Hybrid computed tomography and magnetic resonance imaging 3D printed models for neurosurgery planning.

In the last decade, the clinical applications of three-dimensional (3D) printed models, in the neurosurgery field among others, have expande … hybrid computed tomography (CT) and magnetic resonance imaging (MRI) 3D printed models. The need of neurosurgeons for a better understanding of the complex anatomy of central nervous system (CNS) and spine is pushing the use of these hybrid models, which are able to combine morphological information from CT and MRI, and also able to add physiological data from …

Neurosurgical trauma in Japan.

According to the statistics compiled by the Institute for Traffic Accident Research and Data Analysis, the total number of head trauma patients has stayed virtually the same for the last 10 years in Japan, although a fraction of people suffering minor head trauma has shown a trend to increase. The Japan Society of Neurotraumatology is in the process of establishing a guideline for head trauma management. No major difference is noted in surgical procedures selected by neurosurgeons in Japan as compared to those in other countries. It appears, however, that the ventriculostomy may be less freque…

Impact of the complexity of surgical procedures and intraoperative interruptions on neurosurgical team workload.

BACKGROUND: Neurosurgical teams are exposed to various stressors: complexity of surgical procedures, environment, time pressure and interruptions contribute to increasing the perceived workload. OBJECTIVE: This study aimed to evaluate the impact of interruptions and surgical complexity on neurosurgical team workload. METHODS: A prospective observational study was conducted on thirty surgical procedures of graduated complexity recorded in our Department of Neurosurgery. A scale was created and used by neurosurgeons to evaluate the perceived complexity of the surgical procedure. Interruptions …

Application of 3-Dimensional Printing in a Case of Osteogenesis Imperfecta for Patient Education, Anatomic Understanding, Preoperative Planning, and Intraoperative Evaluation.

BACKGROUND: The techniques and applications of 3-dimensional (3D) printing have progressed at a fast pace. In the last 10 years, there has been significant progress in applying this technology to medical applications. We present a case of osteogenesis imperfecta in which treatment was aided by prospectively using patient-specific, anatomically accurate 3D prints of the calvaria. The patient-specific, anatomically accurate 3D prints were used in the clinic and in the operating room to augment patient education, improve surgical decision making, and enhance preoperative planning. Intraregional differences of perioperative management strategy for lumbar disc herniation: is the Devil really in the details?

BACKGROUND: This study intends to evaluate whether regional common habits or differences in case-volume between surgeons are significative variables in the perioperative management of patients undergoing surgery for lumbar disc herniation. METHODS: An e-mail survey was sent to all neurosurgeons working in Lombardy, Italy's most populated region. The survey consisted of 17 questions about the perioperative management of lumbar disc herniation. RESULTS: Forty-seven percent (47%) out of 206 Lombard neurosurgeons answered the survey. Although in some respects there is clear evidence in current l…

Multiple intraoperative monitoring-assisted microneurosurgical treatment for anterior circulation cerebral aneurysm.

This study investigated the efficacy of multiple intraoperative monitoring techniques including indocyanine green angiography (ICGA), somatosensory evoked potential (SSEP) and motor evoked potential (MEP) in the clinical outcome of microneurosurgical treatment for anterior circulation cerebral aneurysm. Fifty-two anterior circulation cerebral aneurysms (Hunt and Hess [H&H] grades 0, 1 or 2) from 45 Chinese in-patients were completely clipped. In one patient, ICGA directed neurosurgeons to readjust aneurysmal clips in order to eliminate a residual aneurysm and restore patency of a branching art…

[Surgical treatment of glial tumors of the frontal lobe. Current state of the problem. Part I: motor functional system].

In everyday clinical practice, neurosurgeons usually deal with glial tumors localized precisely in the frontal lobe. There is a widespread opinion that almost any neurosurgeon can resect glioma of the frontal lobe regardless of experience and microsurgical skills. However, working at the federal center, we observe how employees often have to operate on patients with frontal lobe gliomas after previous surgery in other hospitals. Therefore, treatment of these patients seems to be quite difficult problem of neurosurgery for glial tumors, and incidence of postoperative complications is still high…

Neuro-ophthalmologic diagnosis and therapy of central nervous system trauma.

