[Neuroradiology and the chiasmatic angle syndrom (author's transl)].

Many diagnostic procedures are available. The main methods are briefly reviewed. Standard X-rays of the skull are always needed. In doubtful cases polytomography is essential. Bone scintigraphy reveals early signs of changes in the bony macrostructures. Brain scintigraphy is still very useful in observing 1. the blood-flow in both intracranial and intraorbital regions, and 2. the cranio-orbital distribution of the radiotracer. Brain scintigraphy is particularly important in detecting meningiomas and vascular lesions. The dynamic study and the comprehensive survey it gives are still not replace

[Complications after hip operations].

From 1999 to 2008, according to a prospective and sequential compilation of data, the 12,590 hip operations (11,059 in adults and 1,531 in children) performed in our hospital showed a postoperative complication rate of 4.96% (5.2% in adults and 1.96% in children). The most frequent complications were 110 thromboses (0.87%), which were clinically apparent and proven by Doppler ultrasonography or phlebography; 191 superficial and deep disorders of wound healing (1.5%), including 57 (0.45%) infections; 88 peripheral nerve lesions (0.7%); and 53 hematoma revisions (0.42%). Operation-specific stati…

Deep venous thrombosis of the lower limb: no difference in duration of symptoms between proximal and distal disease.

In deep vein thrombosis (DVT) of the lower limb, the location of the thrombosis determines how great the risk of pulmonary embolism is. We set out to determine whether the duration of lower limb DVT symptoms could be a predictive factor for the location of the thrombosis. We retrospectively collected a series of 100 lower limb phlebography examinations that were positive for DVT and divided the group into 2 subgroups proximal disease (ie, above the knee) or distal disease (ie, below the knee). The subgroups were then plotted against the individual duration of the symptoms. Of the 100 DVTs, 58%…

[Thrombosis of the right atrium disclosed by severe pulmonary embolism].

The case of a thrombosis of the right auricle, revealed by severe pulmonary embolism in a 56-year-old patient without significant medical history is reported. He was admitted to the ICU with a haemorrhagic cerebrovascular accident. The course was characterized by the occurrence of a venous thrombosis of the right lower limb confirmed by phlebography. The treatment with a platelet antiaggregating agent was effective and the patient left hospital two weeks later. He was re-admitted after 2 months with a clinical picture of massive embolism including polypnea, tachycardia and haemoptysis. The dia…

[Radiological exploration of hematuria].

Hematuria is defined as the presence of blood in urine. Its site of origin can be in any point of the urinary tract. Its discovery leads to … e performed only according to the findings of IVP and cystoscopy. Arteriography combined with renal phlebography is indicated if the assessment is negative in case of recurrent hematuria or of massive hematuria. It is mainly aimed at detecting a vascular malformation. When this exploration is completed, an etiology is found is about 90% of all cases.

Popliteal valve incompetence and postoperative deep vein thrombosis.

In 79 patients undergoing hip or knee replacement surgery the preoperative condition of the popliteal vein valves was assessed by Doppler ultrasonography. Presence of postoperative deep vein thrombosis was determined by bilateral ascending phlebography seven to ten days after surgery. Antithrombotic prophylaxis consisted solely of TED stockings and early mobilization. The overall incidence of deep vein thrombosis was 20%. All thrombi were calf vein thrombi. One of these thrombi extended into the popliteal region and two into the femoral region. The incidence of deep vein thrombosis in patients…

Treatment of major venous obstruction with an expandable endoluminal spiral prosthesis.

Six patients with extensive iliofemoral and/or caval thrombosis were treated by thrombectomy and subsequent implantation of an expandable en … luminally reconstructed veins remained open at early and late (up to 12 months) review confirmed by phlebography. We conclude that with increasing clinical experience endoluminal relining of obstructed major veins will probably become a valuable method of venous reconstruction with minimal surgical trauma.

[Acute phlebographic diagnosis of thrombotic complications in gynecology and obstetrics].

From 1971 to April 1986 phlebography was performed in a total of 459 patients suspected clinically having an acute phlebothrombosis. From 112 female patients (24.4%) coming from the departments of gynaecology and obstetrics in 94 cases an acute or subacute phlebothrombotic process was proved phlebographically. The main age of these female patients was 29 +/- 11 years (17 to 76 years). A combined ileo-femoral-crural vein thrombosis in 44.7% was the most frequent localization and expansion, however, an isolated thrombotic process of pelvic veins was found in 25.6% only. A coincidence of the calc…

Infectious complications of percutaneously inserted central venous catheters.

