A systematic review of emergency room laparotomy in patients with severe abdominal trauma.

Traumatic intra-abdominal hemorrhage contributes to mortality in patients with trauma. However, initiating an emergent laparotomy in the operating room (OR) as a standard treatment can sometimes be time-consuming. To overcome this issue, laparotomy is performed in the emergency room (ER) in some institutions. This systematic review evaluates the efficacy of performing ER laparotomy. Comprehensive searches were conducted in MEDLINE PubMed, EMBASE, and Cochrane databases, up to August 9, 2024. The risk of bias in observational studies was assessed using the ROBINS-I tool. The primary outcome was…

Green surgery: a systematic review of the environmental impact of laparotomy, laparoscopy, and robotics.

Surgery is the most energy-intensive healthcare sector, but data on the environmental impact of abdominal surgical techniques are limited. This systematic review aims to identify the most sustainable approach among open, laparoscopic, and robotic surgeries. We searched MEDLINE, Cochrane, and Web of Science databases (inception to March 2024) for studies on the carbon footprint of abdominal surgery, focusing on carbon dioxide equivalents (CO2e) or CO2 emissions. The Joanna Briggs Institute checklist was used to assess bias. (PROSPERO: 298486). Of 2155 records, eight cohort studies were included

Retrospective cohort study of perioperative factors and outcomes in emergency laparotomy in Palestine.

BACKGROUND: Few studies have evaluated outcomes following emergency laparotomy in Palestinian practice. In this retrospective cohort study, we assessed the association between perioperative factors and the primary outcome of in-hospital mortality. Secondary outcomes included the evaluation of perioperative risk factors for postoperative complications, overall complication and mortality rates, length of hospital stay, intensive care unit (ICU) admission rates, and the proportion of patients requiring postoperative intubation. METHODS: We conducted a retrospective cohort study of 385 consecutiv…

Evaluation of sarcopenia and myosteatosis to determine the impact on mortality after emergency laparotomy.

BACKGROUND: Emergency laparotomy is performed for a wide range of life-threatening conditions and is associated with significant morbidity and mortality. Risk prediction models facilitate accurate communication of operative risk with patients and relatives, in addition to benchmarking unit outcomes. Greater understanding of the impact of sarcopenia or myosteatosis will encourage the adoption of routine radiological reporting of body composition and the incorporation of skeletal muscle gauge (SMG) into risk prediction models. This study investigated the prognostic significance of SMG, an aggreg…

Selection of Laparoscopy or Laparotomy for Treating Cesarean Scar Pregnancy: A Retrospective Study.

OBJECTIVE: Cesarean scar pregnancy (CSP) can have catastrophic consequences, but a standardized diagnosis and treatment for CSP are lacking. At least 10 different treatments are currently available, further confusing treatment selection. The aim of this study was to compare the outcomes of using laparotomy or laparoscopy to treat CSP. METHODS: We reviewed data from 935 patients with suspected CSP from 1 January 2013 to 31 December 2018. A total of 278 patients were included in the study, of whom 121 were treated with laparoscopy and 157 were treated with laparotomy. We compared and analyzed t…

The results of rapid source control laparotomy or open abdomen for acute diverticulitis.

INTRODUCTION: Rapid source control laparotomy (RSCL) for the management of non-traumatic intra-abdominal emergencies has increased over the past 25 years when it was advocated for trauma patients. Little data, however, support its widespread use. We hypothesize that the patients with RSCL will have poorer outcomes than those treated with primary fascial closure (PFC). METHODS: Patients operated for acute diverticulitis from 2014 to 2016 using The American College of Surgeons sponsored National Surgical Quality Improvement Program (NSQIP) data were reviewed. Two groups were identified: PFC, pa…

Mesh closure of epiploic foramen by ventral laparotomy in 17 horses with entrapment.

BACKGROUND: Epiploic foramen entrapment (EFE) of small intestine is a severe cause of strangulating small intestinal obstruction (SSI) with long-term survival seeming lower than for other causes of SSI in horses. Different techniques via laparoscopy or laparotomy for epiploic foramen (EF) closure have been developed. METHODS: This study describes a technique of peroperative mesh closure of the EF in clinical cases and their long-term follow up. RESULTS: In the study period of 5.5 years, 36 horses were admitted to the clinic with EFE. Of these, 17 horses had peroperative mesh closure, with re…

Does the emergency surgery score accurately predict outcomes in emergent laparotomies?