Careful examination and judicious use of neuroimaging will lead to the initiation of appropriate management for injuries of the afferent and efferent visual systems. This article concentrates on those aspects of neuro-ophthalmologic trauma that are most relevant to the neurosurgeon, particularly those disorders that are commonly faced when caring for acutely injured patients. Although direct injury to the orbit and globes can easily result in neuro-ophthalmologic disorders, it is less appreciated that distant head injury can similarly result in injury to the retrobulbar afferent visual pathway

The orbital surgeon.

While the number of orbital surgeons is limited, it is hoped these can be recognized and patients referred to them by ophthalmologists not interested or trained in that specialty. Let the orbital surgeon determine whether he can handle the problem in 1 to 2 days, or whether a neurosurgeon should do the procedure or make it a joint effort. It may well involve other specialty team effort approaches. It is essential to have an understanding of x-rays, CT, angiography, and MRI techniques and films. Sit with these specialists to learn more and help to avoid negative, misdiagnosis reports in the int

Thin-slice T2 MRI imaging predicts vascular pathology in hemifacial spasm: a case-control study.

Hemifacial spasm (HFS) is a condition that may severely reduce patients' quality of life. We sought to determine the sensitivity and specificity of thin-slice T2 magnetic resonance imaging (MRI) for detecting vascular compression in HFS patients. Prospective information was collected on 28 patients with HFS who presented to our center between March 2011 and March 2012 with thin-slice T2 MR imaging. The sensitivity and specificity for differentiating patients from controls were calculated. Sensitivities were 78.6% and 92.9% for the blinded radiologists and 75% for the partially blinded neurosur

[Follow-up of adult patients treated during childhood for hydrocephalus].

The hydrocephalic patient is at risk throughout life of developing complications that may be severe or even fatal. The neurological, developmental, social and occupational outcome is affected by the sequelae of the initial disease and the consequences of intracranial hypertension, but also by imaginary obstacles. Unless proved otherwise, the patient with a shunt must be considered shunt-dependent; shunt independence is rare and must be proved following a rigorous protocol. The hydrocephalic patient should therefore be followed regularly and for life in neurosurgery to screen for and prevent co

Regional differences in reimbursement rates from Medicare, Medicaid, and FAIR Health across common procedures for neurological surgeons.

OBJECTIVE: FAIR Health-a nonprofit, state-funded database-was created as an independent repository of healthcare claims paid data to address allegations of price fixing. Many insurers have forced physicians to negotiate payments based on Medicare rates, rather than utilizing FAIR Health. The authors' objective was to provide an overview of regional differences in reimbursement rates per several sample neurosurgical Current Procedural Terminology (CPT) codes and to compare Medicare, Medicaid, and usual, customary, and reasonable rates via FAIR Health rate estimates. METHODS: The authors compar

Assessment of a 3D printed simulator of a lateral ventricular puncture in interns' surgical training.

PURPOSE: In this study, a simulator for training lateral ventricular puncture (LVP) was developed using three-dimensional (3D) printing technology, and its function of improving the skills of LVP in young interns was evaluated. METHODS: A virtual 3D craniocerebral simulator of a 51-year-old female patient with hydrocephalus was reconstructed with 3D printing technology. The anatomical and practical validity were assessed by all interns on a 13-item Likert scale. The usefulness of this simulator was evaluated once a week by two neurosurgeons, based on the performance of the interns, using the …

Staged Cranial Surgery for Intracranial Lesions: Historical Perspective.

We follow the development of staged resection from its first description by Walter E. Dandy, one of the founding fathers of neurosurgery, in 1925 in which he removed a large vestibular schwannoma. This historical vignette cites neurosurgical case reports and literature to demonstrate the evolution of staged resection of intracranial lesions, from Dandy's initial use to its becoming a more viable and safe option for the treatment of meningiomas, vestibular schwannomas, and skull base lesions (among numerous other intracranial pathologies). We also discuss the current advancements and future pers
