Neoplasms of the Cervical Spine: Diagnostic Workup and Clinical Decision-making Algorithms.

STUDY DESIGN: Narrative review. OBJECTIVE: To synthesize current evidence on diagnostics and clinical decision-making for patients with cervical spine neoplasms. BACKGROUND: Although the cervical spine is disproportionately affected by primary spinal tumors and is the predominant site for intramedullary spinal cord tumors, there remains a relative paucity of reviews focused specifically on cervical spine neoplasms. METHODS: PubMed and Scopus were searched using a combination of terms related to cervical spine neoplasms, classification schemas, diagnostic evaluation, staging, and clinical de

[Biochemical markers of bone formation activity in children with neoplasms].

UNLABELLED: The aim of the study was the analysis of biochemical markers of bone turnover in children with neoplasms comparing with the group of healthy children. We explored the relationships between sex, pubertal development and type of neoplastic disease. 85 children (53 boys) with newly diagnosed neoplasms: 48 with acute lymphoblastic leukaemia (ALL), 13 with Hodgkin's disease and 22 with solid tumors (group I) and 111 healthy children (group II) were studied. We measured in plasma: 1. bone specific alkaline phosphatase (BALP), 2. osteocalcin (OST) as the markers of bone formation and 3. I

Small Bowel Neuroendocrine Neoplasms.

Small bowel neuroendocrine neoplasms (SB-NENs) are a distinct subgroup of gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs), characterized by marked biological heterogeneity and a high prevalence of metastatic disease at diagnosis. Their frequently nonspecific clinical presentation, together with small and often multifocal primary lesions, poses significant diagnostic and therapeutic challenges. This review provides a comprehensive overview of SB-NENs, focusing on recent epidemiological trends, key elements of the 2022 World Health Organization (WHO) classification and of the American

SpyGlass guided PDT for advanced intraductal papillary mucinous neoplasm of the bile tract: A case report and literature review.

Intraductal papillary mucinous neoplasm of the bile tract is a rare biliary tumor characterized by mucin growth within the bile duct. In the early stages, it often presents without significant obstruction, this often leads to its discovery in the advanced stages. We report a case of a 63-year-old female with an intraductal papillary mucinous neoplasm of the bile duct (IPMN-B). The patient had a history of intrahepatic bile duct stones and biliary ascariasis. She gradually developed symptoms such as jaundice and intermittent fever before admission, and a bile duct biopsy confirmed the diagnosis

Synchronously occurring endometrioid carcinomas of the uterine corpus and uterine cervix preceded by different precancerous lesions: A case study and a literature review.

The incidence of two synchronous carcinomas originating from the uterine corpus and uterine cervix, both endometrioid subtypes, is exceedingly rare. Herein, we presented synchronous early stage G1 adenocarcinoma of the uterine corpus with cervical G2 endometrioid adenocarcinoma. Although both neoplasms displayed the same histological subtype, they differed significantly according to the histological grading or clinical stage of the disease. Finally, it is worth emphasizing that both tumors were preceded by different precancerous lesions, atypical endometrial hyperplasia (AEH) and foci of endom

Acute reactive stage proteins in gynaecologic cancers.

The authors report the plasmatic variations of ARPS (acute reactive proteins stage) in fifty eight women, admitted to the Obstetric and Gynaecological Clinic of Padua's University in the years 1978-1979 aged between 35 and 80 years old; those patients were affected by different types of gynaecological neoplasms: twenty three had cervical cancer, eight endometrial cancer, twenty six ovarian cancer. An evident increase was seen in the ARPS in the patients studied. In particular there was an increase in he haptoglobin in the ovarian cancer patients as well as an increase in the alpha-1 antitrypsi

Metachronous early gastric adenocarcinoma presenting coinstantaneously with complete remission of stage IV gastric MALT lymphoma.

Adenocarcinoma and lymphoma represent the two most common malignant tumours of the stomach, with both neoplasms being associated with infection by Helicobacter pylori. However, the presence of lymphoma and adenocarcinoma in the same patient is a rare entity with synchronous neoplasms being more common than metachronous types. We report a case of stage IV gastric MALT lymphoma of the gastric angle with infiltration of the bone marrow successfully treated with chemotherapy and the occurrence of metachronous early gastric adenocarcinoma of the fundus presenting 1 year after the diagnosis of the l

Neuroendocrine tumors of the gastrointestinal tract and pancreas: Is it also a challenge for pediatricians?

