AUY954, a selective S1P(1) modulator, prevents experimental autoimmune neuritis.

Experimental autoimmune neuritis (EAN) is a T cell-mediated autoimmune inflammatory demyelinating disease of the peripheral nervous system and an animal model of human inflammatory demyelinating polyradiculoneuropathy. AUY954, which targets selectively the sphingosine-1-phosphate receptor 1 (S1P(1)), is known to sequester lymphocytes into secondary lymphoid tissues. In EAN rats, AUY954 greatly prevented paraparesis if administrated from the day of immunization. T cell, B cell, and macrophage infiltration, inflammatory demyelination, and local expression of interleukine-17 and matrix metallopro…

Unusual neurophysiological and immunological findings in myasthenia gravis: a case report.

We describe the case of a male patient with ocular myasthenia gravis who developed a diabetic neuropathy similar to chronic inflammatory demyelinating polyradiculoneuropathy associated with transient generalized 'myokymic' discharges and distal weakness. He had antibodies against acetylcholine receptor and GQ1b ganglioside, but not anti-voltage-gated K(+) channel antibodies. Serial electrophysiological and immunological findings showed that diabetes was involved in the immune-mediated mechanism of peripheral neuropathy. We hypothesize that the concomitant appearance of distal motor weakness an…

Treatment of immune-mediated inflammatory neuropathies.

Experimental models have suggested potential new treatments for human inflammatory neuropathy, but current practice is largely based on empirical trials. Evidence from randomized trials supports the use of intravenous immunoglobulin in Guillain-Barré syndrome, chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and multifocal motor neuropathy with conduction block (MMNCB). In Guillain-Barré syndrome and CIDP intravenous immunoglobulin is equivalent to but more convenient than plasma exchange. In MMNCB adequate comparative studies of intravenous immunoglobulin and plasma exchange h…

Clinical features and outcome of Guillain-Barre syndrome in Saudi Arabia: a multicenter, retrospective study.

BACKGROUND: Guillain-Barre syndrome (GBS) is an inflammatory polyradiculoneuropathy characterized by rapidly evolving weakness and areflexia, reaching nadir within 4 weeks. Data on the characteristic of GBS in Saudi Arabia are limited. This study aimed to describe the clinical, electrophysiological, and laboratory characteristics and outcome of a multicenter cohort of patients with GBS. METHODS: This is a retrospective multicenter nationwide study. Patients who had GBS, identified through Brighton Criteria, between January 2015 and December 2019 were included. Data collected included demograp…

A case of Guillain-Barre syndrome after the second dose of AstraZeneca COVID-19 vaccination.

Coronavirus disease 2019 (COVID-19) is a novel virus that primarily involves the respiratory system. Due to the COVID-19 pandemic, an extensive vaccination program is underway worldwide. Herein, we present a 68-year-old woman with paresthesia of both hands associated with gait instability, which started three to four days after receiving the second dose of Oxford/AstraZeneca vaccine against the COVID-19 infection. The acute inflammatory demyelinating polyradiculoneuropathy subtype of the Guillain-Barre syndrome, which is the most common subtype, was diagnosed. Regardless of the beneficial effe…

Autoantibodies in immune-mediated inflammatory neuropathies.

Inflammatory neuropathies are a rare and heterogeneous group of diseases of the nervous system characterized by the dysfunction and damage of different structures of the peripheral nerves. This group includes Guillain-Barré syndrome, chronic demyelinating inflammatory polyradiculoneuropathy, multifocal motor neuropathy or neuropathies associated with monoclonal gammopathy. The aetiology of these diseases is unknown, but B cells and autoantibodies play a key role in their pathogenesis. Autoantibodies against peripheral nerve molecules such as gangliosides, proteins of the Ranvier node or myelin…

[Bickerstaff syndrome: Case report].

Guillain-Barré syndrome is defined as an acute polyradiculoneuropathy, with sudden onset and its origin being mostly autoimmune. It is characterized by flaccid paralysis, symmetrical and ascending, together with areflexia, with or without sensory disturbances. It is the primary cause of acute flaccid paralysis in previously healthy children. Guillain-Barré syndrome presents different variants as part of the same spectrum. One of this is the Bickerstaff syndrome, characterized by ataxia, encephalopathy, hyperreflexia and external ophthalmoplegia. Early diagnosis is important with the view to es…

Demyelinating neuropathy and autoimmune hemolytic anemia in a patient with pancreatic cancer.

We herein report the case of a patient with pancreatic cancer who manifested features of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and autoimmune hemolytic anemia (AIHA). A 78-year-old Japanese man presented with AIHA and was treated with steroids and splenectomy. Although the AIHA improved following splenectomy, the patient suffered from sensorimotor neuropathy soon after undergoing surgery. The electrophysiological features indicated demyelinating neuropathy. The neuropathy was refractory to immunomodulatory treatment, and intensive investigations revealed pancreatic c…

Embolic Ischemic Stroke Associated with Maintenance Intravenous Immunoglobulin Therapy.

We describe the case of a 51-year-old man who experienced an embolic ischemic stroke during long-term maintenance of intravenous immunoglobulin (IVIG) therapy for chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). At 43 years of age, he was diagnosed with CIDP and had tolerated IVIG well. Immediately after completing the 112th infusion, he exhibited acute right-hand clumsiness. Brain magnetic resonance imaging revealed an acute embolic infarction, and neither risk factors for ischemic stroke nor alternative embolic sources were identified. Although IVIG is generally considered s…

CSF CXCL13 is elevated in patients with CIDP and may reflect higher disease activity.

