Ileoileal knotting: a rare cause of intestinal obstruction: a case report.

BACKGROUND: Ileoileal knotting is one of the rarest causes of intestinal obstruction. The pathology involves knotting of the ileum around itself, leading to mechanical intestinal obstruction that can rapidly evolve to gangrene. CASE PRESENTATION: Here we will discuss the case of an 18-year-old Oromo girl who presented with sudden onset of severe abdominal pain and signs of generalized peritonitis.Ultrasound examination showed massive peritoneal and cul-de-sac fluid. Explorative laparotomy was done, with a tentative diagnosis of ruptured ovarian cyst. Intraoperative finding was a gangrenous il

Morel-Lavallée lesion of the anterior leg: a rare anatomical presentation.

A Morel-Lavallée lesion (MLL) is a rare injury caused by blunt force trauma causing separation of subcutaneous tissue from the deep fascia. It is frequently seen in orthopaedic cases involving fractures of the hip or pelvis but is rare in the lower leg. The rarity of this condition often leads to misdiagnosis. A 66-year-old man presented to the emergency department after a 300-pound safe sheered across his left anterolateral leg causing skin avulsion, tenderness, swelling, ecchymosis, and erythema. The patient was treated for suspected cellulitis with oral antibiotics, but the lesion evolved i

Type III Mirizzi, successfully treated with a free gallbladder flap, a case report.

BACKGROUND: Mirizzi syndrome is a type of biliary obstruction caused by an impacted stone in the gallbladder neck or cystic duct that causes and extrinsic obstruction of the common bile duct, this condition if left untreated can lead to duct erosion, fistula, and cholangitis. Preoperative diagnosis is difficult since if not diagnosed correctly can elevate the risk of intraoperative bile duct injury. CASE PRESENTATION: We present the case of a 61-year-old patient, she presented to our hospital with obstructive jaundice, and a type III Mirizzi syndrome was identified. Preoperative diagnosis was

[A Case of Rectosigmoid Cancer with Intestinal Malrotation Treated with Laparoscopic Surgery].

A 67-year-old woman was diagnosed with rectal cancer using colonoscopy. Computed tomography revealed the so-called superior mesenteric vein rotation sign, and intestinal malrotation was suspected. We planned chemotherapy after the surgical resection of the primary cancer because she had multiple lung metastases. Laparoscopic high anterior resection with D3 dissection of lymph nodes was performed. Intraoperative findings showed a non-rotation type intestinal malrotation and severe intra-abdominal adhesion. However, careful releasing operation enabled the typical approach of laparoscopic surgery

Rare localization of an extralobar pulmonary sequestration in a child as a diagnostic challenge: a case report and review of the literature.

We present a child with a rare finding of an extralobar pulmonary sequestration localized in the upper mediastinum. Findings of the prenatal screening enabled early postnatal diagnostic measures that revealed a heterogeneous mass situated next to the thymus. Based on the localization and inconclusive computed tomography images, the preoperative prediagnoses included enterogenic cyst, thymus cyst and teratoma. Intraoperative features of the vascular supply and structure of the mass drew the surgeons' suspicion to extralobar pulmonary sequestration, which was confirmed in the histopathological e

Choledochal cyst: presentation of the disease with a case report.

Choledochal cyst is a rare disease of the biliary tract. There are five main types of choledochal cysts with a few recognized sub-types. The etiology of choledochal cysts still is unclear. The incidence of biliary tract cancer in patients with choledochal cysts increases with age. In the past, choledochal cysts were often treated using drainage procedures; however, the optimal treatment used today is likely to involve the complete excision of the extrahepatic duct, cholecystectomy, and Roux-en-Y hepaticojejunostomy. Endoscopic treatment of type III choledochocele should be limited to the manag

Chondroblastoma: a rare cause of femoral neck fracture in a teenager.

Chondroblastomas usually present in the epiphyseal region of bones in skeletally immature patients. These uncommon, benign tumors are usually treated with curettage and use of a bone-void filler. Here we report a case of a hip fracture secondary to an underlying chondroblastoma in a 19-year-old woman. Open biopsy with intraoperative frozen section pointed toward a diagnosis of chondroblastoma. Extended curettage was performed, followed by cryotherapy with a liquid nitrogen gun and filling of the defect with calcium phosphate bone substitute. The femoral neck fracture was stabilized with a slid

An operative complication in a patient with a true posterior communicating artery aneurysm: case report and review of the literature.

Intraoperative oculomotor nerve injury in a patient with a true posterior communicating artery aneurysm is reported in detail. A comparison of internal carotid artery aneurysms at the posterior communicating artery junction with true posterior communicating artery aneurysms deserves special attention, because the vascular relationships of the aneurysm are more complex. A clip along the internal carotid artery does not occlude blood flow to the aneurysm, and the aneurysmal neck and the distal posterior communicating artery are closer to the oculomotor nerve. This is the 27th reported case of a

Moskowitz's Arterial and Venous Variants (Mo Type 2) in Sigmoid Resection for Diverticular Disease: A Case Report and Literature Review.

