BACKGROUND: 4-part proximal humerus fractures are complex injuries that are often associated with comminution of the greater tuberosity. The purpose of this study is to evaluate the functional outcomes of 4-part humerus fractures that are treated with reverse total shoulder arthroplasty (rTSA) and correlate these outcomes with the healing status of the greater tuberosity.
MATERIAL AND METHODS: A retrospective observational study was performed including 65 patients who underwent rTSA following a 4-part proximal humerus fracture. These patients were categorized into 3 groups according to the he

PURPOSE: Reverse total shoulder arthroplasty (RSA) has revolutionized the management of proximal humerus fractures (PHF) in the elderly patients. There is few or no consensus regarding to management of postoperative rehabilitation in elderly patients. An early rehabilitation from D1 allowed better functional results compared to rehabilitation started to D30 independently from tuberosities consolidation.
METHODS: 94 patients operated on for PHF were evaluated retrospectively, with a minimum radio-clinical follow-up of 2 years. Clinical evaluation included mobilities and four functional scores:

BACKGROUND: Acromial (ASF) and scapular spine (SSF) stress fractures are well-recognized complications of reverse shoulder arthroplasty (RSA), but much of the current data are derived from single-center or single-implant studies with limited generalizability. This study from the American Shoulder and Elbow Surgeons (ASES) Complications of Reverse Shoulder Arthroplasty Multicenter Research Group determined the incidence of ASF/SSF after RSA and identified preoperative patient characteristics associated with their occurrence.

INTRODUCTION: The optimal tuberosity repair method in reverse total shoulder arthroplasty for fracture is unknown.
METHODS: Eight matched pairs of cadaver shoulders were randomly assigned to a stem-based tuberosity repair technique or a nonstem-based repair (Boileau technique) and mechanically tested with a 10 kN load cell. Cyclic loading was performed between 10 and 100 N for 500 cycles at 1 Hz, followed by static pull to failure at 33 mm/s. Ultimate load was determined from the maximum load reached during the pull to failure. A paired Student t-test was used to compare the means of the ulti

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is a widely used treatment for both traumatic and degenerative glenohumeral joint diseases. Despite its popularity, data on outcomes sorted according to indications have revealed inconsistent results. Furthermore, the clinical outcome is rarely analysed in a differentiated manner using several scores. This retrospective, single-center study aimed to investigate the impact of indications on the clinical outcomes of RTSA.
METHODS: The Constant Score, EQ-5D-5L, DASH Score were sent by post to 263 patients who received a RTSA between February

BACKGROUND: Insufficient tuberosity healing is the most common reason for poor outcome after treatment of proximal humerus fractures (PHFs) using hemiarthroplasty (HA). In these cases, revision to reverse total shoulder arthroplasty (RTSA) can improve function and reduce pain in the short term, however, long-term results remain scarce. Aim of this study was to evaluate the clinical and radiological mid- to long-term results in patients with a revision RTSA after failed HA for PHF.
METHODS: In this retrospective study all patients that received a revision to RTSA after failed fracture HA betwe

The objective of this study was to investigate whether frailty is associated with functional outcomes, motion, and reoperation at a minimum of 2 years after reverse total shoulder arthroplasty (rTSA) for proximal humerus fracture. We performed a retrospective study of 153 patients who underwent rTSA for proximal humerus fracture at two level 1 trauma centers from 2003 to 2018 with minimum 2-year follow-up. Frailty was assessed using the modified 5-item frailty index (mFI). The primary outcome variable was the American Shoulder and Elbow Surgeons (ASES) shoulder score at minimum 2-year follow-u

BACKGROUND Posterior shoulder dislocation is an uncommon injury, accounting for less than 5% of all glenohumeral dislocations. Bilateral locked posterior shoulder fracture-dislocation is an extremely rare entity with an incidence of 0.6 cases/100 000 people/year. A few reports have been published in the literature, predominantly associated with epileptic seizures. CASE REPORT We present a rare case of a 55-year-old male patient with a history of epileptic seizures, who presented with bilateral, locked, posterior fracture-dislocation of the shoulder following an epileptic seizure. The patient w