The frequency of early and late complications of percutaneously inserted central venous catheters (CVC) was prospectively assessed. One hundred and fifty-one periods of catheterization (siliconized and heparinized polyethylene and teflon catheters), inserted mainly infraclavicularly and on the right side, were evaluated. Strict aseptic technique was used during insertion and daily care of the CVC. The only early complications noted were three cases of pneumothorax. No nerve injuries or arterial punctures were observed. Of 124 evaluated cases (96 arm-phlebography, 28 post-mortem) 16% developed …

[Myelopathies in impairment of extravertebral venous blood circulation].

Development of myelopathies of venous genesis is based on venous hypertension inside vertebral canal which was initially described by J. Aboulker. According to anatomical and functional features of epidural venous system, two factors contribute in development of venous congestion: decreased outflow and increased inflow. Clinical manifestation is presented by spastic movement disorders. Morphological study performed in 18 cadavers allowed to discover so called 'narrow areas' which cause impaired circulation through large feeders of caval veins, which can be discovered by selective phlebography.…

Portal vein thrombosis: imaging features and associated etiologies.

Thrombosis of the portal vein and its splanchnic tributaries is often unsuspected clinically and may be recognized only after imaging studies of the abdomen are performed for other reasons. Radiologists should be aware of the clinical situations that predispose a patient to portal or mesenteric vein thrombosis and should also be able to recognize the sequelae of chronic thrombosis. Different modalities can be used to image the patient with portal vein thrombosis; each has its strengths and drawbacks. This paper discusses the conditions that predispose to portal and mesenteric vein thrombosis,

Lymphatic and venous examination of the ulcerated leg: a preliminary report.

BACKGROUND: It has been widely accepted that deep venous thrombosis and venous incompetence initiate a series of events resulting in various ulcerations. The role of the lymphatic system in the progression of these events has not been extensively studied. METHODS: Twenty-three patients with nonarterial ulcerations of the legs were examined by phlebography and lymphangiography. All had recurrent ulcerations associated with nonpitting edema, brawny induration, and dermatitis characteristic of the postphlebitic syndrome. Arterial circulation was normal in all patients. RESULTS: Phlebographic ab…

Normal limits of the lateral cerebral ventricles on the lateral phlebogram: a statistical analysis.

The highest points of the subependymal veins of the lateral cerebral ventricles (frontal horn, body and atrium) and the height of the body have been evaluated in selected patients. Through a statistical analysis, they have been demonstrated to depend on the height of the supratentorial compartment of the skull. Taking into account this relation, the results obtained furnish reliable criteria for evaluating ventricular enlargement or sectional deformity.

Cauda Equina Syndrome Caused by Epidural Venous Plexus Engorgement in a Patient with May-Thurner Syndrome.

The purpose of this article is to present a case of cauda equina syndrome in a patient with incomplete motor and sensory deficits due to epidural venous plexus engorgement, owing to May-Thurner syndrome successfully treated with venous iliac stenting. A 40-year-old woman, with previous history of deep vein thrombosis and miscarriages, gradually developed right leg and back pain, with functional limitation, perineal hypoesthesia, and sphincter incontinence. Magnetic resonance imaging revealed epidural venous plexus engorgement and cauda equina roots involvement. Phlebography showed perimedullar…

Retrieval of "Recovery IVC Filter" after 383 days.

AIM: Prolonged temporary implantation of retrievable vena cava filter is little discussed in literature. The aim of our article is to presen … special extraction system without significant technical complications in our department. On control phlebography after extraction of cava filter, there were no signs of injury to the wall of inferior vena cava, extravasation or stenosis. CONCLUSION: Removal of "Recovery IVC Filter" after long term temporary implantation is possible and safe.

[Inferior vena cava malformations and deep venous thrombosis].

We carried out a prospective study of 116 patients under 50 years of age who had deep venous thrombosis of the lower extremities to determine whether the presence of congenital anomaly of the inferior vena cava (IVC) was a risk factor for the disease. All patients were investigated by Doppler echography. Some 37 patients who had iliac vein occlusion also underwent phlebography. In 10 patients in whom the IVC was difficult to image, magnetic resonance angiography or computerized axial tomography was carried out. In all patients, studies of antithrombin, protein C and protein S deficiency, facto…

Successful endoluminal thrombo-aspiration of renal graft venous thrombosis.

Renal transplant vein thrombosis is an unusual event occurring in 0.3-3% of renal transplantations. Prognosis is uniformly poor with graft loss in nearly every case. We report here the first three cases of renal graft vein thrombosis successfully treated by percutaneous endoluminal thromboaspiration. After an initially uneventful course all recipients developed anuria and required hemodialysis. In two cases, an ultrasound examination suggested a diagnosis of venous thrombosis. Emergency arteriography and phlebography were performed, confirming the complete thrombosis of the graft veins. Thromb…
