A Pragmatic Framework for Shared Decision Making in Older Adults: Cardiac Amyloidosis as a Prototype.

Advanced chronic diseases, or multicomplexity in older adults presents unique challenges. Transthyretin cardiac amyloidosis (ATTR-CA) is one such scenario where heart failure is a common presentation, and management remains challenging. We describe the challenges involved in the diagnosis of ATTR-CA in older adults, which has implications on treatment options. We discuss the application of comprehensive geriatric assessment (CGA) and shared decision making (SDM) in the context of these challenges. Based on geriatric medicine principles, an innovative framework for applying SDM in ATTR-CA patie

A scoping review examining patient experience and what matters to people experiencing homelessness when seeking healthcare.

BACKGROUND: Homelessness is associated with significant health disparities. Conventional health services often fail to address the unique needs and lived experience of homeless individuals and fail to include participatory design when planning health services. This scoping review aimed to examine areas of patient experience that are most frequently reported by people experiencing homelessness when seeking and receiving healthcare, and to identify existing surveys used to measure patient experience for this cohort. METHODS: A scoping review was undertaken reported according to the PRISMA-ScR 2

The opportunity for health plans to improve quality and reduce costs by embracing primary care medical homes.

BACKGROUND: The large and growing costs of healthcare will continue to burden all payers in the nation's healthcare system-not only the states that are struggling to meet Medicaid costs and the federal government, but also the private health plans that serve commercial, Medicare Advantage, and Medicaid beneficiaries. Cost will increasingly become a concern as millions more people become newly insured as a result of the Patient Protection and Affordable Care Act (ACA). Primary care delivery through patient-centered medical homes (PCMHs) and other coordinated-care models have improved care and r

A Comprehensive Approach to Achieving Door to Balloon Metric Sustainability.

Over the past 5 years, our teams have focused on establishing an American College of Cardiology Chest Pain Center-accredited percutaneous coronary intervention program. Our initiatives have concentrated on achieving defect-free care, optimizing standardization of acute coronary syndrome management, and reducing door to balloon (D2B) times. Preserving these initiatives can be challenging for a 220-bed, community-based hospital due to increasing health care demands and resource constraints. However, our commitment to integrating sustainability principles into D2B workflow optimization ensures lo

The Burden and Phenotype of Type 2 Diabetes in the Western Pacific Region.

The International Diabetes Federation estimated that in 2024, 215 million adults-more than one-third of the global diabetes population-were living with diabetes within the Western Pacific Region (WPR), which includes nations with some of the highest prevalence worldwide. Rapid urbanization and economic development have profoundly reshaped lifestyles, which interact with biological predispositions resulting in an earlier onset of type 2 diabetes. These predispositions are not uniform across the heterogeneous populations within the WPR. Asian individuals tend to have a β-cell secretory failure p

The Effects of a Reproductive Health Voucher Program on Out-of-Pocket Family Planning and Safe Motherhood Service Expenses: A Yemeni Study.

Background/Objectives: Using healthcare vouchers mitigates the financial burdens of low-income individuals, therefore enhancing mothers' satisfaction and encouraging service utilization. In Yemen, reducing financial barriers results in marked improvement in reproductive health services utilization for mothers and their newborns. Such financial strain can be addressed through reproductive health vouchers, which reduce out-of-pocket expenses of family planning, pregnancy, birth, postnatal care and neonatal care. This study compares the Safe Motherhood and Family Planning Voucher Program in the L

Patient-reported disability progression outcomes among patients with multiple sclerosis: Results of an outcomes-based agreement.

BackgroundOutcomes-based agreements (OBAs) are agreements between payers and manufacturers in which payment for medications is tied to patient outcomes. These contracts aim to measure the value of prescription medications on predefined clinical indicators in real-world patient populations. OBAs are gaining traction in the United States as the health care industry shifts from volume-based to value-based care. Multiple sclerosis (MS) is an appealing therapeutic area for OBAs because of its prevalence, high cost of medications, and multiple effective therapeutic options.ObjectiveTo describe findi

Mapping of disease-specific Oxford Knee Score onto EQ-5D-5L utility index in knee osteoarthritis.

BACKGROUND: EQ5D is a generic measure of health. It provides a single index value for health status that can be used in the clinical and economic evaluation of healthcare. Oxford Knee Score (OKS) is a joint-specific outcome measure tool designed to assess symptoms and function in osteoarthritis patients after joint replacement surgery. Though widely used, it has the disadvantage of lacking health index value. To fill the gap between functional and generic questionnaires with economic value, we linked generic EQ-5D-5L to the specific OKS to give a single index value for health status in KOA pat

Efficacy of the digital health application ORIKOⓇ for attention-deficit/hyperactivity disorder (ADHD) in adults: Protocol of a randomized controlled trial.

Attention-deficit/hyperactivity disorder (ADHD) often continues into adulthood and affects several million adults in Germany. Many do not receive timely, evidence-based treatment because of long waiting times and limited access to care. Digital health applications offer a low-threshold option to provide guideline-based support. A pilot study of the present app-based program showed encouraging improvements in quality of life and reductions in ADHD symptom severity. This randomized controlled trial (RCT) aims to confirm and extend the pilot findings. The primary objective is to examine whether t

Disrespect and abuse in maternity care: individual consequences of structural violence.

Disrespect and abuse of patients, especially birthing women, does occur in the health sector. This is a violation of women's fundamental human rights and can be viewed as a consequence of women's lives not being valued by larger social, economic and political structures. Here we demonstrate how such disrespect and abuse is enacted at an interpersonal level across the continuum of care in Tanzania. We describe how and why women's exposure to disrespect and abuse should be seen as a symptom of structural violence. Detailed narratives were developed based on interviews and observations of 14 rura
