Pushing the limits: perinatal outcomes beyond prolonged second stage.

Objective: To evaluate whether extremely prolonged second stage of labor in nulliparous women affects mode of delivery and perinatal outcomes. Methods: We performed a retrospective cohort study of nulliparous women with singleton gestations and cephalic presentation who reached 10 cm of cervical dilation at gestational age 36 0/7-41 6/7. Women were stratified by epidural status. Deliveries were compared by length of second stage: 0-179 min (normal second stage, NSS), 180-299 min (prolonged second stage, PSS), and ≥300 min (extremely prolonged second stage, EPSS). Primary outcome was incidence o

Timing of operative vaginal delivery and associated perinatal outcomes in nulliparous women.

OBJECTIVE: To compare perinatal outcomes in nulliparous women who had operative vaginal delivery early during second stage (1-3 h) to those who delivered vaginally with a prolonged second stage (>3 h). METHODS: This is a retrospective cohort study of nulliparas with term, singleton, vaginal deliveries beyond the first hour of second stage. Women who underwent operative vaginal deliveries (OVD) during 1-3 h of the second stage were compared to women who delivered vaginally but with a second stage duration of >3 h. Perinatal outcomes were examined using chi-square test, and potential confounder

[Can we reduce the episotomy rate?].

OBJECTIVES: Determine whether scientific evidence supports an "appropriate" episiotomy rate - and whether this rate can be reduced via medical intervention and or preventive actions during pregnancy or at the time of delivery. MATERIAL AND METHODS: A survey of the literature available on Medline and the Cochrane Library between 1980 and 2005 and dealing with the objectives of the present study was undertaken. RESULTS: In France, the national episiotomy rate should not reach 30%. A program aiming at continuous improvement in quality-of-care after episiotomy and including various actions - tra

Postpartum Urinary Retention in Vaginal Delivery Women Under Epidural Analgesia: Risk Factors and Nursing Strategies.

The widespread use of epidural labor analgesia has introduced new challenges in postpartum bladder function management. Postpartum urinary retention (PUR) not only causes maternal discomfort but may also lead to bladder overdistension and long-term voiding dysfunction. This study aimed to investigate the incidence and risk factors for PUR in women undergoing vaginal delivery under epidural analgesia and to develop a predictive model for early identification of high-risk individuals. A retrospective case-control study was conducted on 1002 parturients who underwent vaginal delivery under epidur

Maternal outcomes using delayed pushing versus immediate pushing in the second stage of labour: An umbrella review.

INTRODUCTION: Different systematic reviews have been developed in the last decades about maternal risks of immediate pushing and delayed pushing, depending on the duration of the second stage of labour, but they do not provide conclusive evidence. AIM: The main aim of this overview of systematic reviews was to assess the maternal outcomes using delayed pushing and immediate pushing in the second stage of labour in women receiving epidural analgesia. METHODS: We searched systematically in PubMed (Medline), EMBASE, CINAHL, and Scopus (October 26th, 2023). Methodological quality was analysed us

Effects of flexible sacrum positions during the second stage of labour on maternal and neonatal outcomes: A systematic review and meta-analysis.

AIMS AND OBJECTIVES: To assess the effects of flexible sacrum positions on mode of delivery, duration of the second stage of labour, perineal trauma, postpartum haemorrhage, maternal pain, abnormal foetal heart rate patterns and Apgar scores based on published literature. BACKGROUND: Maternal positions served as a nonmedical intervention may facilitate optimal maternal and neonatal outcomes during labour. Flexible sacrum positions are conducive to expanding pelvic outlet. Whether flexible sacrum positions have positive effects on maternal and neonatal well-being is a controversial issue under

Maternal and neonatal effects of outlet forceps delivery compared with spontaneous vaginal delivery in term pregnancies.

Previous retrospective studies have suggested that the prophylactic use of outlet forceps has a beneficial impact on the neonate because it shortens the second stage of labor and decreases the incidence of neonatal hypoxia. The purpose of this study was to compare the immediate maternal and neonatal effects of outlet forceps delivery (N = 165) with spontaneous vaginal delivery (N = 168) in term parturients. Subjects were randomized to the study or control group immediately before delivery. There were 88 nulliparas and 77 multiparas in the forceps delivery group and 90 and 78, respectively, who

Haemodynamic changes during labour: continuous minimally invasive monitoring in 20 healthy parturients.

BACKGROUND: There are few studies on maternal haemodynamic changes during labour. None have used continuous cardiac output monitoring during all labour stages. In this observational study, we monitored haemodynamic variables continuously during the entire course of labour in healthy parturients. METHODS: Continuous haemodynamic monitoring with the LiDCOplus technique was performed in 20 healthy parturients during spontaneous labour, vaginal delivery and for 15minutes postpartum. Cardiac output, stroke volume, heart rate, systemic vascular resistance, and systolic arterial pressure were measur

Postoperative morbidity following Caesarean delivery.

The current study was designed to determine the postoperative morbidity associated with Caesarean section and to compare the morbidity with the timing of the operation: elective versus emergency Caesarean section; subgroups of women delivered by emergency Caesarean section; women delivered during the first stage of labour versus those delivered during the course of the second stage. A retrospective review was conducted of the obstetric case record and the midwifery notes of all women delivered by Caesarean section over a 1-year period in a university teaching hospital (n = 619). The variables

Pushing in labor: performance and not endurance.

OBJECTIVE: It is believed that delivery is faster if women are instructed to voluntarily bear down in synchrony with their uterine contractions. Confronted by the large variance in the duration of the second stage of labor, many clinicians attribute a "fast" or a "short" expulsion time solely to the patient's willingness to cooperate or to the strength of epidural anesthesia if it is a factor. Yet, knowledge of pushing performance and the factors affecting it remain limited. We investigated the maternal, fetal, and labor characteristics that influence the maternal "pushing performance" and sou

Effect of Spleen 6 Point Acupressure on Severity of Labor Pain, Duration of Labor, and Women's Satisfaction.

INTRODUCTION: Severe and unmanageable labor pain can adversely affect maternal and neonatal outcomes. Among non-pharmacological methods, acupressure is increasingly recognized for its intrapartum benefits; however, further supportive evidence is needed. OBJECTIVES: This study aimed to investigate the effect of spleen 6 point (SP6) acupressure on the severity of labor pain, duration of labor, and women's satisfaction. METHODS: A quasi-experimental one-group pretest-posttest design was employed. The intervention was applied to 96 parturient women in a governmental hospital, following specific

Risk of preterm birth in the subsequent pregnancy following caesarean section at full cervical dilatation compared with mid-cavity instrumental delivery.

BACKGROUND: Expediting delivery in the second stage of labour often involves a choice between a caesarean section at full dilatation or mid-cavity instrumental delivery. Accumulating evidence suggests that the mode of delivery may influence the risk of preterm birth in the subsequent pregnancy. AIMS: To directly compare first birth caesarean section at full dilatation with mid-cavity instrumental delivery for the risk of preterm birth in the subsequent pregnancy (second birth). A further aim was to identify predictive factors associated with these index modes of delivery. MATERIALS AND METHO

Preventing primary cesarean births: midwifery care.

The incidence of cesarean birth in the United States is alarmingly high and cesareans are associated with added morbidities for women and newborns. Thus strategies to prevent cesarean particularly for low-risk, nulliparous women at term with a singleton fetus are needed. This article addresses evidence-based practices that may be used during intrapartum to avoid primary cesarean, including patience with progress in labor, intermittent auscultation, continuous labor support, upright positions, and free mobility. Second-stage labor practices, such delayed pushing and manual rotation of the fetus
