Nonhuman primate models of HIV reveal reservoir dynamics and promising interventions.

PURPOSE OF REVIEW: HIV cure research seeks to eradicate the virus from people with HIV (PWH) or provide posttreatment control (PTC) of virus sufficient to block new transmissions and provide better health outcomes. Nonhuman primate (NHP) models of HIV have proven instrumental to our understanding HIV infection. This review highlights the remarkable contributions of NHPs in both corroborating data from PWH and expanding our understanding beyond what is possible in a clinical setting by utilizing novel virus models, immune activating therapies, and direct in vivo intervention to clear the viral

[Recent advances in research of cure of HIV infection].

Antiretroviral therapy (ART) is a well-established treatment for HIV infection that suppresses viral replication by inhibiting viral enzymatic activity, thereby preventing progression to immunodeficiency. However, discontinuation of ART typically leads to rapid viral rebound within weeks, due to the reactivation of latent HIV from long-lived reservoirs such as resting CD4+ T cells. Eradication of these latent reservoirs is essential to achieve a cure for HIV. In 2009, the first case of HIV cure was reported: an individual with HIV and acute leukemia received an allogeneic hematopoietic stem ce

Molecular mechanisms and targets underlying immune evasion by human cytomegalovirus.

Human cytomegalovirus (HCMV) establishes lifelong latency in humans largely through the evolution of sophisticated immune evasion strategies that allow it to escape recognition and clearance by the host immune system. While significant progress has been made in elucidating the mechanisms underlying HCMV immune evasion, the precise regulation of host immune responses that enables viral persistence remains incompletely understood. In this review, we provide a comprehensive overview of the current knowledge on HCMV immune evasion, with a particular emphasis on the functions of HCMV-encoded genes.

HIV reservoirs in lymphomagenesis: hidden driver in the era of viral suppression?

SUMMARYDespite advancements in antiretroviral therapy, people living with HIV (PLWH) remain at high risk of lymphoma. The persistence of HIV reservoirs and their spatial association with lymphoma highlights the need to clarify their role in lymphomagenesis. HIV reservoirs, which are established early during infection and maintained through clonal expansion, epigenetic silencing, and immune evasion, may contribute to lymphomagenesis through four interconnected mechanisms: provirus integration effects, viral protein-mediated disturbances, microenvironment dysregulation, and reservoir reactivatio

Assessing the effects of antiretroviral therapy-latency-reversing agent combination therapy on eradicating replication-competent HIV provirus in a Jurkat cell culture model.

Eradication of HIV-1 latently infected cells is an important issue in HIV treatment. However, there are limited models available to assess therapeutic efficacy in vitro. Here, we present a protocol for establishing a variety of HIV-infected Jurkat cells, including productive and latent status, evaluating the efficacy of antiviral agents, followed by PCR/sequencing-based detection of replication competent HIV provirus. This protocol is useful for optimization of treatment of HIV-1 and provides insights into the mechanisms of clonal selection of heterogeneous HIV-1-infected cells. For complete d

Immunotherapeutic approaches to HIV cure and remission.

PURPOSE OF REVIEW: Despite improvements in the effectiveness of antiretroviral therapy (ART), there are still unmet needs for people living with HIV which drive the search for a cure for HIV infection. The goal of this review is to discuss the challenges and recent immunotherapeutic advances towards developing a safe, effective and durable cure strategy for HIV. RECENT FINDINGS: In recent years, advances have been made in uncovering the mechanisms of persistence of latent HIV and in developing more accurate assays to measure the intact proviral reservoir. Broadly neutralising antibodies and m

Pharmacokinetic/pharmacodynamic analysis of romidepsin used as an HIV latency reversing agent.

OBJECTIVES: To develop a population pharmacokinetic model for romidepsin given as an HIV latency reversing agent (LRA) and to explore the relationship between romidepsin exposure and its in vivo effects on viral gene expression and antiviral immunity. METHODS: A population pharmacokinetic analysis was performed in 15 HIV-1-infected patients who received three weekly infusions of romidepsin (5 mg/m2) within the BCN02 clinical trial. A full pharmacokinetic profile was obtained for each participant at the first dose, and additional samples thereafter. A population pharmacokinetic model was devel

In Vivo Models of Human Immunodeficiency Virus Persistence and Cure Strategies.

Current HIV therapy is not curative regardless of how soon after infection it is initiated or how long it is administered, and therapy interruption almost invariably results in robust viral rebound. Human immunodeficiency virus persistence is therefore the major obstacle to a cure for AIDS. The testing and implementation of novel yet unproven approaches to HIV eradication that could compromise the health status of HIV-infected individuals might not be ethically warranted. Therefore, adequate in vitro and in vivo evidence of efficacy is needed to facilitate the clinical implementation of promis

Strategies to target HIV-1 in the central nervous system.

