Waardenburg syndrome type 2: an orthodontic perspective.

Waardenburg syndrome is a rare form of neurocristopathy. It is a disorder in the development of neural crest cells, caused by an altered cellular migration during the embryonic phase. That alteration causes an association of different abnormalities such as pigmentary disturbances of the hair, iris, skin, stria vascularis of the cochlea, dystopia canthorum and sensorineural hearing loss. We report a case of a 14-year-old Romanian male, with a family history of Waardenburg syndrome (mother) and Usher syndrome (father - congenitally sensorineural hearing loss and retinal degeneration). The case p

[Multicenter Clinical Study of Vibrant Soundbridge in Japan: Analysis of Subjective Questionnaires].

The Vibrant Soundbridge (VSB) is an active middle ear implant with the Floating Mass Transducer (FMT). We performed a multicenter study to study the efficacy of the VSB by means of "the 10 Questionnaire on Hearing 2002" and "the APHAB questionnaire" at 13 hospitals between 2011 and 2013. In all, 23 patients with mixed or conductive hearing loss received VSB implantation by the round window placement technique. These individuals were generally unable to use, or gained little from conventional hearing aids or bone conduction hearing aids. Two questionnaires were administrated before the surgery

Experimental study on admissible forces at the incudomalleolar joint.

HYPOTHESIS: The forces that cause rupture of the incudomalleolar joint during the fixation of stapedial prostheses can be determined by means of load-deflection measurements at the long process of the incus. As in other tissues, 3 ranges of forces can be defined: micro rupture, rupture, and short-term maximum. BACKGROUND: A crucial step in stapes surgery is the attachment of the stapedial prosthesis to the long process of the incus. It is unknown which forces occur during the crimping process that increase the risk of damage to the incudomalleolar joint or incus luxation. The goal of this stu

Implantable bone conduction hearing device: Audiant bone conductor. Update on our experiences.

An implantable hearing device has been developed and has undergone testing by animal experimentation and clinical trials in humans. Recently, the Food and Drug Administration has approved the use of this device in adults who are within the criteria of its utilization, mainly patients with conductive hearing impairments who have good inner ear reception and who are unable to benefit from surgical correction. This update describes observations and experiences we have had during the device's development and defines its limitations as well as its unique thrust into the future relief of hearing imp

Cholinergic urticaria following active forced-air warming in the post-anesthesia care unit: a case report and literature review.

INTRODUCTION: Cholinergic urticaria (CU), a subtype of chronic inducible urticaria, is triggered by elevations in core body temperature. Postoperative shivering and active warming measures may increase core body temperature in the post-anesthesia care unit (PACU), yet CU in this setting remains underrecognized and has rarely been reported. CASE PRESENTATION: We report a case of an 18-year-old male who developed acute CU in the PACU following cochlear implantation under general anesthesia. After extubation, the patient exhibited postoperative shivering and was treated with a forced-air warming…

Pharmacotherapeutic Interventions for Sensorineural Hearing Loss: A Scoping Review.

BACKGROUND/OBJECTIVES: Sensorineural hearing loss (SNHL) is a chronic condition with no established pharmacological treatment. Recent advanc … r treating SNHL caused by cisplatin, aminoglycosides, noise-induced ototoxicity, and intraoperative cochlear implantation trauma. Emerging regenerative and gene therapies show promise as future pharmacologic treatment options. CONCLUSIONS: Pharmacologic therapies for SNHL are promising but remain constrained by small sample sizes, heterogeneous study designs, and drug …

Impact of Intraoperative Patient-Specific 3D-Printed Temporal Bones: A Randomized Controlled Trial.

OBJECTIVE: To determine if intraoperative inspection of patient-specific 3D-printed TB models, as a supplement to standard imaging, affects the cognitive load of surgeons during cochlear implantation (CI). METHODS: This randomized controlled trial included 40 consecutive adult primary CI surgeries performed by seven attending and five trainee surgeons at a large CI center. Cases were randomized 1:1 to current standard with surgical planning based on imaging review (controls) or to current standard plus intra-operative inspection of a 3D-printed model (intervention). The primary outcome, intra…

It is characterized by … ee patients, including blindness due to optic nerve compression and profound hearing loss requiring cochlear implantation. One patient had concurrent bladder cancer, making the differential diagnosis challenging. Six patients had vitamin D deficiency or insufficiency. Evidence‑informed decision aid for fitness‑to‑dive assessment after otologic surgery.

INTRODUCTION: Fitness-to-dive after otologic surgery is often approached conservatively, with some procedures historically labelled as absol … urgery were commonly described as potentially compatible in appropriately selected individuals. For cochlear implantation, guidance was mainly conditional and based on hyperbaric testing, limited clinical diving reports, and manufacturer-specified pressure or depth limits. Communication emerged as an additional practical consideration in cases of significant hearing lo …

Comparison of Superficial Cervical Plexus Block and Intravenous Morphine for Analgesia in Tympanomastoid Surgeries: A Randomized Double-Blind Control Trial.

Background and aims Superficial cervical plexus block (SCPB) has been indicated for many neck surgeries including thyroid surgery, carotid endarterectomy, clavicular surgeries, and cochlear implantation, but has not been used perioperatively to provide analgesia for otological surgeries. We hypothesized that the use of this block is not inferior to the use of intravenous morphine for postoperative analgesia. The primary objective was to compare the postoperative Visual Analogue Scale (VAS) scores in those receiving SCPB versus morphine. Secondary objectives included intraoperative hemodynamic …

Diagnostic and therapeutic applications of genomic medicine in progressive, late-onset, nonsyndromic sensorineural hearing loss.

The progressive, late-onset, nonsyndromic, sensorineural hearing loss (PNSHL) is the most common cause of sensory impairment globally, with … of presbycusis ranges from 25 to 75%. Current therapies for PNSHL range from sound amplification to cochlear implantation (CI). PNSHL is an excellent candidate for genomic medicine approaches as it is common, has well-described pathophysiology, has a wide time window for treatment, and is amenable to local gene therapy by currently utilized procedural approaches. AD PN …

Role of pepsin and pepsinogen: linking laryngopharyngeal reflux with otitis media with effusion in children.

OBJECTIVES/HYPOTHESIS: To analyze the relationship between laryngopharyngeal reflux (LPR) represented by pepsin and pepsinogen, and pathogenesis of otitis media with effusion (OME). STUDY DESIGN: Prospective case-control study. METHODS: Children with OME who required adenoidectomy and tympanostomy/tympanostomy tubes placement were enrolled in OME group, whereas children with adenoid hypertrophy (AH) who required adenoidectomy and individuals who required cochlear implantation (CI) were enrolled in AH and CI groups, respectively. Sudden bilateral sensorineural hearing loss following polysubstance narcotic overdose.

BACKGROUND: Auditory disorders associated with substance abuse are rare. Hearing loss secondary to heroin and hydrocodone abuse has been described variously as not always responsive to steroid management, as not always reversible, and in some cases, as nonresponsive profound sensorineural hearing loss requiring cochlear implantation. We present a case of a teenager with sudden-onset moderate to severe bilateral sensorineural hearing loss after documented polysubstance "binging." The hearing loss improved substantially after high-dose steroid and vasoactive therapy. PURPOSE: The purpose of thi…
