Role of neck dissection for metastatic nonseminomatous testicular carcinoma: case report and literature review.

OBJECTIVES: To review the incidence, treatment and prognosis of testicular carcinoma metastatic to the neck, and to propose a selective neck dissection be performed for residual disease post-chemotherapy. CASE REPORT: A 17-year-old young man with metastatic testicular carcinoma presented with a left neck mass. A palpable neck mass is the initial sign in approximately 5 per cent of cases of metastatic testicular teratoma. Approximately 30 per cent of patients with advanced-stage testicular cancer have extra-retroperitoneal disease post-chemotherapy, which requires resection. The presented pati…

Vertebral artery dissection after neck extension in an adult patient with Klippel-Feil syndrome.

The association between Klippel-Feil syndrome and vertebral artery dissection is quite rare. We report an adult patient with vertebral artery dissection and Klippel-Feil syndrome, to our knowledge only the third reported case of its kind. A 45-year-old woman with a known history of Klippel-Feil syndrome presented with occipital head and neck pain following forced neck extension. Diagnostic cerebral angiography revealed a high grade vertebral artery stenosis, consistent with vertebral artery dissection. Following 6 months of medical management, a repeat diagnostic angiogram revealed complete he

Prospective study on neck dissection after primary chemoradiation therapy in stage IV pharyngeal cancer.

BACKGROUND: Definitive chemoradiation is a well-established option in the treatment of locally advanced squamous cell carcinoma of the head and neck. The intention of this study was to evaluate its efficacy on cervical lymph node metastases in a prospective study after a standardized protocol for chemoradiation (CRT) and histopathological evaluation, respectively. PATIENTS AND METHODS: The data of 25 patients (10 oropharynx, 15 hypopharynx) who received planned neck dissection after definitive chemoradiation for UICC stage IV carcinomas of the pharynx were analyzed. All patients were sonomorp…

Central neck lymph node dissection for papillary thyroid cancer: comparison of complication and recurrence rates in 295 initial dissections and reoperations.

BACKGROUND: The American Thyroid Association recently changed its management guidelines for papillary thyroid cancer (PTC) to include routine central neck lymph node dissection (CLND) during thyroidectomy. We currently perform CLND during thyroidectomy only if enlarged central nodes are detected by palpation or ultrasonography; we perform CLND in the reoperative setting for recurrence in previously normal-appearing or incompletely resected nodes. Critics of this approach argue that reoperative CLND has higher complication and recurrence rates than initial CLND. We sought to test this argument,

Occult neck metastases risk factors and the role of elective neck dissection in cT3-4N0 adenoid cystic carcinoma of the parotid gland.

OBJECTIVES: To determine the predictor for occult neck metastases and the role of elective neck dissection (END) in cT3-4N0 parotid adenoid cystic carcinoma (ACC). METHODS: Patients with surgically treated parotid ACC were retrospectively enrolled. Predictors of occult neck metastases and the effect of END on disease specific survival (DSS), overall survival (OS), locoregional control survival (LRC), and distant metastasis free survival (DMS) were analyzed. RESULTS: Occult neck metastases occurred in 35 (19.7%) of the 178 patients undergoing an END. The tumor stage [p=0.011, 4.215 (1.387-10.…

Metastatic neck disease beyond the limits of a neck dissection: attention to the 'para-hyoid' area in T1/2 oral tongue cancer.

OBJECTIVE: We evaluated patients with small oral tongue cancer suffering from recurrence, which develops in the intervening area between the … para-hyoid lesion. A similar incidence was observed between the patients with and without previous neck dissection, 6.9% and 5.7%, respectively. All but one patient died due to uncontrolled neck disease. CONCLUSIONS: Recurrent para-hyoid lesions could occur, irrespective of a previous neck dissection. In other words, the para-hyoid area is beyond the limits of …

Risk factors and clinical courses of chylous fistula following lateral neck dissection: a decade-long cohort study in head and neck malignancies.

INTRODUCTION: Chylous fistula (CF) is a serious complication after lateral (ND) in patients with head and neck malignancies. Due to the low incidence of this complication, its risk factors and clinical courses remain unclear. The aim of this study is to provide dependable results regarding these aspects through the largest sample size and comprehensive indicators. METHODS: A retrospective study was conducted at our hospital from 2014 to 2024. Univariate and multivariate regression analyses were used to identify the risk factors for CF. RESULTS: A total of 3819 consecutive neck dissections in…

Postoperative complications following salvage neck dissection after (chemo)radiotherapy for head and neck squamous cell carcinoma: which patients are at high risk?

BACKGROUND: Salvage neck dissection (ND) is the treatment of choice for residual neck disease after (chemo)radiotherapy for head-and-neck squamous cell carcinoma (HNSCC). Although ND is a relatively safe surgical procedure, several studies have shown that salvage ND will increase the morbidity of (chemo)radiotherapy with possible increase in acute and late toxicity and deterioration of quality-of-life. Therefore, unnecessary salvage ND need to be avoided. However, the available literature could not identify potential groups at higher risk of post-operative complications because of the missing …

Elective neck dissection versus observation in the management of early tongue carcinoma with clinically node-negative neck: a retrospective study of 229 cases.

