Prehospital triage for mass casualty incidents using the META method for early surgical assessment: retrospective validation of a hospital trauma registry.

BACKGROUND: In mass casualty incidents (MCI), death usually occurs within the first few hours and thus early transfer to a trauma centre can be crucial in selected cases. However, most triage systems designed to prioritize the transfer to hospital of these patients do not assess the need for surgery, in part due to inconclusive evidence regarding the value of such an assessment. Therefore, the aim of the present study was to evaluate the capacity of a new triage system-the Prehospital Advanced Triage Method (META)-to identify victims who could benefit from urgent surgical assessment in case of

VATS phrenic nerve harvesting for brachial plexus neurotization: literature review and our experience.

INTRODUCTION: Brachial plexus traumatic lesions often lead to severe upper extremity deficits that dramatically compromise quality of life of mostly young patients. Optimal treatment aims to restore elbow flexion transferring various donor nerves. Phrenic nerve (PN) is a powerful source of transferable axons and, despite supraclavicular sectioning being the most used technique, it can be harvested through video-assisted thoracoscopic surgery (VATS). EVIDENCE ACQUISITION: About PN harvesting, less than 20 articles were found in Literature. Most of them are clinical case-reports or case-series

Factors associated with outcomes after reverse total shoulder arthroplasty at midterm follow-up.

BACKGROUND: Reverse total shoulder arthroplasty has become the preferred treatment for many patients with irreparable cuff tears, osteoarthritis, and complex proximal humeral fractures. While outcomes are generally favorable, the influence of demographic and surgical factors-such as age, sex, previous surgery, and dominance of the operated arm-on post-operative results remains incompletely understood. METHODS: After institutional review board approval, a retrospective review of prospectively collected data was conducted for all primary reverse total shoulder arthroplasties performed by a sing

Comparison of variable and fixed angle proximal humeral locking plates for the treatment of displaced proximal humerus fractures.

INTRODUCTION: Controversy surrounds the optimal surgical management of proximal humerus fractures (PHFs). The aim of this study was to evaluate and compare the anatomic and clinical outcomes of open reduction internal fixation (ORIF) of PHFs using FA or VA locking plates. METHODS: This was a retrospective study of 85 patients (19 male, mean age 60.5 ± 14 years) with displaced surgical neck PHFs treated with VA (44 patients) or FA (41 patients) locking plates. Inclusion criteria were a minimum of 1 year postoperative follow up (mean 3.1 years) or earlier revision surgery. Outcome measures incl

Comparison of locking plate alone and locking plate combined with 3D printed polymethylmethacrylate augmentation in treating proximal humerus fractures in elderly.

BACKGROUND: Locking plates are widely used in open reduction internal fixation for proximal humeral fracture (PHF). However, the optimal surgical treatment of unstable, displaced PHF in elderly patients remains controversial. This study aimed to compare the radiological and clinical outcomes of surgical treatment of PHF in the elderly with locking plate (LP) alone and LP combined with 3D printed polymethylmethacrylate (PMMA) prosthesis augmentation (LP-PA). METHODS: From May 2015 to April 2021, a total of 97 patients aged ≥60 years with acute unstable PHF who underwent osteosynthesis with eit

Allograft-Prosthetic Composite Reconstruction for Massive Proximal Humeral Bone Loss in Reverse Shoulder Arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) performed in the setting of massive proximal humeral bone loss often requires special reconstructive techniques. Restoration of the proximal part of the humerus with an allograft provides a number of theoretical benefits, including implant support, restoration of humeral length, deltoid tensioning, and an opportunity to repair the posterior aspect of the cuff to improve strength in external rotation and repair of the subscapularis to improve stability. However, reverse allograft-prosthesis composites (APCs) are costly, are technically dema

Complications associated with the use of corticosteroids in the treatment of athletic injuries.

BACKGROUND: The potent anti-inflammatory pharmacologic effects of the corticosteroids (cortisone and synthetic derivatives) has led to their extensive usage in the management of rheumatologic diseases and athletic musculoskeletal injuries. The efficacy and risks of locally injected or systemically administered corticosteroids in the treatment of athletic injuries are unclear. OBJECTIVE: To review critically the medical literature and determine complications and risks associated with corticosteroid treatment of athletic injuries. DATA SOURCES: A search of 3 databases-MEDLINE, CINAHL, and Coch

Coonrad-Morrey total elbow arthroplasty for tumours of the distal humerus and elbow.