BACKGROUND: The emergency surgery score is a mortality-risk calculator for emergency general operation patients. We sought to examine whether the emergency surgery score predicts 30-day morbidity and mortality in a high-risk group of patients undergoing emergent laparotomy. METHODS: Using the 2011-2012 American College of Surgeons National Surgical Quality Improvement Program database, we identified all patients who underwent emergent laparotomy using (1) the American College of Surgeons National Surgical Quality Improvement Program definition of "emergent," and (2) all Current Procedural Ter…

Outcome of primary closure of abdominal wounds following laparotomy for peritonitis in children.

BACKGROUND: Primary wound closure following laparotomy for peritonitis is generally believed to be associated with wound complications and long hospital stay. Open wound management has long been the most common practice after laparotomy for peritonitis. Primary closure (PC), however, has recently been advocated to reduce cost and morbidity. This study determined the incidence and severity of wound complications and their impact on hospital stay and overall outcome when PC of abdominal wounds is done following laparotomy for peritonitis. PATIENTS AND METHODS: A prospective review of patients w…

[A case of cardiac arrests before and after emergent exploratory laparotomy for panperitonitis].

We report a case of cardiac arrest before and after emergent exploratory laparotomy for panperitonitis in an 84-year-old woman with a history of hypertension, gastric ulcer, uterine myoma and dementia. She complained of lower abdominal pain, and suffered from septic shock and DIC. The first cardiac arrest occurred after anesthesia induction. Following resuscitation, a left hemicolectomy and colostomy were performed. The second cardiac arrest occurred immediately after the operation. Cardiac arrest in this case may have been due to preexisting cardiac dysfunction enhanced by septic shock. Promp…

Emergency laparotomy helped the resection of an intralobar pulmonary sequestration with haemorrhagic shock.

Massive intrapulmonary haemorrhage and haemothorax are uncommon presentations associated with pulmonary sequestration. Here, we describe the case of a 40-year-old man who suffered from high fever and haemoptysis for 1 week before he was admitted to our hospital with a complaint of chest discomfort with shock. Computed tomography revealed that pulmonary sequestration supplied from the coeliac artery with persistent bleeding. The patient underwent right lower lobectomy and an emergent laparotomy for ligation of the aberrant artery. A pulmonary sequestration has a severe complication resulting in…

Adverse events associated with laparoscopy vs laparotomy in the treatment of endometrial cancer.

OBJECTIVE: The objective of the study was to compare adverse event rates between laparoscopic vs open surgery for endometrial cancer. STUDY DESIGN: This was a retrospective cohort study comparing 107 women who underwent laparoscopy with 269 age- and body mass index-matched women who underwent laparotomy for treatment of endometrial cancer. RESULTS: Adverse event rates were similar between cohorts (37% laparoscopy vs 43% laparotomy, P=.248). Laparotomies had higher rates of cellulitis (16% vs 7%, P=.018) and open wound infection (9% vs 2%, P=.02), whereas laparoscopy had higher rates of senso…

Anesthetic management of the emergency laparotomy for a patient with multiple sclerosis -A case report-.

A 33-year-old male patient with multiple sclerosis (MS) received an emergency laparotomy because of perforated appendicitis. He had been suffering from MS for 2 years and the symptoms of MS were paraplegia and urinary incontinence. Anesthesia was induced with propofol and remifentanil and maintained with nitrous oxide, sevoflurane and remifentanil. Rocuronium was used for tracheal intubation. Train of four ratio and bispectral index scale were also monitored for adequate muscle relaxation and anesthetic depth. The patient emerged from general anesthesia smoothly and was extubated without any c…

Management of colon wounds in the setting of damage control laparotomy: a cautionary tale.

BACKGROUND: Although colon wounds are commonly treated in the setting of damage control laparotomy (DCL), a paucity of data exist to guide management. The purpose of this study was to evaluate our experience with the management of colonic wounds in the context of DCL, using colonic wound outcomes after routine, single laparotomy (SL) as a benchmark. METHODS: Consecutive patients during a 7-year period with full-thickness or devitalizing colon injury were identified. Early deaths (<48 hour) were excluded. Colon-related complications (abscess, suture or staple leak, and stomal ischemia) were co…

Intestinal perforation by a foreign body, without identifying a perforation site at laparotomy.

A case is reported of a young man, who was presented with abdominal pain and an inflammatory abdominal mass resulting from intestinal perforation by a wooden spike. Thorough surgical exploration of the abdominal cavity didn't reveal the perforation site. An inflammatory mass in small bowel mesentery was excised and the cavity drained. The patient had an uneventful recovery. In cases of intestinal perforation by wooden spikes the site of perforation may not be identified. Healing of the opening after the passage of the spike is the possible mechanism. The outcome of these patients even without