BACKGROUND: The aim of the study is to evaluate the results of elective neck dissection (END) versus conservative management (observation) in the treatment of stages I and II squamous cell carcinoma of tongue. PATIENTS AND METHODS: This is a retrospective study including 229 patients with surgical treatment between June 1993 and May 2010. RESULTS: There were 15 (9.6%) patients in the END group and 14 (19.2%) patients in the observation group who developed nodal recurrence alone without associated local recurrence or distant metastasis. Node-related mortality rate was 5.1% (8/156) for END and…

A proposal for redefining the boundaries of level V in the neck: is dissection of the apex of level V necessary in mucosal squamous cell carcinoma of the head and neck?

In 1991, the Committee for Head and Neck Surgery and Oncology of the American Academy of Otolaryngology-Head and Neck Surgery proposed to define the anatomic boundaries between the lymph node levels in the neck, as initially described by the Memorial Sloan-Kettering Cancer Center (New York, NY). Recently, radiological parameters have been outlined to identify boundaries between various neck levels. The lymphatics of the posterior triangle of the neck are gathered in level V, recently subdivided into 2 subgroups: level Va and level Vb. The superior boundary of level Va is defined by the apex of

Efficacy of salvage neck dissection for isolated nodal recurrences in early carcinoma of oral tongue with watchful waiting management of initial N0 neck.

BACKGROUND: Recent studies have shown that salvage treatment may be effective for early tongue carcinoma with nodal failures after watchful waiting policy. We wanted to assess the efficacy of salvage neck dissection. METHODS: We conducted a retrospective review of all patients with pT1 and pT2 oral tongue cancer who underwent partial glossectomy without any neck surgery and determined the survival after salvage neck dissection in this group of patients. RESULTS: Seventy patients with N0 disease underwent partial glossectomy, 20 patients had development of isolated nodal failures after watchf…

Comparable outcomes with improved esthetics: da Vinci-assisted neck dissection for early stage oral cancers.

This study compares the retro-auricular robotic-assisted approach using two or three robotic instruments with conventional open neck dissection in patients with early N stage oral cavity cancer. Twenty-six patients with T1-3N0-1M0 disease underwent supraomohyoid neck dissection between 2018 and 2023 (13 robotic, 13 open). Perioperative outcomes, including hospital stay, drainage duration, complications, and lymph node yield, showed no significant differences (median lymph nodes: 20 vs. 21; p = 0.950). During a median follow-up of 743 days, no recurrences or deaths occurred. In the last five ro…

First Experience of Single Port Robotic Areolar (SPRA) Thyroidectomy and Modified Radical Neck Dissection (MRND).

Background and Objectives: After introducing the single-port robotic surgical system (da-Vinci SP), thyroid surgeries using da-Vinci SP are becoming more popular. Although many methods have been designed for thyroidectomy using the da-Vinci SP, there are very few reports on methods that can perform not only thyroidectomy but also lateral cervical lymph node dissection. In this study, we want to report the first clinical experience with SPRA-MRND (Single Port Robotic Areolar-Modified Radical Neck dissection), using right breast access. Materials and Methods: From April 2024 to January 2025, a t…

Risk of Contralateral Central Compartment Recurrence Following Unilateral Therapeutic Neck Dissection for Papillary Thyroid Carcinoma.

BACKGROUND AND OBJECTIVES: The utility of bilateral central compartment neck dissection (CCND) in patients with papillary thyroid carcinoma (PTC) and unilateral clinically node-positive disease remains debatable. Previous studies evaluated contralateral occult lymph-node metastases, which do not necessarily correlate with clinical recurrences. The objective of our study was to evaluate whether unilateral CCND is sufficient, specifically evaluating recurrence in the contralateral central neck. METHODS: Patients with PTC treated with total thyroidectomy and therapeutic unilateral CCND with at l…

Neck dissection for advanced laryngeal cancer: Role and relevance in KwaZulu-Natal Province, South Africa.

BACKGROUND: The global standard of care for advanced laryngeal squamous cell carcinoma (SCC) is total laryngectomy and neck dissection. While this approach aligns with international guidelines, there is no consensus on whether elective neck dissection (END) should be incorporated during primary surgery for clinically negative neck nodes (cN0) or as a therapeutic option after nodal relapse. It is therefore imperative to evaluate associated oncological outcomes and local contextual factors regarding END surgical approach in advanced laryngeal SCC. OBJECTIVE: To evaluate the lymph node outcomes …

Cutaneous rupture of infected carotid artery pseudoaneurysm following neck dissection and radiotherapy for thyroid cancer.

We report a case of a 72-year-old man with advanced thyroid cancer who presented with massive neck bleeding from a ruptured infected pseudoaneurysm of the left common carotid artery, following radical neck dissection and radiotherapy. Due to active haemorrhage, tissue infection, and haemodynamic instability, endovascular treatment was contraindicated. Emergency surgical ligation of the left common and internal carotid arteries was performed. Postoperative infection control was achieved with broad-spectrum antibiotics and wound care. The patient recovered without neurological deficits. Prompt r…
