Effects of PAX6 mutations on retinal function: an electroretinographic study.

PURPOSE: To investigate the retinal function in aniridic patients with documented PAX6 mutations to determine the range of electroretinogram abnormalities in aniridic patients and to relate electroretinogram findings with specific PAX6 mutations. METHODS: Eleven patients with typical aniridia and fully characterized PAX6 mutations underwent electroretinography. RESULTS: In all 11 patients, electroretinogram recordings were abnormal, ranging from mild to severe. Rod-related and cone-related activities were equally affected. The amplitude of the oscillatory potentials was the most reduced, fol…

Status of prezygotic chromosome lesions in relation to cancer.

There are currently three recognized prezygotic chromosome defects that are associated with specific cancers. These are a deletion of band 13q14 with retinoblastoma, deletion of band 11p13 with Wilms' tumor associated with aniridia, and a reciprocal translocation of chromosomes 3 and 8 with clear cell cancer of the kidney. The two deletions have been assumed to be variants of the hereditary subgroup of tumors. There are many obvious similarities between retinoblastoma associated with deletion of 13q13 and Wilms'-aniridia associated with deletion of 11p13, but there are also significant dispari…

A mixed methods study of the impact of WAGR spectrum disorder on individuals and their caregivers.

BACKGROUND: WAGR spectrum disorder (WSD) is a rare genetic condition classically characterized by Wilms tumor, Aniridia, Genitourinary abnormalities, and Range of developmental delay. Following our publication, the terminology has shifted from “syndrome” to “spectrum disorder” to better capture its phenotypic variability.1 While other multi-systemic features have been identified, there is a paucity of literature around how WSD impacts individuals and their families. To guide clinical management recommendations and development of outcome measures for WSD clinical studies, this study explored th…

Association of brain-derived neurotrophic factor (BDNF) haploinsufficiency with lower adaptive behaviour and reduced cognitive functioning in WAGR/11p13 deletion syndrome.

In animal studies, brain-derived neurotrophic factor (BDNF) is an important regulator of central nervous system development and synaptic plasticity. WAGR (Wilms tumour, Aniridia, Genitourinary anomalies, and mental Retardation) syndrome is caused by 11p13 deletions of variable size near the BDNF locus and can serve as a model for studying human BDNF haploinsufficiency (+/-). We hypothesized that BDNF+/- would be associated with more severe cognitive impairment in subjects with WAGR syndrome. Twenty-eight subjects with WAGR syndrome (6-28 years), 12 subjects with isolated aniridia due to PAX6 m…

Combined pars plana vitrectomy and artificial iris diaphragm implant after globe rupture.

BACKGROUND: Retinal detachment (RD) associated with aniridia due to globe rupture (GR) is an uncommon condition with a severe prognosis. Surgical technique must address anterior and posterior segment issues secondary to the altered compartmentalization and increased risk for corneal toxicity. The purpose of this paper is to report a series of GR patients undergoing combined pars plana vitrectomy (PPV) and artificial iris diaphragm (AID) implant for the repair of RD associated to aniridia. METHODS: The authors retrospectively reviewed 12 consecutive patients operated on by a single surgeon. Su…

Aniridic glaucoma: An update.

Aniridia is a congenital bilateral ocular disorder with dominant autosomal inheritance. More than 50% of patients will develop aniridic glaucoma (AG) during their lives. Open angle glaucoma is more common in aniridia, but a closed angle mechanism has been described in relation with anterior rotation of the rudimentary iris, occluding trabecular meshwork. Diagnosis and follow-up of AG is difficult in relation with the presence of keratopathy, nystagmus and foveal hypoplasia. Central corneal thickness usually measures more than 600 microns, which prevents achieving a reliable value of intraocula…

The fovea regulates symmetrical development of the visual cortex.

The foveal region contains the highest cell density in the human retina; consequently a disproportionately large area of the visual cortex is dedicated to its representation. In aniridia and albinism the fovea does not develop, and the corresponding cortical representation shows a reduction in gray matter volume. In albinos there are chiasmatic irregularities in the hemispheric projections, which are not found in aniridics. Here, we ask whether the anomalies in central retinal development, present in albinism and aniridia, have a wider impact on the architecture of the visual cortex. The lengt…

Gillespie Syndrome: Greek Patient with Novel Mutation and Atypical Presentation - Expansion of the Phenotype and Literature Review.

