[Clinical investigations of five patients with intracranial lesions presenting tinnitus].

We investigated 5 patients with intracranial lesions. The initial complaint was tinnitus with normal hearing or sight sensorineural hearing loss. The lesions were 2 cerebello-pontine angle meningiomas, 1 cerebello-pontine angle epidermoid cyst, 1 supratentorial falx meningioma and 1 dural arteriovenous malformation at the temporal bone. Because of pulsatile or severe persistent tinnitus, we performed CT and MRI to determine the presence of intracranial lesions. Some patients had an episode of cerebral infarction and the complaint of headache or head heaviness. Quantitative measurement of tinni

Computational Hemoacoustic Investigation for Phonoangiography-Based Rupture Prediction in Compliant Fusiform Abdominal Aortic Aneurysm.

For sound signal-based diagnosis and rupture prediction of abdominal aortic aneurysms (AAA), this study performs a physiological fluid flexible-structure acoustic interaction (FfSAI) analysis for pulsatile blood-flow using an in-house solver. The presence of murmurs is computationally presented for the first time using a qualitative indicator of the acoustic signal. A pulsatile Newtonian blood-flow at the inlet, with Womersley number Wo $$ Wo $$  = 16.5, is considered. For a fusiform (axisymmetric) AAA, a parametric FfSAI study is presented with various height ( H $$ H $$ ) to diameter (

Unraveling the fluid mechanical mechanisms of sound production in pulsatile tinnitus using high-fidelity CFD.

Venous pulsatile tinnitus (PT) is sound associated with abnormal blood flow near auditory structures in the head, which can impact mental health and may be associated with intracranial hypertension. High-frequency flow instabilities, vortical flow, and regions of high time-averaged wall shear stress (TAWSS) have previously been associated with the sounds of PT, but do not identify potential mechanisms for sound production. In this study, the spectral power index of the wall pressure ( SPI P ) was used to determine the locations where sound could be transmitted through the walls of cerebral ve

Effect of Timings of the Lavare Cycle on the Ventricular Washout in an In Vitro Flow Visualization Setup.

Left ventricular assist devices inherently alter the intraventricular flow field and create areas of blood stasis with potential thrombus formation. The Lavare cycle of the Medtronic HeartWare HVAD was designed to improve ventricular washout. This study aims to evaluate its effects on ventricular washout in a pulsatile in vitro setting with a focus on the timing of pump speed changes. Ventricular flow fields were obtained via particle image velocimetry in two modes: With constant left ventricular assist devices speed and with the Lavare cycle applied. The start of the Lavare cycle was shifted

Radiological assessment of hydrocephalus: new theories and implications for therapy.

It is almost a century since Dandy made the first experimental studies on hydrocephalus, but its underlying mechanism has been unknown up to now. The conventional view is that cerebrospinal fluid (CSF) malabsorption due to hindrance of the CSF circulation causes either obstructive or communicating hydrocephalus. Analyses of the intracranial hydrodynamics related to the pulse pressure show that this is an over-simplification. The new hydrodynamic concept presented here divides hydrocephalus into two main groups, acute hydrocephalus and chronic hydrocephalus. It is still accepted that acute hydr

Endoscopic observation of a slit-valve mechanism in a suprasellar prepontine arachnoid cyst: case report.

OBJECTIVE AND IMPORTANCE: There are several theories regarding the genesis of arachnoid cysts. However, controversy continues over what mechanisms are involved in the formation of the cysts. CLINICAL PRESENTATION: A 6-year-old female patient presented with precocious puberty. The results of a neurological examination were unremarkable. Magnetic resonance imaging revealed a large suprasellar prepontine arachnoid cyst. INTERVENTION: An endoscopic ventriculocystostomy via a right frontal burr hole was performed. During inspection of the cyst, we clearly observed a slit-valve mechanism that was

Imaging of tinnitus: a review.

Tinnitus, a buzzing or ringing in the ear, may be pulsatile or continuous (nonpulsatile). The distinction, with a detailed clinical evaluation, determines the most appropriate imaging study. Pulsatile tinnitus suggests a vascular neoplasm, vascular anomaly, or vascular malformation. Most of the neoplasms are glomus tympanicum and glomus jugulare tumors. Vascular anomalies may cause pulsatile tinnitus, but the mechanism is unknown, and another (treatable) cause should be sought. Most neoplasms and anomalies are best seen on bone algorithm computed tomographic (CT) studies. Dural vascular malfor

Facilitating noncardiac surgery for the patient with left ventricular assist device: A guide for the anesthesiologist.

The introduction of left ventricular assist device (LVAD) has improved survival rates for patients with end-stage heart failure. Two categories of VADs exist: one generates pulsatile flow and the other produces nonpulsatile continuous flow. Survival is better for patients with continuous-flow LVADs. With improved survival, more of such patients now present for noncardiac surgery (NCS). This review, written for the general anesthesiologists, addresses the perioperative considerations when the patient undergoes NCS. For best outcomes, a multidisciplinary approach is essential in perioperative ma…

Relationship between arterial and venous Doppler and perinatal outcome in fetal growth restriction.

