Coverage strategies in exposed implants.

Implants are widely used and are not without complications. The most serious complication is infection with consequent exposure of the implant and is characterized by increased morbidity and mortality. Until recently, infected implants were removed and replaced with another one. These operations are accompanied by increased morbidity and might result in permanent disability. Coverage of the implant with a flap is an alternative solution. The objective of this article is to study the effectiveness of muscle or myocutaneous flaps in covering infected and exposed implants. A series of 17 patients

Fungal infection of implantable cardioverter-defibrillators: case series of five patients managed over 22 years.

BACKGROUND: With the increasing use of implantable cardioverter-defibrillators (ICDs), device complications are becoming more common. Fungal-related ICD infections have rarely been reported, and little is known about the presentation, prevalence, and treatment options for these morbid infections. OBJECTIVES: The purpose of this study was to characterize the clinical features, treatment, and outcomes of patients with fungal ICD infections. METHODS: We performed a retrospective review of ICD procedures performed at a single academic center and identified all ICD-related infections managed betw

Transvenous Lead Extraction Outcomes in Hemodialysis Patients: Markers for Increased Mortality.

BACKGROUND: The increasing use of cardiac implantable electronic devices (CIEDs) has led to a progressive increase in transvenous lead extraction (TLE) procedures. Due to immune compromise and systemic dysfunction, hemodialysis (HD) patients face higher risks of device-related infection and need for TLE. OBJECTIVES: This study evaluates TLE outcomes in dialysis patients and identifies predictors of 30-day all-cause mortality (ACM). Subgroup analysis focuses on dialysis patients undergoing TLE for infectious indications to determine predictors of adverse outcomes. METHODS: A retrospective rev

Association of frailty with outcomes of patients undergoing transvenous lead removal for cardiac implantable electronic device infections in the United States.

BACKGROUND: Frailty, marked by reduced physiological resilience, is associated with adverse outcomes in patients undergoing various cardiovascular procedures. The rising incidence of cardiac implantable electronic device (CIED) infections necessitate transvenous lead removal (TLR), yet the impact of frailty on TLR outcomes remains unclear. OBJECTIVE: The purpose of this study was to evaluate the association between frailty and outcomes in patients undergoing TLR for CIED infections. METHODS: This retrospective study analyzed the National Readmissions Database (2016-2021) for patients hospita

Performance and potential utility of the BioFire Joint Infection Panel with synovial fluid and joint tissue specimens.

The BioFire Joint Infection Panel (BJIP) (bioMérieux) is a rapid sample-to-answer multiplex polymerase chain reaction (PCR) platform for the diagnosis of joint infections. This retrospective study evaluated its performance with synovial fluid and joint tissue specimens from native and prosthetic joints. Joint fluid and tissue specimens received from November 2023 to September 2024 were included. Some specimens were pooled prior to BJIP testing. BJIP results were compared to composite standard laboratory testing (culture, 16S rRNA gene PCR and sequencing [where performed], and concurrent microb

Two-staged time-dependent materials for the prevention of implant-related infections.

Infection is a main cause of implant failure. Early implant-related infections often occur in the first 4 weeks post-operation. Inhibiting bacterial adhesion and biofilm formation at the early stage and promoting subsequent implant osseointegration are important for implant success. Our previous studies demonstrated that dimethylaminododecyl methacrylate (DMADDM) provided dental materials with antibacterial effects. In the present study, DMADDM and hydroxyapatite (HA) are loaded on to the titanium (Ti) surface via poly dopamine (PDA) self-polymerization. This local DMADDM-delivery Ti is referr

Synovial C-reactive protein features high negative predictive value but is not useful as a single diagnostic parameter in suspected periprosthetic joint infection (PJI).

OBJECTIVES: Synovial fluid C-reactive protein (syCRP) has been recently described as a new biomarker in preoperative diagnostics to identify periprosthetic joint infections (PJI). The aim of this study was to evaluate syCRP in a large cohort of patients with suspected PJI and to calculate the optimal cut-off to diagnose PJI. METHODS: Between September 2015 and June 2017, we prospectively included patients with suspected PJI, in which syCRP was additionally measured along with routine preoperative diagnostic serum and synovial biomarkers. We analysed the sensitivity and specificity of syCRP us

The Impact of Supplemental Intraoperative Air Decontamination on the Outcome of Total Joint Arthroplasty: A Pilot Analysis.

BACKGROUND: During the early era of arthroplasty, the concept of ultraclean operating room (OR) was introduced based on the principle that the number of airborne particles in the OR directly influences incidence of device-related infections. The hypothesis of this pilot study was that use of an innovative UV-C air decontamination technology would lead to a reduction in the incidence of periprosthetic joint infection (PJI) following total joint arthroplasty. METHODS: A retrospective, observational, surveillance study was conducted with a consecutive series of patients who underwent total joint

Systemic Absorption of Antibiotics From Antibiotic-Loaded Cement Spacers for the Treatment of Periprosthetic Joint Infection.

BACKGROUND: Two-stage treatment of periprosthetic joint infections involves placement of high-dose antibiotic-loaded cement spacers (ACSs). Reports of ACS-induced nephrotoxicity have raised concern regarding systemic absorption of antibiotics after ACS placement. We sought to characterize the serum concentrations of antibiotics that occur after ACS placement. METHODS: We performed a prospective study of patients with an infected primary total hip (THA) or knee arthroplasty (TKA) treated with standardized ACSs with vancomycin, gentamicin, and tobramycin. Serum antibiotic levels were collected

Comparison of mortality in women versus men with infections involving cardiovascular implantable electronic device.

Device infection is a complication of implantable cardioverter-defibrillator (ICD) therapy that significantly increases mortality. Risk factors associated with death and ICD infection are poorly understood. The purpose of this study was to identify patient characteristics associated with death after cardiovascular implantable electronic device (CIED) infection. This is a retrospective cohort study of 64,903 Medicare fee-for-service patients who received an ICD in 2007, including 1,855 with device infection. Long-term survival was significantly reduced with CIED infection (71.6% vs 85.0%, p <0.

Impact of prior aspirin therapy on clinical manifestations of cardiovascular implantable electronic device infections.

AIMS: Cardiovascular implantable electronic device (CIED) infection may present as pocket infection or as infective endocarditis (CIED-IE) with vegetation on device leads or heart valves. As aspirin has both anti-inflammatory properties and interferes with platelet aggregation, we hypothesized that ongoing anti-platelet therapy with aspirin may impact clinical and echocardiographic manifestations of CIED infection. METHODS AND RESULTS: We retrospectively reviewed 415 cases of CIED infection admitted to Mayo Clinic Rochester from 1991 to 2008. Information regarding aspirin use was available in