INTRODUCTION: Gillespie syndrome is characterized by bilateral aniridia, cerebellar hypoplasia leading to ataxia, congenital hypotonia, and variable degrees of intellectual disability. The condition is attributed to pathogenic variants in ITPR1, either in a biallelic manner or as heterozygous dominant-negative mutations. CASE PRESENTATION: Herein, we report the case of a male neonate with generalized hypotonia, and bilateral aniridia, identified during routine newborn eye screening. His abdominal ultrasound revealed a left dysplastic pleiocystic kidney, and his brain magnetic resonance imagin…

Assessing Splicing Variants in the PAX6 Gene: A Comprehensive Minigene Approach.

Haploinsufficiency of the PAX6 gene causes aniridia, a congenital eye disorder characterised by the absence or malformation of the iris and foveal hypoplasia. Previous studies indicate that pathogenic splice variants account for up to 15% of all disease-causing PAX6 variants. However, this proportion may be significantly underestimated because the pathogenicity of splice variants can only be accurately established through experimental validation. In this study, we developed and validated a system of eight minigene constructions for the functional analysis of splicing variants in the PAX6 gene.…

Role for the Wilms tumor gene in genital development?

Detailed molecular definition of the WAGR region at chromosome 11p13 has been achieved by chromosome breakpoint analysis and long-range restriction mapping. Here we describe the molecular detection of a cytogenetically invisible 1-megabase deletion in an individual with aniridia, cryptorchidism, and hypospadias but no Wilms tumor (WT). The region of overlap between this deletion and one associated with WT and similar genital anomalies but no aniridia covers a region of 350-400 kilobases, which is coincident with the extent of homozygous deletion detected in tumor tissue from a sporadic WT. A c…

Phenotypic variations in patients with a 1630 A>T point mutation in the PAX6 gene.

BACKGROUND: The extreme clinical case-to-case variability of aniridia, even within families, can cause difficulties in making the correct diagnosis, prognosis and treatment plan. We describe seven patients from two families demonstrating variable expression of this syndrome, all with the same single point mutation within the PAX6 gene. METHODS: Case presentations. The authors review the clinical ophthalmic findings of the aniridia-keratopathy syndrome from two families, one involving four generations, the other with two generations. Polymerase chain reaction amplification of all 14 exons of t…

WAGR syndrome with tetralogy of Fallot and hydrocephalus.

Wilms tumor, aniridia, genitourinary abnormalities, and mental retardation (WAGR) syndrome occurs sporadically due to deletion of chromosome 11p13. A variety of other abnormalities involving different systems have been reported in patients with WAGR syndrome. We report on a patient with WAGR syndrome with accompanying tetralogy of Fallot and hydrocephalus.

PAX6 missense variants in two families with isolated foveal hypoplasia and nystagmus: evidence of paternal postzygotic mosaicism.

PAX6 is considered the master regulator of eye development, the majority of variants affecting this gene cause the pan-ocular developmental eye disorder aniridia. Although no genotype-phenotype correlations are clearly established, missense variants affecting the DNA-binding paired domain of PAX6 are usually associated with non-aniridia phenotypes like microphthalmia, coloboma or isolated foveal hypoplasia. In this study, we report two missense heterozygous variants in the paired domain of PAX6 resulting in isolated foveal hypoplasia with nystagmus in two independent families: c.112 C > G; p.(…

[A case of Axenfeld's syndrome].

This syndrome has occurred in three generations of one family. It involves congenital aniridia, lens opacity, congenital glaucoma because of anterior chamber angle dysgenesis. The authors emphasize a high risk of inheriting this condition and recommend the subjects suffering from it consult medical geneticists if they wish to marry and have children.

WAGR syndrome and multiple exostoses in a patient with del(11)(p11.2p14.2).

The WAGR syndrome (Wilms' tumour, aniridia, genital anomalies, and mental retardation) is well documented to be associated with a deletion of 11p13. We present a patient with a del(11)(p11.2p14.2) who as well as all the features of WAGR syndrome has multiple exostoses. We suggest that this could be a possible locus for hereditary multiple exostoses.

Wilms' tumor and associated malformations; report of two cases with WAGR and Drash syndrome.

The combination of Wilms' tumor with aniridia, ambiguous genitalia/gonadoblastoma and mental retardation is defined as WAGR syndrome. The association with pseudohermaphroditism and nephropathy was also described as Drash syndrome. Here we report two cases with WAGR and Drash syndrome. The importance of early recognition and appropriate management of Wilms' tumor with those concurrent malformations is highlighted.
