A new technique is described for outpatient dental rehabilitation of a severely atrophic mandible. Calvarial bone graft fixed to the mandible through an extraoral approach was used to treat a 47 year-old woman with class VI mandibular atrophy. 5 months later, six endosseous dental implants were placed between the mental foramina and rehabilitation was completed using a fixed implant-supported prostheses. After 8 years, the denture remains stable on the implants and no bony changes have been noted. This technique is an efficient option for treating patients with a severe mandibular atrophy.

Implant dentistry has evolved into the mainstream of restorative practices all over the world. Maintenance of bone after tooth loss to improve or maintain facial esthetics and improved retention, function, and performance of removable restorations are only some of the advantages for the edentulous patient. No longer are implants considered only when traditional restorations cannot be fabricated. Evidence-based reports indicate implant restorations last longer than those on teeth and the abutment teeth are at less risk of loss or complication.

Clinical indications for the newly released Ti-Zr (Roxolid®) alloy mini-implants (MDIs) aimed for overdenture (OD) retention in subjects with narrow alveolar ridges are not fully defined. The aim of this study was to analyze peri-implant and posterior edentulous area microstrains utilizing models of the mandible mimicking a "real" mouth situation with two (splinted with a bar or as single units) or four unsplinted Ti-Zr MDIs. The results are reported on 34 edentulous patients, who underwent interposed autogenous bone graft augmentation in the symphysis of the mandible, combined with bone-hydroxylapatite onlay augmentation of the area posterior to the mental foramina. Two to four implants were placed in the grafted symphysis after 3-5 months. An overdenture was constructed three months later. The follow-up period ranged from one to seven years. An average loss of mandibular bone height of 10-13% was observed. The data provided no evidence of a further time-dependent resorption from two-and-half to seven years.
