Reconstruction of proper digital nerve defects in the thumb using a pedicle nerve graft.

BACKGROUND: Traumatic defects of the proper digital nerve in the thumb affect tactile perception of the thumb pulp. This article reports on the treatment of the defect using a pedicle nerve graft taken from the dorsal branch of the proper digital nerve of the index or long finger, or both. METHODS: From May of 2006 to March of 2010, the pedicle nerve graft was used in 16 thumbs in 16 patients. There were 13 male and three female patients, with an average age of 33 years. Nerve repair was performed on one side in six thumbs and on both sides in 10 thumbs. The average length of the defects was

Incidence and clinical significance of zygomaticomaxillary complex fracture involving the temporomandibular joint with emphasis on trismus.

Trismus is frequently a sequel of temporomandibular joint (TMJ) involvement in a zygomaticomaxillary complex (ZMC) fracture. Although trismus is commonly observed in patients with ZMC fracture, continuous follow-up examinations of their degree of mouth opening have rarely been documented. The aim of this retrospective study was to determine the incidence and clinical significance of ZMC fracture involving the glenoid fossa or articular eminence of the TMJ with an emphasis on trismus. The medical and computed tomography (CT) imaging data of 28 patients with ZMC fracture treated by oral and maxi

Anti-CCP and RF IgM: predictors of impaired endothelial function in rheumatoid arthritis patients.

OBJECTIVE: To determine whether the presence of anti-cyclic citrullinated peptide (anti-CCP) antibodies and rheumatoid factor immunoglobulin M (RF IgM) is associated with endothelial dysfunction in patients with rheumatoid arthritis (RA). METHODS: We studied the presence of anti-CCP antibodies and RF IgM and endothelial function in terms of the reactive hyperaemic index (RHI) in 53 consecutive RA patients. Endothelial function was measured by using a finger plethysmograph. RESULTS: RHI was significantly lower in anti-CCP-positive RA patients (n = 33, RHI = 1.78, SD = 0.30) than in anti-CCP-n

[Nail recession emergency treatment: a new technique for the repair of fingertip amputations].

INTRODUCTION: In spite of the recent advances made in microsurgery, the reconstruction of oblique fingertip amputations remains problematical. In cases where reimplantation is technically impossible, the surgeon can utilize a number of different flaps to preserve digital length. METHODS: In certain cases, instead of local flap repair using an advancement flap, the nail complex can be recessed so that after bone shortening of the distal phalanx the free edge of the nail can be stitched to the skin without resulting tension. Two longitudinal incisions are made, and a flap including the nail com

Comparison of discomfort associated with surface and percutaneous intramuscular electrical stimulation for persons with chronic hemiplegia.

Neuromuscular stimulation may facilitate motor recovery after stroke or brain injury, reduce shoulder pain associated with hemiplegia, and reduce cerebral spasticity. However, the discomfort of surface neuromuscular stimulation significantly limits the clinical implementation of this modality for persons with hemiplegia. The study contained herein tests the hypothesis that stroke and brain injury survivors with chronic hemiplegia (>6 mo) and intact sensation tolerate percutaneous intramuscular stimulation better than surface stimulation. Four stroke and two traumatic brain injury survivors par

Us and them: Anticipated imitation between groups.

People's propensity to imitate others' actions is well documented. However, research on imitation remains mainly focused on dyadic social interactions, disregarding that we often interact with others as parts of larger groups. Extending sparse research on imitation beyond dyadic interactions, the current study investigated how anticipated imitation effects become modulated in social interactions between groups. Participants performed an anticipated imitation task in a virtual environment depicting four animated hands that were controlled by the participant, a virtual co-actor, and two virtual

Automated Hand Flexor Tendon-Thickness Measurement in Systemic Sclerosis.

OBJECTIVE: Systemic sclerosis (SSc) can affect flexor tendons, contributing to hand function problems and reduced quality of life. Tendon changes are currently assessed with ultrasonography and measured manually, a time-consuming process prone to interobserver variability. This study aimed to develop and evaluate an automated method for standardized measurement of finger flexor tendon thickness on longitudinal ultrasonography images in patients with SSc. METHODS: In this proof-of-concept study, ultrasonography images were obtained from the multicenter HANDSOME cohort. The training dataset inc

In-silico biomechanical simulation of tendon transfers for finger extension in radial nerve palsy.