Neuroendocrine tumors (NET) of the gastrointestinal tract and pancreas are extremely rare in the pediatric population and limited data is available. In most cases, NET of the gastrointestinal tract in children are located in the appendix. Pancreatic NET are a small but partially distinct group of the gastrointestinal neuroendocrine neoplasms. The most common in this group are insulinomas; however, in some research, the gastrinoma type neoplasms are perceived to be most common in children. This study reviews the typical clinical presentation, appropriate diagnostics, staging, and treatment of t

Myeloid/Lymphoid Neoplasms with Eosinophilia and TK Fusion Genes, Version 3.2021, NCCN Clinical Practice Guidelines in Oncology.

Eosinophilic disorders and related syndromes represent a heterogeneous group of neoplastic and nonneoplastic conditions, characterized by more eosinophils in the peripheral blood, and may involve eosinophil-induced organ damage. In the WHO classification of myeloid and lymphoid neoplasms, eosinophilic disorders characterized by dysregulated tyrosine kinase (TK) fusion genes are recognized as a new category termed, myeloid/lymphoid neoplasms with eosinophilia and rearrangement of PDGFRA, PDGFRB or FGFR1 or with PCM1-JAK2. In addition to these aforementioned TK fusion genes, rearrangements invol

Urachal Mucinous Cystic Tumor of Low Malignant Potential with Concurrent Sigmoid Colon Adenocarcinoma.

Urachal mucinous tumors are rare neoplasms with behaviour that can range from relatively benign to malignancy that can spread distantly or throughout the peritoneum as pseudomyxoma peritonei or peritoneal carcinomatosis. Here we describe a unique case of urachal mucinous cystic tumor of low malignant potential confined to an intact cyst at the dome of the urinary bladder, without rupture or peritoneal spread. The urachal mucinous tumor was an incidental finding on a staging CT scan performed for sigmoid colon adenocarcinoma. We believe that this case illustrates a potential diagnostic pitfall

Ingestion of a foreign body unmasks an asymptomatic small bowel carcinoid tumor.

Bowel obstruction is a common surgical admission around the world. On the other hand, small intestinal tumors, such as midgut carcinoid, are uncommon neoplasms and an infrequent cause of intestinal obstruction leading to hospitalization. A foreign body is an extremely rare cause of intestinal obstruction and when ingested, foreign bodies most often lodge in the narrowest portion of the gastrointestinal tract. Narrowing of the small bowel due to a neoplasm can prohibit the passage of an accidentally ingested foreign object and produce an obstruction that neither the neoplasm nor the foreign bod

Expression of cell cycle-related molecules in biliary premalignant lesions: biliary intraepithelial neoplasia and biliary intraductal papillary neoplasm.

Cholangiocarcinoma of intrahepatic and extrahepatic bile ducts has a multistep carcinogenesis. Two premalignant lesions have been suggested for invasive cholangiocarcinoma: biliary intraepithelial neoplasia and intraductal papillary neoplasm of the bile duct. How the carcinogenetic process differs between biliary intraepithelial neoplasia and intraductal papillary neoplasm of the bile duct is not clear. In this study, we performed a pathological study to reveal the expression of key molecules related to the cell cycle during 2 carcinogenetic lineages. We immunohistochemically examined the expr

Primary Malignant Mesothelioma of the Tunica Vaginalis: A Case Report and Review of the Literature.

Primary malignant mesothelioma of the tunica vaginalis is an extremely rare neoplasm, and its diagnosis is often unclear initially due to its nonspecific presentation. We present the case of an 87-year-old gentleman who presented with a left-sided hydrocele that increased in size over seven months, with no associated history of asbestos exposure. On initial ultrasound, he had a benign appearing hydrocele and proceeded to hydrocelectomy, where suspicious lesions were noticed and sent for histopathology. A subsequent radical inguinal orchiectomy was performed. Histopathologic examination reveale

Breast Plasmacytoma as the Initial Manifestation of Multiple Myeloma in a 36-Year-Old Woman.

