatic sentences which realize pain descriptions. A pain experience space was posited on the basis of this grammatical analysis. Each well-form …

Effects of age, sex, language ability and second language listening on children's recognition of nonwords and sentences in noise and reverberation.

OBJECTIVES: To evaluate children's ability to recognise speech and its relationship to language ability using two newly developed tests: the Listening in Spatialised Noise and Reverberation test (LiSN-R) and the Test of Listening Difficulties - Universal (ToLD-U). DESIGN: LiSN-R and ToLD-U used nonword and sentence recognition in spatially separated noise and reverberation. Language ability was assessed using the Clinical Evaluation of Language Fundamentals (CELF) sentence recall. STUDY SAMPLE: Normally-hearing children aged 6-11 years, who spoke English either as a Second Language (ESL) or …

Stigmatizing Language in Large Language Models for Alcohol and Substance Use Disorders: A Multimodel Evaluation and Prompt Engineering Approach.

OBJECTIVES: Large language models (LLMs) are increasingly used in health care communication but can inadvertently perpetuate stigmatizing language toward individuals with alcohol and substance use disorders. Despite growing interest in LLM performance, a focused evaluation of their propensity for SL and strategies to mitigate it remains lacking. METHODS: We generated 60 clinically relevant questions ["prompts"; 20 each for alcohol use disorder (AUD), alcohol-associated liver disease (ALD), and substance use disorder (SUD)] and tested 14 LLMs. Two physicians independently assessed all response…

Speech categorization consistency is associated with language and reading abilities in school-age children: Implications for language and reading disorders.

Speech perception is fundamental to language and reading abilities. While these skills are correlated, most studies examining the role of speech perception on outcomes do not test both concurrently. Moreover, traditional forced-choice tasks have limitations in accurately indexing these relationships. This study used a visual analog scaling task-a continuous measure of speech categorization-to examine speech categorization alongside conventional language and reading assessments in a large sample of children (n = 237), including those with language and/or reading disabilities. Children with poor…

Validation of the Sydney Language Battery naming subtest and utility of latency analysis in characterizing language impairment in multiple sclerosis.

BACKGROUND: Language deficits are frequently described by patients with multiple sclerosis (MS); however, objective characterization remains somewhat limited due to its omission from standard MS cognitive evaluation and the inconsistent findings that arise from current language measures. OBJECTIVE: To establish alternative approaches to characterizing single-word level language in MS, this study (i) validates the Sydney Language Battery (SYDBAT) visual confrontation naming subtest and (ii) examines the insights provided by examining naming errors and latencies. METHODS: 40 MS patients from R…

No evidence of altered language laterality in people who stutter across different brain imaging studies of speech and language.

A long-standing neurobiological explanation of stuttering is the incomplete cerebral dominance theory, which refers to competition between two hemispheres for 'dominance' over handedness and speech, causing altered language lateralization. Renewed interest in these ideas came from brain imaging findings in people who stutter of increased activity in the right hemisphere during speech production or of shifts in activity from right to left when fluency increased. Here, we revisited this theory using functional MRI data from children and adults who stutter, and typically fluent speakers (119 part…

The impact of early speech and language cognitive training care on the developmental quotient of children with language impairments: A retrospective study.

This study aims to investigate the effects of early speech and language cognitive training on the nursing outcomes of children with language impairments and their impact on developmental quotient. From October 2018 to October 2023, the data of 80 children with language impairments treated at our hospital were selected. They were divided into an observation group (n = 40) and a control group (n = 40) based on the treatment plan. The control group received routine rehabilitation training, while the observation group received early cognitive language rehabilitation training. The treatment efficac…

The scope of using pragmatic language tests for early detection of dementia: A systematic review of investigations using figurative language.

INTRODUCTION: Dementia cases are expected to rise to 81.1 million in 2040. Efforts are underway to develop diagnostic methods to facilitate early detection of the disease. Herein we review research findings focusing on pragmatic dysfunction in patients with dementia and evaluate the usefulness of assessing dementia and its progress with a battery of tests assessing figurative language skills. METHODS: A total of 74,778 article titles were identified from EMBASE, PubMed, and Google Scholar databases. After systematic screening, 51 journal articles were selected for the final review. RESULT: T…

UK speech and language therapists' assessment of children's expressive language, and functional impairment and impact, following the CATALISE publications.

BACKGROUND: In 2016/17, the CATALISE Consortium published the results of a multinational and multidisciplinary Delphi consensus study, representing agreement among professionals about the definition and process of identification of children with Developmental Language Disorder (DLD) (Bishop et al., 2016, 2017). The extent to which the current clinical practice of UK speech and language therapists (SLTs) reflects the CATALISE consensus statements is unknown. AIMS: To investigate how UK SLTs' expressive language assessment practices reflect the CATALISE documents' emphasis on the functional imp…

Sources of variability in the prospective relation of language to social, emotional, and behavior problem symptoms: Implications for developmental language disorder.

Children with developmental language disorder (DLD) are at risk for social, emotional, and behavioral (SEB) maladjustment throughout development, though it is unclear if poor language proficiency per se can account for this risk as associations between language and SEB appear more variable among typical-language children. This study investigated whether the relationship between language and SEB problems is stronger at very low levels of language and considered confounders including socioeconomic status, sex, and nonverbal intelligence. These were examined using a population-based survey design…

A View of the Lexical-Grammatical Link in Young Latinos With Specific Language Impairment Using Language-Specific and Conceptual Measures.

Purpose This study investigated the nature of the association of lexical-grammatical abilities within and across languages in Latino dual language learners (DLLs) with specific language impairment (SLI) using language-specific and bilingual measures. Method Seventy-four Spanish/English-speaking preschoolers with SLI from preschools serving low-income households participated in the study. Participants had stronger skills in Spanish (first language [L1]) and were in the initial stages of learning English (second language [L2]). The children's lexical, semantic, and grammar abilities were assesse…

Recent Issues in the Use of Signed Language Assessments for Diagnosis of Language Disorders in Signing Deaf and Hard of Hearing Children.

In recent years, normed signed language assessments have become a useful tool for researchers, practitioners, and advocates. Nevertheless, there are limitations in their application, particularly for the diagnosis of language disorders, and learning disabilities. Here, we discuss some of the available normed, signed language assessments and some of their limitations. We have also provided information related to practices that should lead to improvement in the quality of signed language assessments.

Is weak oral language associated with poor spelling in school-age children with specific language impairment, dyslexia or both?

The purpose of this study was to test the hypothesis that word reading accuracy, not oral language, is associated with spelling performance in school-age children. We compared fourth grade spelling accuracy in children with specific language impairment (SLI), dyslexia or both (SLI/dyslexia) to their typically developing grade-matched peers. Results of the study revealed that children with SLI performed similarly to their typically developing peers on a single-word spelling task. Alternatively, those with dyslexia and SLI/dyslexia evidenced poor spelling accuracy. Errors made by both those with…

Language use in social interactions of school-age children with language impairments: an evidence-based systematic review of treatment.

INTRODUCTION: Pragmatic models were first applied to the treatment of children with language impairment in the late 1970s. Since that time, the study of language use has had considerable impact on language assessment and treatment. Despite the need to address pragmatic language skills clinically, there has been no systematic examination of the efficacy of treatments developed for this purpose. METHOD: In accordance with the evidence-based practice policy agenda, the American Speech-Language-Hearing Association (ASHA) convened an ad hoc committee on language use in social interactions in schoo…