PURPOSE OF REVIEW: To review current knowledge of viral reservoirs in the central nervous system (CNS) and identify the CNS-specific barriers and strategies to cure human immunodeficiency virus type 1 (HIV-1) within the brain. RECENT FINDINGS: The cumulative data of HIV-1 infection of the CNS support the ability of the CNS to act as a viral reservoir for HIV-1. The HIV-1 viral strains found in the CNS are distinct to those found in other parts of the body. These differences have been well documented for env and also extend to the viral promoter, the long terminal repeat, and influence the abi

HIV-1 latency: an update of molecular mechanisms and therapeutic strategies.

The major obstacle towards HIV-1 eradication is the life-long persistence of the virus in reservoirs of latently infected cells. In these cells the proviral DNA is integrated in the host's genome but it does not actively replicate, becoming invisible to the host immune system and unaffected by existing antiviral drugs. Rebound of viremia and recovery of systemic infection that follows interruption of therapy, necessitates life-long treatments with problems of compliance, toxicity, and untenable costs, especially in developing countries where the infection hits worst. Extensive research efforts

Current issues in global tuberculosis control.

Despite attempts to standardize tuberculosis (TB) control strategies, there remains wide variation in the selection and implementation of control strategies within and among nations. Some of this variation is appropriate; based on wide variations in the available resources, the prevalence of TB infection, the incidence of TB disease, the relative contribution of reactivation versus recent transmission to incident cases, and the rate of HIV co-infection. This review will discuss three controversial questions relevant to global TB control: (1) What is the role of the treatment of latent TB infec

Molecular pathogenesis of Theiler's murine encephalomyelitis virus-induced demyelinating disease in mice.

After an acute phase of virus growth in neurons (e.g. anterior horn cells), Theiler's murine encephalomyelitis virus (TMEV) persists as a chronic productive infection, largely in macrophages in the CNS white matter. TMEV replication in macrophages is highly restricted, probably as the result of host cell factors. The preponderance of evidence indicates that TMEV persistence leads to immunopathologic damage of myelin, mediated by major histocompatibility class II-restricted Th1 lymphocytes directed at a virus epitope(s) rather than host neuroantigens at least early in the infection. Analysis of

Pathogenesis of varicelloviruses in primates.

Varicelloviruses in primates comprise the prototypic human varicella-zoster virus (VZV) and its non-human primate homologue, simian varicella virus (SVV). Both viruses cause varicella as a primary infection, establish latency in ganglionic neurons and reactivate later in life to cause herpes zoster in their respective hosts. VZV is endemic worldwide and, although varicella is usually a benign disease in childhood, VZV reactivation is a significant cause of neurological disease in the elderly and in immunocompromised individuals. The pathogenesis of VZV infection remains ill-defined, mostly due

Early-life gammaherpesvirus infection results in an expanded long-term latent reservoir.

Gammaherpesviruses are ubiquitous oncogenic pathogens that establish lifelong latency, are associated with multiple types of malignancies, and are increasingly believed to play a role in autoimmune disease. Human Epstein-Barr virus (EBV) and Kaposi sarcoma-associated herpesvirus (KSHV) frequently infect across various stages of childhood development. Nevertheless, how infection during the earliest stages of life, when immune defenses are still developing, shapes the risk of malignant disease and autoimmunity remains poorly understood. Here, we characterized acute and chronic infection followin

HIV Reservoirs in Brain parenchyma and Cerebrospinal Fluid During Suppressive ART: Same-Same or Very Different?

PURPOSE OF REVIEW: The central nervous system (CNS) remains a relatively understudied reservoir of HIV that may have significant implications for HIV cure and the pathogenesis of cognitive disorders that persist despite viral suppression with antiretroviral therapy (ART). This review will describe our current understanding of the nature, size and composition of the CNS reservoir and the possible contribution of viral persistence in the CNS on viral replication, neuroinflammation and cognitive disease. RECENT FINDINGS: Reservoirs of intact and defective HIV proviral DNA have recently been dete

Latent, Lytic, and Linked to Multiple Sclerosis-How EBV Drives Autoimmunity.

Epstein-Barr virus (EBV) is a human tumor virus best known for its B cell-transforming capacity and association with lymphomas and carcinomas. Epidemiological studies have suggested that its infection, in addition, is necessary for the development of the autoimmune disease multiple sclerosis (MS). The very same oncogenes that drive EBV-associated malignancies might also induce differentiation of B cell subsets that initiate neuroinflammation. This review will discuss how insufficient immune control might allow for sizeable populations of T-bet+CXCR3+ B cells to infiltrate the central nervous s

Applications and limitations of the passenger hypothesis for HIV reservoir persistence and cure.

Antiretroviral therapy (ART) suppresses HIV replication in people living with HIV (PWH), but a persistent population of reservoir cells prevents cure. Reservoir cells are mostly anatomically dispersed, latently infected CD4+ T cells harboring one copy of chromosomally integrated, replication-competent HIV proviral DNA. Despite their low frequency (0.01%-0.1%) among CD4+ T cells and the quiescence of most genetically intact proviruses, viremia usually recurs within weeks after ART cessation. When PWH are not on ART, the reservoir is sustained through viral infection and infected cell proliferat