We reviewed 20 patients who had undergone a Coonrad-Morrey total elbow arthroplasty after resection of a primary or metastatic tumour from the elbow or distal humerus between 1980 and 2002. Eighteen patients underwent reconstruction for palliative treatment with restoration of function after intralesional surgery and two after excision of a primary bone tumour. The mean follow-up was 30 months (1 to 192). Five patients (25%) were alive at the final follow-up; 14 (70%) had died of their disease and one of unrelated causes. Local control was achieved in 15 patients (75%). The mean Mayo Elbow Per

Total shoulder arthroplasty versus hemiarthroplasty for humeral head avascular necrosis: a meta-analysis.

BACKGROUND: Avascular necrosis (AVN) of the humeral head can progress to joint collapse and marked functional limitation, often requiring surgical treatment. Total shoulder arthroplasty (TSA) and hemiarthroplasty (HA) are the main surgical treatment options, but their comparative clinical value remains a topic of debate. In particular, uncertainty persists regarding how functional benefit, complication risk, and implant durability should be balanced in contemporary practice. In this meta-analysis, we compared functional outcomes, complication rates, and revision risk between TSA and HA in pati

Intramedullary Unicortical Button and All-Suture Anchors Provide Similar Maximum Strength for Onlay Distal Biceps Tendon Repair.

PURPOSE: To evaluate the biomechanical profile of onlay distal biceps repair with an intramedullary unicortical button versus all-suture anchors under cyclic loading and maximal load to failure. METHODS: Twenty paired fresh-frozen human cadaveric elbows were randomized to onlay distal biceps repair with either a single intramedullary button or with two 1.35-mm all-suture anchors. A 1.3-mm high tensile strength tape was used in a Krackow stitch to suture the tendons in both groups. Specimens and repair constructs were loaded for 3,000 cycles and then loaded to failure. Maximum load to failure,

Reverse shoulder arthroplasty in patients younger than 65 years, minimum 5-year follow-up.

BACKGROUND: Indications for reverse total shoulder arthroplasty (RTSA) are expanding, and more young patients are undergoing RTSA. Younger patients are expected to place increased functional demands on their shoulder, which may affect implant performance and longevity. Reports on longer-term outcomes in young patients remain limited. This study evaluates the minimum 5-year functional outcomes of RTSA in patients younger than 65 years. METHODS: A retrospective review was performed using a multinational prospective shoulder arthroplasty database of a single implant system, Exactech Equinoxe (Ga

Comparison of Surgical Encounter Direct Costs for Three Methods of Cubital Tunnel Decompression.

BACKGROUND: In situ decompression, subcutaneous transposition, and submuscular transposition for cubital tunnel syndrome have historically yielded similar outcomes. The authors' null hypothesis is that no differences exist in surgical encounter total direct costs for in situ decompression, subcutaneous transposition, and submuscular transposition. METHODS: Adult patients treated surgically for cubital tunnel syndrome by four fellowship-trained hand surgeons between August of 2011 and December of 2016 were identified by CPT code (64718) at their tertiary academic institution. Distal biceps tendon injuries--current treatment options.

Three percent of all biceps tendon ruptures occur at the distal aspect, where the tendon inserts into the radial tuberosity. Distal bicep tendon ruptures typically occur in middle-aged males after an eccentric extension load is applied to the elbow. Patients usually complain of a sudden, sharp, and painful tearing sensation in the antecubital region, with a palpable defect. The biceps squeeze and hook tests are specific maneuvers by which to diagnose distal biceps ruptures on physical examination. Magnetic resonance imaging (MRI) or ultrasound maybe be helpful to distinguish between partial an

Clinical application of basic research on continuous passive motion for disorders and injuries of synovial joints: a preliminary report of a feasibility study.

Since 1970, when the concept of continuous passive motion (CPM) was originated by one of the authors (R.B.S.), he and a succession of his research fellows have investigated its biological effects on the healing and regeneration of articular tissues in a wide variety of experimental models in rabbits. From this basic research he concluded that CPM is well tolerated, seems to be painless, stimulates the healing and regeneration of articular tissues, prevents joint stiffness, and permits the normal healing of arthrotomy incisions. Beginning in 1975, one of the authors (H.W.H.), and in 1978, the r

Variation in trabecular bone microarchitecture across rhesus macaque (Macaca mulatta) load-bearing joints.

Globally, human population structure is quickly trending older, increasing the prevalence and systemic burden of age-related skeletal disorders such as osteoporosis. Osteoporosis is characterized by the loss of bone mass, including trabecular bone tissue, leading to skeletal fracture. Although clinically important, fundamental questions remain about normal trabecular bone variation and age-related bone loss. In this study, we use free-ranging rhesus macaques (Macaca mulatta) from the Cayo Santiago Field Station to explore variation in trabecular bone structure. We measured several aspects of t