BACKGROUND: Tendon transfers are commonly used to restore finger extension in patients with radial nerve palsy, but quantitative comparisons of different techniques remain limited. Achieving the correct tension continues to be a surgical art, guided more by experience than by objective standards. In-silico biomechanical simulation may serve as a valuable tool to evaluate and optimize tendon transfer strategies in a more reproducible and quantitative manner. METHODS: We used OpenSim software to simulate four tendon transfers targeting the extensor digitorum communis (EDC): flexor carpi radiali

Phalangeal fracture in a 60-year-old man after a low-voltages electric shock: A case report.

RATIONALE: Fractures caused by low-voltage electric shocks are extremely rare, especially those involving the distal appendicular skeleton. Most reported cases involve the proximal skeleton, such as the shoulder or vertebrae, and phalangeal fractures following low-voltage shocks without direct mechanical trauma have not been reported previously. PATIENT CONCERNS: A 60-year-old man presented with persistent pain, swelling, limited thumb motion, and a non-healing ulcer after sustaining a 220V household electric shock to his left thumb. The patient reported falling onto his buttocks immediately

Comparison of force profiles from two musculoskeletal palpation methods: A methodological study.

BACKGROUND: The Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) protocol recommends a 5 s and 1 kg force dynamic palpation around the lateral condylar pole of the temporomandibular joint. However, the accuracy and precision of the generated force are not known. OBJECTIVE: To assess and compare the force profiles generated from dynamic palpation manually and using a palpometer, based on the forces and time recommendations suggested by the DC/TMD protocol. METHODS: Nineteen healthy adults applied forces of 0.5 kg, 1.0 kg and 2.0 kg on a calibrated force sensor in a circular motion

Biomechanical Assessment Methods Used in Chronic Stroke: A Scoping Review of Non-Linear Approaches.

Non-linear and dynamic systems analysis of human movement has recently become increasingly widespread with the intention of better reflecting how complexity affects the adaptability of motor systems, especially after a stroke. The main objective of this scoping review was to summarize the non-linear measures used in the analysis of kinetic, kinematic, and EMG data of human movement after stroke. PRISMA-ScR guidelines were followed, establishing the eligibility criteria, the population, the concept, and the contextual framework. The examined studies were published between 1 January 2013 and 12

Risk of Upper Limb Diseases in Osteoarthritis Patients: A Propensity-score-matched Cohort Study.

BACKGROUND/AIM: Osteoarthritis (OA) is a prevalent degenerative joint disease that significantly impacts quality of life, particularly when affecting the hands. However, whether patients with OA are associated with higher risk of developing upper limb disorders, specifically trigger finger (TF) and carpal tunnel syndrome (CTS), remains unclear. This study aimed to evaluate the risk of upper limb disease in OA patients. PATIENTS AND METHODS: Using the US Collaborative Network, a subset of the TriNetX research network, we identified patients diagnosed with OA and matched them 1:1 with non-OA co

Suppurative tenosynovitis with chronic carpal tunnel syndrome due to corticosteroid injections: A case report.

INTRODUCTION: Steroid hormone blocking is a common treatment for tenosynovitis. However, local steroid hormone blocking requires careful attention to the local inflammatory response, as infection can cause severe local soft tissue inflammation and damage. PRESENTATION OF CASE: A 46-year-old female patient received local steroid hormone blocking treatment for tenosynovitis of the right thumb at another hospital 3 months earlier. Five days later, the patient gradually experienced redness, swelling, and pain in her right hand. By day 7, the symptoms worsened with increased swelling, wrist pain,

Current Management of Trigger Digit in Rheumatoid Arthritis Patients: A Survey of ASSH Members.

BACKGROUND: Traditional dogma regarding management of rheumatoid arthritis (RA) patients with trigger digit symptoms holds that A1 pulley release should be avoided. Surgical release was thought to further destabilize the metacarpophalangeal joint. Biologic disease modifying anti-rheumatic drugs (DMARDs) have limited the development of hand deformities. Despite advances in RA treatment, many textbooks continue to discourage release of the A1 pulley in RA patients. The aim of this study was to determine if this belief is consistent with current trends in surgical management of trigger digits in