Diverticular disease of the sigmoid colon is common in Western countries, and acute diverticulitis may lead to complications requiring surgical intervention. Vascular anatomical variants, such as Moskowitz's artery and inferior mesenteric vein variations, can significantly influence surgical planning and outcomes. We report the case of a 57-year-old male with acute sigmoid diverticulitis complicated by a pericolic abscess. CT imaging revealed the presence of Moskowitz's artery and a venous variant of the inferior mesenteric vein draining directly into the portal vein. Due to clinical deteriora

Narrative review of management of thyroid surgery complications.

Currently, thyroid surgery is the most common and safe operation worldwide. However, thyroidectomy is still not free from the risks of complications and death due to the anatomical structure and physiological function particularity of the thyroid gland. Postoperative complications affect the life quality and life safety of patients after surgery. The common complications include hypoparathyroidism (HP), recurrent laryngeal nerve (RLN) injury, injury to the external branch of the superior laryngeal nerve (EBSLN), postoperative bleeding (PB), thoracic duct injury, laryngeal edema, tracheospasm,

Continuous monitoring of the recurrent laryngeal nerve in thyroid surgery: a critical appraisal.

BACKGROUND AND PURPOSE: Intraoperative neuromonitoring (IONM) contributes in several ways to recurrent laryngeal nerve (RLN) protection. Notwithstanding these advantages, surgeons must be aware that the current, intermittent, mode of IONM (I-IONM) has relevant limitations. To overcome these I-IONM limitations, a continuous IONM (C-IONM) technology has been proposed. METHODS: A PubMed indexed literature review of the current limitations of I-IONM is presented and a commentary about C-IONM is provided presenting the preliminary results of research on this topic. MAIN FINDINGS: I-IONM, despite

Rates of perioperative complications associated with laminectomies in patients with achondroplasia.

BACKGROUND: Lumbar decompressive surgery can be complicated by dural tears, infection, nerve root injury, deep venous thrombosis, and epidural hematoma. However, perioperative complications of multilevel laminectomies in patients with achondroplasia rarely are reported. Our objective was to determine the perioperative complication rates associated with laminectomies in patients with achondroplasia. METHODS: We reviewed the medical records of the initial laminectomies for all ninety-eight patients with achondroplasia at our institution, which included twenty-eight patients who had had previous

Abnormal bleeding in the cardiac operating room: An observational study of interrater reliability between anesthetists and surgeons.

OBJECTIVES: Postcardiopulmonary bypass bleeding is frequent and distinctive because of its associated coagulopathy. Despite diagnostic and therapeutic progress, substantial variability persists in hemostatic transfusion practices. Because transfusion of hemostatic components should target abnormal bleeding, this study assessed the situational awareness of anesthetists and surgeons in such situations. METHODS: A nationwide questionnaire using real intraoperative video clips-3 normal, 8 ambiguously abnormal, and 6 clearly abnormal situations-evaluated interrater reliability for identifying abno

Mucinous Carcinoma in a Male: First Documented Case in Nicaragua.

Breast malignancy in men is an exceedingly rare condition, representing a small fraction of all diagnosed breast cancer cases. The most common histological subtype is invasive ductal carcinoma, while the mucinous type is extremely rare. This pathology has a high mortality rate due to its poor prognosis and diagnosis in advanced stages, often initially overlooked with limited screening. Surprisingly, more men have died from breast cancer than from testicular cancer. This report details a case of invasive mucinous carcinoma in a 75-year-old male who presented with a five-week history of chronic

Getting it right first time: The importance of a structured tissue sampling protocol for diagnosing fracture-related infections.

INTRODUCTION: Fracture-related infection (FRI) is an important complication following surgical fracture management. Key to successful treatment is an accurate diagnosis. To this end, microbiological identification remains the gold standard. Although a structured approach towards sampling specimens for microbiology seems logical, there is no consensus on a culture protocol for FRI. The aim of this study is to evaluate the effect of a structured microbiology sampling protocol for fracture-related infections compared to ad-hoc culture sampling. METHODS: We conducted a pre-/post-implementation co

Exoscope-assisted spine surgery: Insights from orthopedic and neurosurgical teams through a survey.

INTRODUCTION: Spine surgery requires precise visualization to ensure safety in anatomically complex regions. Traditional microscopes, though effective, have ergonomic and communication limitations. High-definition three-dimensional (3D) exoscopes address these challenges by providing superior image quality, depth perception, ergonomics, collaboration, and teaching opportunities, while supporting minimally invasive techniques. This study reports the experience of two Italian centers adopting the exoscope as a primary visualization tool, integrating surgeons' and staff perspectives through struc
