Bugs and Beyond: A Case of Chronic Osteomyelitis.

Osteomyelitis is the infection of bone that can be either acute or chronic. Infection occurs through hematogenous spread and bacterial seeding from a distant site of infection, growth of nearby infected tissue, or direct inoculation from trauma or surgery. Hematogenous spread is more common in children and can cause spontaneous osteomyelitis. Chronic osteomyelitis is a rare occurrence that is often prevented by prompt diagnosis and treatment. This case report demonstrates a rare occurrence of chronic osteomyelitis with osteonecrosis and pathological fracture in a young patient with delayed dia…

Tubercular osteomyelitis of the distal radius in infancy.

Osteoarticular tuberculosis (TB) of long bones is an uncommon manifestation and a frequently challenging diagnosis due to the absence of typical symptoms. We report a case of an infant with tubercular osteomyelitis involving the distal radial metaphysis. He presented with a lytic lesion in the distal radius, which was treated with decompression, debridement and biopsy. The TB was diagnosed by detecting Mycobacterium tuberculosis (low) on cartridge-based nucleic acid amplification test, and histopathological analysis verified the diagnosis. Anti-tubercular medication treatment improved radiolog…

Primary sternal osteomyelitis in an immunocompetent adolescent.

Primary sternal osteomyelitis is an uncommon but challenging condition that frequently poses diagnostic difficulties. Imaging plays a crucial role in both diagnosing the condition and assessing potential complications. Treatment typically involves a combination of surgical drainage and a prolonged course of antibiotics. We report the case of a 17-year-old immunocompetent male who presented with chest pain. Laboratory investigations revealed elevated inflammatory markers, and blood cultures, along with contrast-enhanced computed tomography, confirmed the diagnosis of primary sternal osteomyelit…

Blood to the Bone: Epistaxis Leading to Osteomyelitis.

Apixaban is a direct Factor Xa inhibitor that has been increasingly prescribed, particularly for stroke prevention in atrial fibrillation. While safer than warfarin, it still carries bleeding risks, including epistaxis, which is commonly managed with nasal packing. We report a 75-year-old woman on apixaban who developed methicillin-resistant Staphylococcus aureus (MRSA) bacteremia after nasal packing for epistaxis. She presented a few weeks later with worsening back pain and was diagnosed with L2-L3 vertebral osteomyelitis from MRSA. The case highlights the rare risk of distant osteomyelitis f…

Diagnostic and Management Hurdles in Pelvic Osteomyelitis.

Pelvic osteomyelitis (PO) is a rare, debilitating bone infection. It often arises insidiously, posing diagnostic and therapeutic challenges. We present a 74-year-old male patient with a history of diabetes mellitus, hypertension, and prostate cancer post-radiation therapy (RT), who presented with dysuria, groin swelling, and inner thigh pain. Imaging revealed a pubovesical fistula, small retropubic abscesses, and findings consistent with PO. Prior cultures showed extended-spectrum beta-lactamase (ESBL) Klebsiella pneumoniae and Candida glabrata. Despite the patient's refusal of urinary diversi…

Imaging and clinical features of emphysematous osteomyelitis.

OBJECTIVE: To characterize the clinical and imaging features of emphysematous osteomyelitis (EO) in a single-center cohort and to expand upon existing literature, which is largely limited to isolated case reports. MATERIALS AND METHODS: This retrospective, IRB-approved study included patients identified through a PACS search for "intraosseous gas" or "intraosseous air" on CT or MRI from January 2014 to May 2025. Patients were excluded if intraosseous gas was attributable to trauma, open wounds, penetrating injury, or recent surgery. Clinical, laboratory, imaging, microbiologic, and histopatho…

Osteomyelitis Masquerading as Cellulitis: A Case Report.

Osteomyelitis, a significant global healthcare issue, often results from infections related to open fractures, surgery, or conditions like diabetic foot ulcers. This report describes a case of osteomyelitis in a 62-year-old female with various pre-existing health conditions. The patient initially presented with swelling, pain, and difficulty walking in the right lower extremity, accompanied by systemic symptoms. Despite an initial diagnosis of cellulitis and treatment with ceftriaxone, a subsequent CT scan revealed a pretibial abscess and confirmed osteomyelitis caused by pan-sensitive Escheri…

Tuberculous Osteomyelitis of Calcaneum: A Rare Case.