Extramedullary plasmacytomas (EMPs) of the breast are extremely rare and may present as the initial manifestation of multiple myeloma (MM). We report the case of a 36-year-old woman who presented with a rapidly growing right breast mass. Mammography and ultrasound revealed an oval, circumscribed, heterogeneous, vascular mass, measuring up to 4.2 cm, categorized as Breast Imaging Reporting and Data System (BI-RADS) 4. Core needle biopsy revealed a plasmablastic/plasmacytic neoplasm with strong CD138 expression, lambda light chain restriction, high Ki-67 index, and negative Epstein-Barr virus ea

Optimal Surgical Approaches for Incidentally Discovered Ruptured Mucinous Appendiceal Neoplasms.

Mucinous neoplasms of the appendix, including low-grade appendiceal mucinous neoplasms (LAMNs), are rare but significant due to their potential for peritoneal dissemination. These tumors are often discovered incidentally during imaging or surgery for unrelated conditions. The presence of mucinous material in the peritoneum raises concern for pseudomyxoma peritonei, necessitating careful intraoperative and postoperative management. A 52-year-old male presented with testicular pain, leading to a diagnosis of seminoma. Staging computerized tomography (CT) revealed a dilated appendix with a surrou

A rectal gastrointestinal stromal tumor case accompanied by elevated CEA mimicking rectal carcinoma on 18F-FDG PET/CT.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasm arising from gastrointestinal tract and can be benign or malignant. Rectal GISTs are rare and have poor prognosis. We here reported an older male who presented with features of distending discomfort in the rectum and pain in the anus due to a large rectal tumor. Physical examinations detected a mass in the rectum without blood staining on the gloved finger. Carcinoembryonic antigen (CEA) was found to be slightly elevated and the prostate-specific antigen level was normal. 18F-FDG PET/CT showed a soft tissue densit

Colon adenocarcinoma associated with intestinal sarcoidosis.

Sarcoidosis is a multisystem chronic inflammatory disease of unknown etiology, characterized by non-caseating granulomas. Sarcoidosis has the potential to involve every tissue in the body, which mainly affect the lymphatic system and lungs; gastrointestinal system, and particularly the colon, is an extremely rare location. We report the case of a 64-year-old male with history of pulmonary and cutaneous sarcoidosis diagnosed with neoplasm in the hepatic flexure of the colon and a polyp with high-grade dysplasia in the transverse colon by colonoscopy after a positive fecal occult blood test. Anorectal Mucosal Melanoma: A Case Report and Literature Review.

BACKGROUND Anorectal mucosal melanoma (AMM) is a rare and aggressive neoplasm, with a 5-year survival rate of 10%. Due to its rarity and nonspecific symptoms, the diagnosis is often made late. Surgical resection remains the criterion standard for treatment of anorectal melanoma. CASE REPORT We present the case of an 81-year-old woman presenting with hematochezia, anal secretion, tenesmus, difficulty in defecation, and perianal pain. On physical examination, there was a prolapse of a 5-cm melanocytic nodule in the anal canal, hard on palpation. Biopsy confirmed anorectal melanoma. Staging revea

How I Diagnose Primary Mediastinal (Thymic) Large B-Cell Lymphoma.

OBJECTIVES: Primary mediastinal (thymic) large B-cell lymphoma (PMBL) is an uncommon large B-cell neoplasm recognized by the World Health Organization as a distinct entity on the basis of its unique clinical features, histogenesis, phenotype, and pathogenetic mechanisms. The diagnosis of PMBL can be challenging because of features that may overlap with other (Hodgkin and non-Hodgkin) lymphoma types. This review describes our approach to the diagnosis of PMBL. METHODS: Two cases are presented to illustrate how we diagnose PMBL and separate PMBL from related histologic and biological mimickers,

Impact of retroperitoneal lymph node dissection in ovarian cancer - time for paradigm shift?

Ovarian cancer is the second most common malignant neoplasm of the genital tract. Staging procedures for ovarian cancer include longitudinal laparotomy, hysterectomy, bilateral salpingo-oophorectomy and infragastric omentectomy as well as systematic pelvic and para-aortic lymphadenectomy. In general, the goal of this primary procedure is to achieve the maximum cytoreduction of all abdominal diseases; a residual disease <1 cm defines optimal cytoreduction, however, a maximal effort should be made to remove all gross disease as this offers superior survival outcomes. The role of lymphadenectomy