OBJECTIVE: The aim of this investigation was to assess the relationship between abnormal arterial and venous Doppler findings and perinatal outcome in fetuses with intrauterine growth restriction (IUGR). METHODS: Doppler velocimetry of the umbilical artery (UA), middle cerebral artery (MCA), inferior vena cava (IVC), ductus venosus (DV) and free umbilical vein was performed in 121 IUGR fetuses with a UA pulsatility index (PI) > 2 SD above the gestational age mean and subsequent birth weight < 10th centile for gestational age. Groups based on the last Doppler exam were: 1 = abnormal UA-PI only

Co-occurrence of cerebral arteriovenous malformation and jugular bulb dehiscence as the etiology of pulsatile tinnitus: A case report.

Pulsatile tinnitus is a unique auditory condition that may be associated with cerebrovascular or cranial structural abnormalities such as arteriovenous malformations (AVMs) and jugular bulb dehiscence (JBD). AVMs involve high-flow arteriovenous shunting, while JBD reflects a bony defect exposing the jugular bulb to the middle ear. Both conditions can cause vascular tinnitus, and their co-occurrence can pose clinical challenges. The authors present the case of an 84-year-old male with a 3-month history of left-sided pulsatile tinnitus relieved by manual compression of the ipsilateral internal j

Clinical use of the abiomed BVS 5000 as a pulsatile extracorporeal membrane oxygenation unit.

The traditional extracorporeal membrane oxygenation circuit uses a centrifugal pump. These pumps require close monitoring and are subject to complications. In addition, they do not take advantage of the potential benefits of pulsatile flow. These extracorporeal membrane oxygenation circuits use a single pump with an inline oxygenator. If cardiac failure persists after respiratory recovery has occurred, removal of the oxygenator requires an additional procedure to convert the patient to biventricular support. This report describes a circuit in which an oxygenator is connected to a pulsatile ven…

Reversed end-diastolic flow in the middle cerebral artery preceding death in a normally grown fetus.

BACKGROUND: Reversed diastolic flow in the middle cerebral artery has been described as a terminal sign in the growth-restricted fetus. The implication of this in a normally grown fetus is unknown. CASE: Middle cerebral artery Doppler velocimetry was performed at 28 weeks of gestation in the setting of decreased fetal movement and a category 2 fetal heart tracing. The middle cerebral artery pulsatility index was elevated, and follow-up studies revealed reversed flow. The fetus had appropriate growth, with mild polyhydramnios and no signs of cardiomegaly, effusion, or hydrops. Fetal death was

Constrictive pericarditis: portal, splenic, and femoral venous Doppler pulsatility: a case series.

PURPOSE: Pulsatile flow of the portal vein has been implicated as an indicator of right ventricular dysfunction in cardiac patients. In patients with significantly elevated right atrial pressure, pulsatile venous flow may be transmitted to the portal, splenic, renal, and femoral veins. We describe the evolution of these echocardiographic findings in four patients with constrictive pericarditis (CP) undergoing pericardiectomy with simultaneous hemodynamic waveform and cerebral oximetry monitoring in the operating room and in the intensive care unit. CLINICAL FEATURES: Patient 1 presented class…

The benefits of pulsatile machine perfusion of standard criteria deceased donor kidneys at a geographically remote transplant center.

Benefits of pulsatile machine perfusion (pumping) of standard criteria donor (SCD) kidneys are unclear. Our center is located 4½ hours from our Organ Procurement Organization. We evaluated outcomes of pumping SCD kidneys under such circumstances by conducting a retrospective examination of all SCD kidneys transplanted between January 2007 and March 2012, comparing kidneys pumped (28 [group 1]) versus standard cold storage (77 [group 2]). Group 1 had fewer delayed graft function (DGF, 3.57% vs. 23.38%, p = 0.02) and slow graft function (SGF, 7.14% vs. 24.68%; p = 0.047) and faster serum creatin

Effect of pulsatility on the mathematical modeling of rotary blood pumps.

In this study, the effect of time derivatives of flow rate and rotational speed was investigated on the mathematical modeling of a rotary blood pump (RBP). The basic model estimates the pressure head of the pump as a dependent variable using measured flow and speed as predictive variables. Performance of the model was evaluated by adding time derivative terms for flow and speed. First, to create a realistic working condition, the Levitronix CentriMag RBP was implanted in a sheep. All parameters from the model were physically measured and digitally acquired over a wide range of conditions, incl

Use of k-space segmentation in MR velocity mapping for rapid quantification of CSF flow.

In this work, a k-space segmentation technique for quantitative studies of pulsatile cerebrospinal fluid (CSF) flow is suggested. Three k-space lines are sampled for each of two interleaved gradient-echo sequences (velocity-compensated and velocity-encoded) within each repetition interval. Nine cardiac phases are obtained at a heart rate of 60 bpm with maintained nominal resolution and a factor of 3 in reduction of acquisition time relative to our conventional nonsegmented flow quantification protocol. Segmented and conventional sequences were compared in phantoms, in healthy volunteers, and i
