Adverse events of hyperthermic intravesical chemotherapy for non-muscle invasive bladder cancer patients.

INTRODUCTION: Non-muscle invasive bladder cancer (NMIBC) is one of the most frequent neoplasms in Denmark. Treatment of high-risk NMIBC usually consists of transurethral resection of bladder (TUR-B) followed by intravesical Bacillus Calmette-Guérin (BCG) instillations. Unfortunately, some patients are BCG-unresponsive and will relapse over time. Radical cystectomy is the recommended salvage treatment following BCG-failure or BCG-intolerance. However, not all patients are candidates for surgery and thus, in need of other treatment. This study investigates the adverse events of Hyperthermic Intr

Surrogacy of intermediate clinical endpoints for overall survival in BCG-naïve high-risk non-muscle-invasive bladder cancer.

INTRODUCTION: Recent trials in BCG-naïve high-risk non-muscle-invasive bladder cancer (HR-NMIBC) have added immunotherapy to BCG, often using event-free survival (EFS) as the primary endpoint. However, neither EFS nor progression-free survival (PFS) has been validated as a surrogate for overall survival (OS) in this setting. METHODS: We retrospectively analyzed data from 18 international centers, including patients with BCG-naïve HR-NMIBC treated between 2001 and 2025. Adequate BCG exposure (International Bladder Cancer Group criteria) was the treatment contrast for its effects on intermediat

Comparative effectiveness of intravesical gemcitabine vs. sequential gemcitabine and docetaxel for high-risk nonmuscle-invasive bladder cancer.

BACKGROUND: Sequential intravesical gemcitabine and docetaxel (Gem/Doce) are increasingly used for nonmuscle-invasive bladder cancer (NMIBC), especially in the setting of Bacillus Calmette-Guérin (BCG) shortage or failure. However, the contribution of docetaxel to gemcitabine remains unquantified. We compared the effectiveness of gemcitabine alone vs. Gem/Doce in high-risk NMIBC. METHODS: We conducted a retrospective study of 296 patients treated with gemcitabine (n = 173) or Gem/Doce (n = 123) at 3 Mayo Clinic sites between 2018 and 2023. The primary outcome was high-grade recurrence-free su

The prognostic significance of preoperative platelet-to-lymphocyte ratio and interleukin-6 level in non-muscle invasive bladder cancer.

BACKGROUND: Non-muscle invasive bladder cancer (NMIBC) is the most prevalent type of bladder cancer, typically associated with a favorable prognosis and a risk of recurrence during the follow-up period. Inflammatory markers have been used to predict prognosis in various cancer types. The aim of this study was to explore the prognostic value of the readily accessible inflammatory markers, platelet-to-lymphocyte ratio (PLR) and interleukin-6 (IL-6), in NMIBC. METHODS: The study comprised a retrospective analysis of clinical data collected from NMIBC patients diagnosed between October 2018 and O

Modulation of the tumor microenvironment in non-muscle-invasive bladder cancer by OncoTherad® (MRB-CFI-1) nanoimmunotherapy: effects on tumor-associated macrophages, tumor-infiltrating lymphocytes, and monoamine oxidases.

Non-muscle-invasive bladder cancer (NMIBC) presents management challenges due to its high recurrence rate and a complex tumor microenvironment (TME). This study investigated the effects of OncoTherad® (MRB-CFI1) nanoimmunotherapy on the TME of BCG-unresponsive NMIBC, focusing on alterations in monoamine oxidases (MAO-A and MAO-B) and immune markers: CD163, FOXP3, CD8, and CX3CR1. A comparative analysis of immunoreactivities was made before and after OncoTherad® treatment and an immune score (IS) was established to evaluate the correlation between immunological changes and clinical outcomes. The prognostic value of expressions of STAT3, PD-L1, and PD-L2 in Ta/T1 urothelial carcinoma before and after BCG treatment.

INTRODUCTION: Nonmuscle invasive bladder cancers (NMIBC) are common tumors diagnosed in older individuals and men (median age: 69 years). Immunotherapy is a treatment option in cases resistant to Bacillus-Calmette-Guerin (BCG) therapy. We aimed to evaluate the prognostic role of programmed-cell-death ligand (PD-L)-1 (PD-L1), PD-L2, and signal transducer and activator of transcription 3 (STAT3) expressions, which are closely related to immune mechanisms, in the response to BCG treatment of NMIBC. METHODS: The data of patients at the Ta and T1 stages of the cancer without muscularis propria inv

Efficacy and Safety of Keyhole Limpet Hemocyanin in Bacillus Calmette-Guérin Unresponsive Non-muscle Invasive Bladder Cancer.

