The control of childhood viral infections by pulse vaccination.

Pulse vaccination, the repeated application of vaccine over a defined age range, is gaining prominence as a strategy for the elimination of childhood viral infections such as measles and polio. However, unlike routine or continuous mass infant immunization, epidemiological understanding of this control method is in its infancy. This paper develops initial work by Agur et al. (1993) using simple steady-state and age-structured dynamic models to extend the theory of the mechanism of action of pulse vaccination, and to explore the relationship between the maximum permitted interval between pulses

Use of a marginal structural model to determine the effect of aspirin on cardiovascular mortality in the Physicians' Health Study.

The 1982-1988 aspirin component of the Physicians' Health Study, a randomized trial of aspirin and beta-carotene in primary prevention of cardiovascular disease and cancer among 22,071 US male physicians, was terminated early primarily because of a statistically extreme 44% reduction in first myocardial infarction, with inadequate precision and no apparent effect on the primary endpoint, cardiovascular death. Because of the demonstrated efficacy of aspirin in secondary prevention of cardiovascular death, nonfatal cardiovascular events may simultaneously be time-dependent confounders and interm

Standardization and Prediction to Control Confounding: Estimating Risk Differences and Ratios for Clinical Interpretations and Decision Making.

What is the added risk for death from smoking? Logistic regression has become the most common statistical method to answer such questions in the biomedical literature. However, the typical analyses estimate odds ratios, a metric too often misunderstood and misinterpreted. Although estimates of risks, and their differences and ratios, offer transparent clinical interpretations, commonly used statistical models have known methodological shortcomings. "Standardization" through modeling, weighting, or matching offers a solution. The goals of this article are to review classical concepts of standar

Optimizing Disease Outbreak Forecast Ensembles.

On the basis of historical influenza and COVID-19 forecasts, we found that more than 3 forecast models are needed to ensure robust ensemble accuracy. Additional models can improve ensemble performance, but with diminishing accuracy returns. This understanding will assist with the design of current and future collaborative infectious disease forecasting efforts.

Modeling the Dynamic of Multiwave Diseases: The Model of Hand, Foot and Mouth Disease.

An HFMD outbreak spread over the city of Hải Phòng from summer 2011 to autumn 2012. This epidemic was chosen because it was the very first HFMD epidemic in North Vietnam, eliminating thus interferences with previous outbreaks. This epidemic displayed three separate waves. A complete dataset was collected for more than 9500 patients during this period, which enabled us to analyze this epidemic at different scales. Access to the healthcare system was crucial during this period, which was possible due to a reorganization of the system in February-March 2012. An analysis at the commune level enabl

Spatial and temporal analysis of disease occurrence for detection of clustering.

Data on disease occurrence often consist of the number of cases recorded in a set of regions during each of several time periods. In this paper a method of analysis of such data is proposed which allows one to distinguish and test for spatial clustering, temporal clustering, and space-time clustering of cases. Departures of case numbers from expectation are partitioned into region and time period main effects and region-by-time interactions. Spatial and temporal relationships among regions and time periods are incorporated by pooling estimates over neighborhoods. In addition to providing tests

Biobehavioral mechanisms of work-related upper extremity disorders: a new agenda for research and practice.

BACKGROUND: Epidemiological studies provide support for the role of organizational and individual psychosocial stressors in work-related upper extremity disorders (WRUEDs). Despite this evidence, the biological plausibility of a relationship between exposure to various psychosocial and work organizational stressors and WRUEDs remains unclear METHODS: The Georgetown Symposium on Biobehavioral Mechanisms of Work-Related Upper Extremity Disorders was held in Washington D.C. on November 6-7, 2000 to improve the understanding of potential biobehavioral mechanisms, identify future areas for researc

Underdetected dispersal and extensive local transmission drove the 2022 mpox epidemic.

The World Health Organization declared mpox a public health emergency of international concern in July 2022. To investigate global mpox transmission and population-level changes associated with controlling spread, we built phylogeographic and phylodynamic models to analyze MPXV genomes from five global regions together with air traffic and epidemiological data. Our models reveal community transmission prior to detection, changes in case reporting throughout the epidemic, and a large degree of transmission heterogeneity. We find that viral introductions played a limited role in prolonging sprea

Identifiability and model misspecification for modelling recurrent infections using routine health care data.

For infectious diseases where people can experience multiple infections during their lifetime, the time between observed infections in individuals (or "time to recurrence") can provide valuable information on infection and transmission dynamics. Routinely collected data, such as electronic health records, are a potential source of time to recurrence data. However, they are challenging to analyse because patients can drop out of the data set in a way that is not visible to the data collection process. Standard epidemiological approaches, such as parametric survival analysis with imputation, can

Dynamical analysis and numerical assessment of the 2019-nCoV virus transmission with optimal control.

In this article, we discuss the qualitative analysis and develop an optimal control mechanism to study the dynamics of the novel coronavirus disease (2019-nCoV) transmission using an epidemiological model. With the help of a suitable mathematical model, health officials often can take positive measures to control the infection. To develop the model, we assume two disease transmission sources (humans and reservoirs) keeping in view the characteristics of novel coronavirus transmission. We formulate the model to study the temporal dynamics and determine an optimal control mechanism to minimize t

Do "soft" interventions matter more than vaccination? Rabies as an example.

