Caslick's, episioplasty, urethroplasty, cervical, and rectovestibular laceration repair are, by preference, performed in the standing patient. Normal castration is performed routinely uneventfully in the standing patient. The duration of the procedures and the ease with which they are accomplished in a standing horse of normal temperament make general anesthesia unnecessary. More invasive procedures, such as removal of urinary bladder calculi in male horses or removal of a neoplastic ovary in a mare, can be accomplished in a standing patient but serious consideration should be given to the use

Non-expandable lung (NEL) commonly occurs secondary to chronic pleural processes, including pleural effusions, endobronchial obstruction, atelectasis, or chronic pleural inflammatory processes. Patients with NEL frequently undergo unnecessary procedures (e.g., thoracentesis), resulting in pneumothorax and discomfort (usually chest pain). Cervical cerclage is currently one of the primary methods of treatment for cervical insufficiency (CI). Identifying methods in order to selectively apply cerclage may allow us to further limit unnecessary procedures. In this regard, a limited number of inflammatory and extracellular matrix biomarkers measurable non-invasively at the level of the cervix have been examined. In this review we summarize the current research on the use of cervical biomarkers in predicting cerclage failure and propose potential objectives for future research.

IgG4-related disease is an emerging clinicopathologic entity. Hypophysitis, diffuse thickening of dura, and enlargement of the trigeminal nerve are well-known intracranial involvements of IgG4-related disease. This report of a case of systemic IgG4-related disease is the first to present neuroimaging of apparent supratentorial meningioma-like lesions and thickening and contrast enhancement of the walls of the intracranial internal carotid arteries. It is important to recognize IgG4-related intracranial pseudotumors so that patients do not undergo unnecessary surgical procedures.

We report on 15 patients (10 boys, 5 girls) with clinical anophthalmos. Two of them had bilateral anophthalmos, 10 had systemic anomalies, and six had abnormalities of their remaining eye. Only two appeared to have an associated underlying aetiology. Fourteen patients underwent orbital reconstruction or socket enlargement with varying degrees of success. Our own experience suggests that unnecessary lid procedures should be avoided, but we recommend early prosthetic fittings. We feel that a multidisciplinary approach is necessary to attain useful rehabilitation.

In the management of patients with molar pregnancy, a repeat uterine curettage is generally advocated after evacuation of the hydatidiform mole. To assess the usefulness of a repeat curettage, we reviewed our experience with this procedure over an 8-year period. We found that it was unnecessary in 90% of the cases and did not predict or influence the outcome in all but one case of invasive mole. We feel that the procedure is not cost-effective and should be reserved for patients with specific indications such as incomplete evacuation and abnormal uterine bleeding.

Various minimally invasive techniques have been reported as an alternative to conventional lumbar decompression. The major advantage of these minimally invasive procedures lies in their reduction of unnecessary exposure and tissue trauma. Our objective was to describe a minimally invasive procedure for lumbar spinal stenosis decompression by enlarging the lumbar interspinous space, approaching it with a tubular retractor, and assisting with microscopy. Elective thyroid and breast procedures are classified as clean surgery, yet prolonged postoperative antibiotic use remains common in many low- and middle-income settings despite guideline recommendations, contributing to unnecessary antimicrobial exposure. We conducted a randomised trial in a public tertiary-care hospital in central India, assigning adults undergoing elective clean thyroid or breast surgery to either three days of postoperative antibiotics or no antibiotics beyond a single pre-incision prophylactic dose. The primary outcome was 30-day surgical site infection.

Positron-emission tomography scans (PET) with fluorine-18-fluorodeoxyglucose ((18)F- FDG) are usually negative in leiomyomas. Two patients underwent a PET that showed an increased (18)F- FDG uptake of the distal oesophagus suggestive for malignancy. Both patients were operated on and histologic examination revealed a benign leiomyoma in both cases. We conclude that oesophageal leiomyomas are a potential cause of a false-positive PET. A high level of caution is needed in these diagnostically challenging cases to prevent unnecessary surgical procedures.

Acne Fulminans (AF) is a severe form of cystic acne primarily affecting Caucasian adolescent males. The systemic clinical manifestations of the disease make it likely that the patient will present to his or her primary care provider rather than to a dermatologist. Promptly recognizing this disorder will aid in appropriate management, avoiding costly and invasive diagnostic procedures, multiple consultations, unnecessary pharmacologic treatments, and residual scarring of the skin. We report a case and a brief review of the literature regarding this enigmatic disorder.

Mediastinal lipomatosis is a benign condition characterized by a large amount of mature adipose tissue within the mediastinum. It widens the mediastinum and may simulate mass lesions, thus leading to diagnostic errors. We describe a new case of a huge mediastinal lipomatosis and review 36 cases previously reported in the literature. Computed tomography has an important role in the diagnosis of this disease. To avoid invasive and unnecessary procedures, this diagnosis should be considered in any patient with Cushing's syndrome.

Reversible posterior leukoencephalopathy (PLE) is a newly recognized syndrome with characteristic radiologic findings. The clinical picture resembles that of hypertensive encephalopathy. PLE is caused by uncontrolled hypertension as well as other neurotoxic conditions. We report on 3 patients with this syndrome. Uterus didelphys with unilateral imperforate vagina is rare. Interestingly, in all reported cases in which the urinary tract was investigated renal agenesis was found on the side of the obstructed hemivagina. We report 2 cases of this unusual and interesting condition, and review the embryogenesis of müllerian duct abnormalities associated with renal agenesis, as well as the clinical findings, diagnosis and management. We stress the importance of prompt and accurate diagnosis, and treatment as a means to avoid unnecessary surgical procedures.

Orbital myiasis cases by dipterous fly larvae are rarely reported. A case of massive orbital myiasis caused by Cochliomyia hominivorax resulting in complete loss of the globe is described. Oral ivermectin was used successfully to eliminate the larvae prior to enucleation, making the mechanical removal of larvae an unnecessary procedure.

A simple procedure of using a Smillie knife to release the lateral patellar retinaculum and the capsule in the unstable patella syndrome is described. The syndrome includes recurrent dislocation, recurrent subluxation and acute dislocation of the patella. One common indication for soft-tissue filler in the nose, or liquid rhinoplasty, is the temporary treatment of secondary nasal deformities after rhinoplasty. Multiple aspects of the patient evaluation, timing with respect to the previous rhinoplasty and planned revision, and procedural principles and steps must be carefully considered.