BACKGROUND: Despite undergoing transurethral resection and appropriate Bacillus Calmette-Guérin (BCG) administration for high-risk non-muscle invasive bladder cancer (NMIBC), the non-responder rate is approximately 25%. Although radical cystectomy is the gold standard of treatment recommended by various international guidelines, many patients are either unwilling or ineligible for this therapeutic option. In Argentina, keyhole limpet hemocyanin (immunocyanin KLH) is approved for those patients. The primary objectives of this study were to evaluate the overall treatment response, defined as dis

Cost-per-responder analysis of TAR-200 versus other Food and Drug Administration-approved novel and generic treatments among patients with Bacillus Calmette-Guérin-unresponsive, high-risk, non-muscle-invasive bladder cancer with carcinoma in situ in the United States.

AIMS: Bacillus Calmette-Guérin (BCG)-unresponsive high-risk non-muscle-invasive bladder cancer (HR-NMIBC) with carcinoma in situ (CIS) is aggressive and treatment options are suboptimal. TAR-200, a novel intravesical drug-releasing system, received United States (US) Food and Drug Administration (FDA) approval on 09/09/2025 for this population. An economic model compared the cost per responder for US patients with BCG-unresponsive HR-NMIBC with CIS treated with TAR-200 versus other FDA-approved treatments. METHODS: A 15-month cost-per-responder model was developed from a Medicare payer perspe

Nanomotor-Assisted Intravesical Chemotherapy for Bladder Tumor Reduction and Suppression of Early Tumor Regrowth.

Nanoparticles are widely used in nanomedicine for controlled drug delivery and improved bioavailability. However, their effectiveness is often limited by passive diffusion, especially in confined, fluid-filled environments like the bladder, where rapid drug clearance and uneven distribution reduce therapeutic impact. These challenges contribute to high recurrence in bladder cancer despite intravesical chemotherapy. To address this limitation, we present urease-powered nanomotors (NM) based on mesoporous silica nanoparticles loaded with Mitomycin C (MMC), the standard chemotherapeutic for nonmu

Cost-Effectiveness of Immune Checkpoint Inhibitor Therapy Plus Bacillus Calmette-Guérin for High-Risk Nonmuscle-Invasive Bladder Cancer: Analyses of CREST, POTOMAC, and ALBAN.

PURPOSE: Three recent trials evaluated immune checkpoint inhibitors (ICIs) + bacillus Calmette-Guérin (BCG) for treatment of high-risk nonmuscle-invasive bladder cancer (NMIBC). Two of these trials (CREST, POTOMAC) demonstrated that ICI + BCG improved event-free survival compared with BCG alone but resulted in higher rates of treatment-related adverse events. In this article, we evaluated the cost-effectiveness of ICI + BCG compared with BCG alone. We then created a publicly available cost-effectiveness calculator to facilitate future NMIBC drug value comparisons. MATERIALS AND METHODS: We us

Disseminated Mycobacterium bovis-BCG Infection Following Intravesical BCG Administration in Solid Organ Transplant Recipients.

BACKGROUND: Intravesical Bacillus Calmette-Guerin (BCG) is a live attenuated vaccine for tuberculosis. Intravesical BCG (iBCG) administration is a highly effective immunotherapy for non-muscle invasive bladder cancer (NMIBC). Although live attenuated vaccines are generally contraindicated in solid organ transplant recipients (SOTR), iBCG has been utilized for NMIBC in SOTR. We describe the clinical manifestations, management, and outcomes of disseminated Mycobacterium bovis-BCG infection (Mb-BCGi) following iBCG therapy in SOTR for NMIBC. Effect of extending the period from oral administration of 5-aminolevulinic acid hydrochloride to photodynamic diagnosis during transurethral resection for non-muscle invasive bladder cancer on diagnostic accuracy and safety: a single-arm multicenter phase III trial.

BACKGROUND: In Japan, the authorized period (2-4 h) between oral administration of 5-aminolevulinic acid hydrochloride (5-ALA) and transurethral resection for non-muscle invasive bladder cancer (NMIBC) may restrict photodynamic diagnosis (PDD) usage. Therefore, this prospective, single-arm, phase III study aimed to evaluate the diagnostic accuracy and safety of PDD at an extended administration period (4-8 h). METHODS: From January 2022 to May 2023, 161 patients with NMIBC were enrolled from eight hospitals. The primary endpoint was the blue light (BL) sensitivity of pathologically positive b

Presence of an Artificial Intelligence-powered Predictive Biomarker Is Associated with a Poor Response to Intravesical Bacillus Calmette-Guerin but Not to Intravesical Sequential Gemcitabine/Docetaxel in Patients with High-grade Non-muscle-invasive Bladder Cancer.

Intravesical bacillus Calmette-Guerin (BCG) is considered first-line adjuvant therapy for high-risk or high-grade non-muscle-invasive bladder cancer (NMIBC). Recently, sequential intravesical gemcitabine and docetaxel (Gem/Doce) has emerged as a promising alternative to intravesical BCG. Biomarkers to select the optimal treatment regimen could facilitate clinical decision-making. The Computational Histologic Artificial Intelligence (CHAI) platform was previously used to develop an artificial intelligence-augmented histologic assay (CHAI biomarker) that identified patients with NMIBC at an incr