Interventions such as vaccinations, treatment et cetera are usually the gold standard of disease control, as measured by reducing the reproduction number below unity. However, in practice, few diseases are reduced below this eradication threshold and instead persist despite active intervention campaigns. We propose an epidemic model of rabies with a saturated incidence rate that represents "soft" interventions such as public-awareness campaigns, animal curfews, fences etc. We prove local and global stability results based on the reproduction number. However, numerical simulations suggest that

SEIRS model for TB transmission dynamics incorporating the environment and optimal control.

Tuberculosis (TB) remains a significant global health challenge, claiming more than 2 million lives annually, predominantly among adults. Existing studies often neglect the environment, reinfection, relapse/reactivation, and model calibration, thus limiting their applicability. This study presents a novel data-driven model that incorporates these factors to analyze the dynamics of TB transmission. Using the next-generation matrix approach, a basic reproduction number ( R 0 ) of 1.737266 was calculated, indicating that active TB disease will persist in the human population without robust publi

Phylogenetic insights into the transmission dynamics of arthropod-borne viruses.

Arthropod-borne viruses (arboviruses) impose substantial global health and economic burdens, affecting both human and animal populations. These viruses - including dengue, chikungunya, Rift Valley fever, Crimean-Congo haemorrhagic fever and bluetongue viruses - have complex transmission cycles involving vertebrate hosts and arthropod vectors. Their circulation in livestock and wildlife complicate surveillance, as traditional epidemiological approaches rely mainly on human clinical data. Climate change and increasing global interconnectedness are accelerating their emergence and invasion, neces

Integrating circulating microRNAs with epidemiological factors enhances breast cancer detection across subtypes: the MCC-Spain study.

Circulating microRNAs (miRNAs) are promising non-invasive biomarkers for cancer detection; however, their diagnostic performance across breast cancer molecular subtypes and their incremental value beyond demographic and epidemiological variables remain incompletely characterized. We conducted a multicenter case–control study including 317 breast cancer cases and 127 population-based controls. Serum levels of 44 literature-derived miRNAs were quantified by RT-qPCR. Feature selection was performed using LASSO penalization, followed by multivariable logistic regression to estimate odds ratios (OR

A hybrid STL-LightGBM framework with probabilistic forecasting for Influenza A incidence in the post-pandemic Saudi Arabia.

Influenza A outbreaks in Saudi Arabia exhibit different seasonal patterns, influenced by significant changes, including a near-total halt during the COVID-19 pandemic (2020-2021) and a substantial rebound, as evidenced by national surveillance data, until the end of 2023. Traditional time-series models rely on stationarity and stable seasonal patterns; however, these assumptions are significantly undermined by regime shifts. This study introduces a forecasting method that uses light gradient boost machine (LightGBM) regression, along with Seasonal-Trend decomposition using LOESS (STL), to bett

Sample size in social contact surveys for epidemic modelling and risk prediction.

Social contact surveys, measuring who contacts whom, are widely used for epidemic intelligence and public health decision-making. Despite their widespread use, sample size calculations are not performed. We conducted a review of social contact surveys (2008-2025) to characterize current practice. We characterized the impact of survey size on epidemic metrics using two social contact surveys, the UK Social Contact Survey and POLYMOD (Europe), and two methods. For each dataset and approach, we generated repeated subsamples and calculated the resulting reproduction numbers, characterized their di

Threshold dynamics and epidemic-informed machine learning for forecasting of mpox: A U.S. case study.

Mpox (monkeypox) is a re-emerging viral disease with increasingly sustained human transmission, particularly in previously unaffected regions. In this study, we develop a compartmental epidemic model that captures mpox transmission dynamics and incorporates a treatment function to assess the impact of medical interventions on disease progression and recovery. The model is calibrated using U.S. case data, with parameters estimated via the Trust Region Reflective optimization method to ensure close alignment with observed trends. We perform global sensitivity analysis using partial rank correlat

Evaluation of non-vaccination brucellosis control strategies in class II regions using a transmission dynamics model: a case study from Hubei Province.

OBJECTIVE: To develop a transmission dynamics model for brucellosis in Hubei Province and evaluate non-vaccination control strategies. METHODS: We constructed a SsIs-W-ShEhIhCh(Susceptible sheep - Infected sheep - Environmental bacterial load - Susceptible humans - Exposed humans - Acute infected humans - Chronic infected humans) transmission dynamics model and assessed disease transmissibility through R0 calculation. Four intervention measures (environmental disinfection, improved diagnosis, personal protection, and infected sheep management) were evaluated independently and in combination. R

The effect of screening on the health burden of chlamydia: An evaluation of compartmental models based on person-days of infection.

Sexually transmitted diseases (STDs) are detrimental to the health and economic well-being of society. Consequently, predicting outbreaks and identifying effective disease interventions through epidemiological tools, such as compartmental models, is of the utmost importance. Unfortunately, the ordinary differential equation compartmental models attributed to the work of Kermack and McKendrick require a duration of infection that follows the exponential or Erlang distribution, despite the biological invalidity of such assumptions. As these assumptions negatively impact the quality of prediction
