Pseudoaneurysm of the Petrosal Internal Carotid Artery in the Middle Ear as a Complication of Middle Ear Cholesteatoma.

We report the case of a 66-year-old man who had significant otorrhagia caused by a ruptured pseudoaneurysm in the petrous internal carotid artery (ICA). The patient had middle ear cholesteatoma, and computed tomography (CT) showed bony erosion and exposure of the ICA into the middle ear cavity. Further angiography of the right carotid artery revealed a pseudoaneurysm protruding from the petrosal ICA into the middle ear cavity. The pseudoaneurysm was promptly treated with coil embolization and stenting. The patient continues to be monitored regularly with clinical follow-up, and carotid angiogr…

Erythema associated with pain and warmth on face and ears: a variant of erythermalgia or red ear syndrome?

Erythermalgia is a rare cutaneous disorder characterized by attacking of erythema, pain and increased temperature, which primarily involves the extremities and may infrequently extend to the neck, face, ears and even the scrotum. We reported an 18-year-old woman who presented with 3 years history of sole involvement of attacking erythema, pain and warmth over her face and ears without any other associations. The frequency and severity of the flares progressed gradually during the course. Cutaneous examination revealed erythema, increased temperature and tenderness on the face and ears during t…

Nickel release from earrings purchased in the United States: the San Francisco earring study.

BACKGROUND: Nickel sensitization is frequent among US patients with dermatitis and in the general population. In Europe, decreasing prevalences of nickel sensitization are observed as a result of the European Union Nickel Directive. However, no directive exists in the United States. OBJECTIVES: We sought to examine nickel release from inexpensive earrings and discuss possible methods of reducing nickel exposure among consumers. METHODS: A total of 34 different stores and artists were visited. A total of 277 earrings were purchased and examined with the dimethylglyoxime (DMG) test. RESULTS: …

Effect of the status after ear surgery and ear pathology on the results of infrared tympanic thermometry.

Most publications that deal with infrared tympanic thermometry (ITT) have performed a comparison between the established temperature measurements and ITT. However, to date an understanding of the influence of pathological ear findings on ITT test results is incomplete. Therefore, in the present study ITT was performed in healthy adults (n =21), adult patients with monaural central perforation (n =31) or strong scar formations (n =24) of the tympanic membrane and 18 adult patients after monaural canal wall down surgery. Right and left ear and oral temperature were measured three times by one in…

Is the aged rat ear more susceptible to noise or styrene damage than the young ear?

Noise- and styrene-induced hearing and hair cell loss were studied in young (3 months) and aged (24-26 months) Long-Evans rats. The animals were exposed 6 h/d, 5 d/w for 4 weeks to (a) broadband noise centered at 8 kHz (92 or 97dB SPL), or b) styrene (700 ppm). Auditory sensitivity was tested by recording evoked potentials from the inferior colliculus. Histological analyses of the organ of Corti, stria vascularis, and the spiral ganglions were also performed. Aged controls showed outer hair cell (OHC) loss at the basal and apical regions of the organ of Corti, and an increase in pigmentation c…

Middle Ear Transfer Function for Sound Conduction Study of Middle Ear Implants: A New Method.

Human temporal bone specimens are used in experiments to measure the sound transfer function of the middle ear. This standard method for the development of middle ear implants requires temporal bones that possess appropriate mechanical properties and structural stability and are free from pathological conditions. A laser Doppler vibrometer is commonly employed to measure the vibration of the stapes footplate. These experiments are typically conducted in the middle ear by leaving the inner ear intact, exposing the stapes footplate through the tympanic cavity, and directing the laser beam onto t…

Sheep as a large animal ear model: Middle-ear ossicular velocities and intracochlear sound pressure.

Animals are frequently used for the development and testing of new hearing devices. Dimensions of the middle ear and cochlea differ significantly between humans and commonly used animals, such as rodents or cats. The sheep cochlea is anatomically more like the human cochlea in size and number of turns. This study investigated the middle-ear ossicular velocities and intracochlear sound pressure (ICSP) in sheep temporal bones, with the aim of characterizing the sheep as an experimental model for implantable hearing devices. Measurements were made on fresh sheep temporal bones. Velocity responses…

Blue Ear Cyst: A Rare Eccrine Hidrocystoma of the Ear Canal and Successful Endoscopic Excision.

Aims. Hidrocystomas are benign cystic growths of the apocrine and eccrine sweat glands. These cystic lesions have been well documented on the face, head, and neck, but rarely in the external auditory canal. Presentation of Case. A 67-year-old woman presented with a bluish cystic mass partially occluding the external auditory canal and interfering with hearing aid use. Lesion was excised completely via a transcanal endoscopic approach with excellent cosmetic results, no canal stenosis, and no recurrence at 1-year follow-up. Discussion. We present a rare eccrine hidrocystoma of the external audi…

Ultrasound-guided Greater Auricular Nerve Block for Emergency Department Ear Laceration and Ear Abscess Drainage.

