[A case of bilateral recurrent laryngeal nerve palsy after thyroid surgery under intraoperative nerve monitoring].

A 66-year-old woman suffering from Basedow disease had total thyroidectomy under intraoperative monitoring (IOM) of recurrent laryngeal nerve (RLN) using Medtronic Xomed Nerve Integrity Monitor-2 (Medotronic, Mineapolice, Minesota, USA). IOM indicated a positive signal for her right RLN while the signal for her left RLN disappeared during the operation. During the surgery, surgeons identified her left RLN which was anatomically intact. During the attempted extubation of the endotracheal tube, the patient experienced severe dyspnea resulting in re-intubation. Fiber-optic examination revealed bi

Paradoxical vocal cord motion: an important cause of stridor.

Paradoxical vocal cord motion (PVCM) is an important cause of laryngeal stridor and dysphonia; however, only two previous cases have been reported. We report three additional cases, one of which was tracheotomized on two occasions before the diagnosis was made. These patients, typically young females who have had a recent upper respiratory infection, present with stridor. They can phonate weakly, but they cannot cough. Indirect laryngoscopy reveals smooth, symmetric vocal cord adduction on inspiration and abduction on expiration. Laboratory values are normal. PVCM appears to be self-limited (u

Cervical Aggressive Fibromatosis Causing Airway Obstruction.

Fibromatosis or desmoid tumors are rare benign fibroblastic lesions that are rarely present in the head and neck regions. When they do occur in these regions, however, they tend to be aggressive toward the surrounding tissue and be associated with heavy morbidity and mortality. We report the case of a 26-year-old Tunisian female who presented with acute obstructive dyspnea and a 3-week history of cervical swelling. The swelling was initially only located in the left submandibular area, it then gradually extended to all the anterior cervical supra- and infrahyoid regions causing a clinical pres

Video-assisted transcervical-transtracheal repair of posterior wall laceration of thoracic trachea: A new approach. Case Report.

Iatrogenic tracheal lacerations are a rare but potentially fatal event. In selected acute cases, surgery plays a key role. Treatment can be conservative, for lacerations of less than 3 cm; surgical or endoscopic, depending on the size and location of the lesion and fan efficiency. There is no clear indication of the use of any of these approaches and the decision is therefore linked to local expertise. We present an emblematic clinical case of a 79 years old female patient undergoing polytrauma as a result of a road accident, without neurological damage, which required intubation and subsequen

Immediate postoperative non-invasive positive pressure ventilation following midface microvascular free flap reconstruction.

BACKGROUND: There is a rare need for postoperative non-invasive positive pressure ventilation (NIPPV) following microvascular reconstruction of the head and neck. In midface reconstruction, the free flap vascular pedicle is especially vulnerable to the compressive forces of positive pressure delivery. CASE: A 60 year old female with Amyotrophic Lateral Sclerosis (ALS) presented with squamous cell carcinoma of the anterior maxilla, for which she underwent infrastructure maxillectomy and fibula free flap reconstruction. To avoid tracheotomy, the patient was extubated postoperatively and transit

Aerodynamic, acoustic and functional results of posterior transverse laser cordotomy for bilateral abductor vocal fold paralysis.

Bilateral abductor vocal fold paralysis (BAVFP) is a rare but life-threatening condition which may require an emergency tracheotomy procedure. The ideal surgical technique for this condition should improve quality of life by relieving the airway obstruction while preserving laryngeal functions such as phonation and deglutition. Posterior transverse laser cordotomy (PTLC) was first described by Dennis and Kashima as a technique for providing an airway at the posterior glottis without pre-operative tracheotomy; they reported it as a successful method with satisfactory functional results. The aim

Tolerance of the Passy-Muir Speaking Valve in infants and children less than 2 years of age.

Research literature confirms the intuitive reasoning that tracheotomy may adversely influence speech acquisition in infants and children. The Passy-Muir Speaking Valve (PMSV) permits inspiration through the tracheotomy stoma and expiration through the glottis, allowing for phonation. Although adults with tracheostomies have demonstrated the ability to speak using the valve without respiratory compromise, there is scant literature regarding the use of this valve in the infant or child population. A retrospective review of 64 charts was conducted to examine documented evidence of tolerance of th

Malignant laryngotracheal obstruction: a way to treat serial stenoses of the upper airways.

BACKGROUND: Six patients known to have inoperable esophageal carcinoma presented with stridor due to both malignant tracheal stenosis (n = 6) and bilateral vocal cord paralysis. Two patients also had respiratory-digestive fistula. METHODS: Patency was restored by endotracheal stenting plus unilateral cordectomy. Four patients had immediate relief. Two patients required enlargement of the cord incision. One of them declined reoperation and underwent tracheotomy. RESULTS: Stent function was uneventful. There was no dislodgement or mucous impaction. Fistula seal was complete. There was no aspir

[Airway management in tracheal stenosis caused by malignant goiter].

We report a case of malignant goiter with severe tracheal stenosis. The patient was a 61-year-old female, who had orthopnea on admission. Radiological examinations revealed a tracheal stenosis extending from 4.5 cm to 8 cm below the glottis; the smallest caliber being 5 mm. On the seventh day after admission, the patient complained of dyspnea even while sitting. An emergency tracheotomy was scheduled. While the patient was awake in a sitting position, a fiberscope was inserted endotracheally to confirm the intact inner surface of the trachea, and then an armored endotracheal tube (outer diamet

[Severe congenital laryngeal stridor. Endoscopic surgical treatment].

Congenital laryngeal stridor or laryngomalacia is a congenital disease causing an usually shrill and solitary inspiratory noise, sometimes associated with disorders of deglutition and dyspnea when crying. Most often, the symptoms spontaneously disappear before the age of two. However, some cases are very severe, with permanent dyspnea, leading to tracheal intubation or tracheotomy. To avoid the risks of prolonged tracheotomy in infants, a new surgical technique has recently been described, beside epiglottectomy and hyomandibulopexy, now abandoned. This is simple endoscopic section and resectio

Congenital bilateral recurrent nerve paralysis and ptosis: a new syndrome?

Bilateral vocal cord paralysis is a rare, but not unheard of, occurrence in the perinatal period. It is often incompatible with life unless the problem is recognized promptly, the child intubated, and--following this--a tracheotomy performed. Bilateral congenital ptosis is a very rare occurrence and is almost always associated with either significant central nervous system deficit or with other oculomotor problems. The combination of congenital bilateral recurrent nerve paralysis and congenital ptosis has not been previously reported in the English language literature. The author has had the o