Tuberculosis (TB) is a major public health challenge in rapidly developing economies. TB commonly affects the lungs but, in rare cases, can spread to other organs such as intestines, bones, and lymph nodes. Tuberculous osteomyelitis (OM) of calcaneum in adults is rarely encountered and not very commonly reported in the literature. Most of the cases are either misdiagnosed or treating physicians often face significant delays in diagnosing Tuberculous OM of the calcaneum. We report the case of a female in her mid-thirties who had a known case of pulmonary TB for the past 10 years with tuberculou…

Cryptococcal Osteomyelitis of the Patella: A Case Report.

CASE: A 38-year-old woman presented to our clinic with right knee pain and difficulty ambulating. After cultures were obtained, a cryptococcal infection of the patella was identified. Subsequent workup revealed previously undiagnosed HIV/AIDS and a disseminated cryptococcal infection. CONCLUSION: Over a 20-month course, the patient was treated with fluconazole and antiretroviral therapy with very limited medication compliance. The disseminated infection caused cutaneous, hepatic, and cerebral complications. Eventually, compliance improved, and a final procedure to obtain post-treatment cultur

Hydrogel-Based Strategies for the Management of Osteomyelitis.

Osteomyelitis is a type of bone infection caused by bacteria, with Staphylococcus sepsis being responsible for most cases. Osteomyelitis treatment generally requires a multifaceted approach that may include intervention of surgery and administration of antibacterial agents, where several materials have been utilized as delivery vehicles for antibiotics and other antibacterial materials. Hydrogel has become a popular candidate for osteomyelitis treatment due to its biocompatibility, water-containing porous structure, and adaptable physicochemical properties. In this review, we discuss several h…

Emphysematous osteomyelitis: a rare and aggressive disease.

Emphysematous osteomyelitis is an extremely rare entity consisting of the presence of intraosseous gas that can extend to the joints and adjacent soft tissues. It is an aggressive infectious process associated with high mortality, especially in patients with risk factors such as tumors or diabetes mellitus. Because early diagnosis and immediate treatment are crucial to prevent the potentially devastating consequences of this condition, imaging tests such as computed tomography play a fundamental role in its diagnosis and management. Therefore, radiologists must be aware that intraosseous gas i…

Abscess pulsatility: a sonographic sign of osteomyelitis.

INTRODUCTION: Early diagnosis and aggressive treatment of acute osteomyelitis may improve prognosis and prevent further complications. Sonography is useful in the evaluation of osteomyelitis. It can demonstrate early signs of inflammation, such as soft tissue changes near the affected bone, periosteal thickening, periosteal elevation, and subperiosteal abscess. CASE PRESENTATION: A 68-year-old female presented to the emergency department with 3 weeks of worsening left lower extremity pain. She was initially seen by urgent care for left shin erythema and swelling and treated for cellulitis wit…

Tubercular osteomyelitis of calcaneum in an immunocompetent adult.

Tuberculosis (TB) affecting calcaneum is relatively rare in immunocompetent adults. Due to its nonspecific presentation and the absence of constitutional symptoms of TB, diagnosis is often delayed. The authors present a case of TB of calcaneum in a young male. A 20-year-old male presented with persistent pain and mild swelling of the right heel for 6 months. Upon evaluation with radiographs, a lytic lesion was noted in the posteromedial aspect of the right calcaneum. Magnetic resonance imaging was done and was reported as subacute osteomyelitis with Brodie's abscess. An open biopsy was perform…

Mycobacterium abscessus Complex Osteomyelitis of the Wrist.

Mycobacterium abscessus (M. abscessus) is a ubiquitous, rapidly growing non-tuberculous mycobacterium, which is known to cause life-threatening lung infections in immunocompromised individuals following exposure to contaminated injectable products. We report a case of M. abscessus osteomyelitis of the right wrist in a 28-year-old patient with a history of intravenous drug use and a recent surgical repair of the right radial artery pseudoaneurysm. The patient underwent surgical debridement of the right distal radius infection. Histopathological examination and culture of the debrided tissue rev…