BACKGROUND: Adequate emergency department (ED) anesthesia for painful ear conditions, such as ear lacerations or ear abscesses, can be challenging. Much of the sensory innervation of the ear is supplied from the anterior and posterior branches of the greater auricular nerve (GAN). The GAN is a branch of the superficial cervical plexus, which arises from the C2/C3 spinal roots. The GAN innervation includes most of the helix, antihelix, the lobule, and the skin over the mastoid process and parotid gland. Anesthesia of the GAN is commonly performed in emergency medicine as part of a landmark-base…

A Review of the Middle Ear Risk Index as a Prognostic Tool for Outcome in Middle Ear Surgery.

This review article aims to scrutinize the studies conducted to determine a relationship between preoperative Middle Ear Risk Index (MERI) factors and postoperative graft acceptance and audiological gain in patients undergoing tympanoplasty procedures in middle ear surgeries. Critical analysis is done on numerous research and types of studies that were done in this area during the past years. The clinical and technical aspects connected to disease and its care have a variety of effects on the morphological and functional outcome of tympanoplasty. A better comprehension of these characteristics…

A quick method to expose the structures and relations of the middle ear and inner ear by cadaveric dissection.

INTRODUCTION: The temporal bone contains structures related to hearing and balance, and is a valuable learning resource for medical students and trainee surgeons. The middle ear and inner ear are difficult to demonstrate by cadaveric dissection as the structures are closely contained in a small space in the dense temporal bone. Consequently, the teaching and learning of the ear are largely relegated to virtual and theoretical images, and models, which has resulted in a knowledge gap in medical students and prospective surgeons. The present study aimed to elucidate a technique that exposes the …

A Case Report of a CPA Meningioma Presenting as Middle Ear Mass With Middle Ear Effusion.

Middle ear (ME) benign tumors are rare, and among them is meningioma. An ME meningioma might be isolated or merely a lateral extension of a CPA meningioma. We report a case with presentation of ME effusion followed by the appearance of an aural polyp after repeated myringotomies. Computed tomography (CT) revealed a benign-looking ME and mastoid mass. After debulking and biopsy, it turned out to be a meningioma. However, when MRI was performed, a large CPA meningioma was detected. ME masses are rare; however, they might be encountered, and CT must be performed followed by biopsy or total remova…

The insertion loss distribution function of an ear plug, and its implications for the ear plug acceptability.

INTRODUCTION: In order to establish the acceptability of a hearing protector device (HPD) used in a given noisy environment, two key elements must be known with the highest possible accuracy: the insertion loss of the HPD and the associated variability. Methods leading to objective field measurements of insertion loss have become widely available in the last decade and have started to replace the traditional subjective "Real-Ear Attenuation at Threshold" (REAT) laboratory measurements. The latter have long been known to provide a gross overestimate of the attenuation, thus leading to a strong …

Middle-ear and inner-ear contribution to bone conduction in chinchilla: The development of Carhart's notch.

While the cochlea is considered the primary site of the auditory response to bone conduction (BC) stimulation, the paths by which vibratory energy applied to the skull (or other structures) reaches the inner ear are a matter of continued investigation. We present acoustical measurements of sound in the inner ear that separate out the components of BC stimulation that excite the inner ear via ossicular motion (compression of the walls of the ear canal or ossicular inertia) from the components that act directly on the cochlea (cochlear compression or inertia, and extra-cochlear 'third-window' pa…

Effects of ear canal static pressure on the dynamic behaviour of outer and middle ear in newborns.

OBJECTIVE: The present study investigated the effect of ear canal pressure on the dynamic behaviour of the outer and middle ear in newborns with and without a conductive condition using the sweep frequency impedance (SFI) technology. METHODS: A test battery consisting of automated auditory brainstem response (AABR), transient evoked otoacoustic emission (TEOAE) and 1000-Hz tympanometry (HFT) was performed on 122 ears of 86 healthy newborns and 10 ears of 10 newborns with a conductive condition (failed TEOAE and HFT). The dynamic behaviour of the outer and middle ear, when the pressure applied…

Prominent ears: Anthropometric study of the external ear of primary school children of Harare, Zimbabwe.

BACKGROUND: Prominent ear is the most common congenital ear deformity affecting 5% of children in the Western world and has profound psychosocial effects on the bearer. It is important to know the prevalence in the local population to have a better appreciation of the local burden of the abnormality as well as to know the parameters of ear morphology locally. These parameters can be useful in the diagnosis and evaluation of ear anomalies and may help reconstructive surgeons in reproducing an anatomically correct ear of an African/Zimbabwean child. OBJECTIVES: To evaluate the frequency of prom…