To evaluate B-cell involvement in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), 11 patients with CIDP, 8 patients with Guillain-Barré syndrome and 13 patients with idiopathic normal pressure hydrocephalus (iNPH) were studied. CSF cytokine and chemokine (IL-10, IL-15, TNF-α, TGF-β1, GM-CSF, BAFF, CXCL10, and CXCL13) levels were measured by ELISA. The CSF CXCL13 level was significantly higher in patients with CIDP than in those with iNPH. The CSF CXCL13 level was significantly higher in CIDP patients with higher annualized relapse rates and higher modified Rankin scale scores…

Serological study using glycoarray for detecting antibodies to glycolipids and glycolipid complexes in immune-mediated neuropathies.

We performed a serological investigation using glycoarray in Guillain-Barré syndrome (GBS), chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), and multifocal motor neuropathy (MMN). Antibodies to 10 glycolipids and 45 glycolipid complexes were tested. Anti-GM1/sulfatide and anti-GA1/sulfatide IgG antibodies were common in GBS (20.0% and 19.0%, respectively). Anti-GQ1b/sulfatide IgG antibody was detected in 14.0% of GBS patients. IgG antibodies to antigens containing GQ1b were significantly correlated with ophthalmoplegia in GBS (p<0.01). IgM antibodies to antigens containing GM1…

Evidence-based treatment of chronic immune-mediated neuropathies.

Immune-mediated neuropathies are treatable disorders that can result in considerable disability in some cases. Chronic immune-mediated neuropathies include chronic inflammatory demyelinating polyradiculoneuropathy, multifocal motor neuropathy and neuropathy associated with monoclonal gammopathy. In recent years, therapeutic strategies for these disorders have become clearer as a result of large, randomized controlled trials. However, the treatments used can have serious adverse effects and at least 30% of patients do not respond to conventional therapies. The aim of this paper is to review ava…

Relapsing chronic low-dose corticosteroid-responsive multifocal motor neuropathy with conduction block.

Multifocal Motor Neuropathy (MMN) is a newly described disorder that might mimics the Motor Neuron Disease (MND). MMN is considered by some authors to be an entity distinct from Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIDP), nevertheless because a pure-motor variant of CIDP is well recognized and CIDP can present Partial Motor Conduction Block (PMCB), several authors believe that MMN can be a variant of CIDP. We describe a 27-year old male with a relapsing chronic low-dose corticosteroid-responsive MMN with conduction block. Our case stresses that MMN includes a wide range o…

Cranial magnetic resonance imaging in chronic demyelinating polyneuropathy.

Twenty one patients with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and five patients with chronic demyelinating polyneuropathy associated with benign monoclonal paraproteinaemia none of whom had signs or symptoms of central nervous system disease, had cranial magnetic resonance imaging (MRI) on a 1.5 Tesla unit. Areas of increased white matter signal intensity were seen in one of 10 patients aged less than 50 years and in five of 16 patients aged more than 50 years. In only two of the patients (8%), neither of whom had paraproteinaemia, did the appearance strongly sugges…

A Rare Presentation of Guillain-Barré Syndrome With Associated Horner Syndrome: A Case Report.

Guillain-Barré syndrome (GBS) is an acute inflammatory polyradiculoneuropathy involving the peripheral nervous system. Autonomic dysfunctions are well-known complications of GBS and are major contributors to mortality. Autonomic dysfunctions are classically described during the acute phase of illness. In the literature, Horner syndrome as a manifestation of GBS has been reported in very few cases. Here, we describe a case of GBS with an acute presentation of flaccid paraparesis associated with unilateral Horner syndrome. Detecting the cause of acute flaccid paraparesis with unilateral Horner s…

Subdural haematoma, the great imitator, mimicking acute spinal cord lesion.

Acute non-traumatic paraparesis is usually caused by vascular, inflammatory or neoplastic myelopathies; however, it is sometimes caused by non-myelopathic pathologies, including polyradiculoneuropathies, myopathies, psychogenic aetiologies or parasagittal cortical pathologies. A 73-year-old woman reported weakness of the bilateral lower limbs and urinary incontinence. Together with the sensory level at the left T6 dermatome, we initially considered thoracic myelopathy as the most likely diagnosis. However, MRI of the cervicothoracic cord was negative and subsequent cranial CT revealed a bilate

Contribution of electron microscopy to the study of neuropathies associated with an IgG monoclonal paraproteinemia.

A typical monoclonal IgG dysglobulinemia whether benign (monoclonal gammopathy of undetermined significance, MGUS) or malignant can give rise to peripheral neuropathy by damaging nerves. At first, neurotoxicity of the chemotherapy if the patient is treated must be ruled out in such cases. Indeed, a variety of other mechanisms have been described: endoneurial deposits of immunoglobulin, infiltration of the immunoglobulin within myelin sheaths, POEMS syndrome, deposits of amyloid, chronic inflammatory demyelinating polyradiculoneuropathy and infiltration of malignant cells. Ultrastructural exami…

Diagnosis and treatment in inflammatory neuropathies.

The inflammatory neuropathies are a large diverse group of immune-mediated neuropathies that are amenable to treatment and may be reversible. Their accurate diagnosis is essential for informing the patient of the likely course and prognosis of the disease, informing the treating physician of the appropriate therapy and informing the scientific community of the results of well-targeted, designed and performed clinical trials. With the advent of biological therapies able to manipulate the immune response more specifically, an understanding of the pathogenesis of these conditions is increasingly