The successful treatment of proximal humeral fractures remains challenging for shoulder surgeons, and failure rates are high, regardless of initial treatment. This study aimed to analyze the clinical and radiographic midterm results of onlay lateralized cementless stem reverse shoulder arthroplasty (RSA) in patients with valgus/varus malunion proximal humerus fracture sequelae without metaphyseal osteotomy. We retrospectively studied 35 cases with the diagnosis of fracture sequelae of the proximal part of the humerus with valgus/varus malunion. The mean duration of follow-up was 4.6 years (ran

PURPOSE: Open reduction and internal fixation with a locking plate are performed frequently to treat fractures of the proximal humerus. Avascular necrosis and non-union or malunion are potential complications of this procedure, which lead to specific fracture sequelae. The aim of this study was to investigate the clinical and radiological results of patients treated by removal of a failed locking plate of the proximal humerus and simultaneous implantation of a reverse total shoulder prosthesis.
METHODS: Twenty-one patients (f = 17, m = 4; mean age 70 years) out of 29 patients were available f

AIMS: The reasons for failure of a hemirthroplasty (HA) when used to treat a proximal humeral fracture include displaced or necrotic tuberosities, insufficient metaphyseal bone-stock, and rotator cuff tears. Reverse total shoulder arthroplasty (rTSA) is often the only remaining form of treatment in these patients. The aim of this study was to evaluate the clinical outcome after conversions from a failed HA to rTSA.
MATERIAL AND METHODS: A total of 35 patients, in whom a HA, as treatment for a fracture of the proximal humerus, had failed, underwent conversion to a rTSA. A total of 28 were avai

PURPOSE: The role of reverse total shoulder arthroplasty (RTSA) for three and four-part proximal humerus fractures is evolving. However, there does not appear to be a clear consensus amongst surgeons. The purpose of this study is to further define the standard of care, assessing surgeon preference and treatment considerations for management of such fractures.
METHODS: Orthopaedic surgeons were surveyed on their training, practice setting, and experience regarding management of four-part proximal humerus fractures. The survey also presented five representative cases to assess treatment prefere

PURPOSE: Greater tuberosity (GT) fragment healing has been associated with improved outcomes following reverse total shoulder arthroplasty (RTSA) for proximal humerus fractures, yet the relationship between pre-operative GT fragment volume and clinical outcomes remains largely unexplored. We hypothesised that larger GT volumes would be associated with improved outcomes and larger anatomical dimensions.
METHODS: We retrospectively studied 153 patients who underwent RTSA for proximal humerus fracture. GT fragment volume was measured on pre-operative computed tomography (CT). Clinical outcomes i

BACKGROUND: The molecular and histologic characteristics associated with the development of post-traumatic capsular fibrosis of the shoulder joint following open reduction and internal fixation (ORIF) of proximal humerus fractures (PHF) remain incompletely understood. To help elucidate these mechanisms, this hypothesis-generating pilot study aimed to examine gene expression and histological changes in the joint capsule of patients undergoing implant removal and adhesiolysis for post-traumatic stiffness after surgical fixation of PHF.
METHODS: For this pilot study, tissue samples were collecte

BACKGROUND: Proximal humeral fractures (PHFs) are a significant healthcare burden, representing the third most common osteoporotic fracture in older adults. Although open reduction and internal fixation (ORIF) with the Proximal Humeral Internal Locking System (PHILOS) provides a reliable rate of osseous union, postoperative complications such as shoulder stiffness and residual pain remain prevalent. Current rehabilitation protocols are predominantly glenohumeral-centric and often overlook fracture-associated scapular dyskinesis. This randomized controlled trial (RCT) aims to evaluate whether i
