Modalities and Complications Associated With the Use of High-Flow Nasal Cannula: Experience in a Pediatric ICU.

BACKGROUND: High-flow nasal cannula (HFNC) therapy is increasingly used in pediatric ICUs as an intermediate level of support between conventional oxygen delivery and noninvasive ventilation (NIV). The safety of HFNC has seldom been studied, and some cases of barotrauma have been reported. This retrospective study aims to describe HFNC use in a tertiary care pediatric ICU, with a focus on the complications associated with this therapy. METHODS: Between January 2013 and January 2014, all children <18 y old treated with HFNC in the pediatric ICU were included. Demographic data, HFNC settings, c…

Percutaneous central venous catheters versus peripheral cannulae for delivery of parenteral nutrition in neonates.

BACKGROUND: Neonatal parenteral nutrition may be delivered via peripheral cannulas or central venous catheters (umbilical or percutaneous). As the result of complications associated with umbilical catheters, many neonatal units prefer to use percutaneous catheters after initial stabilisation. Although they can be difficult to place, these catheters may be more stable than peripheral cannulae and require less frequent replacement. These delivery methods may be associated with different risks of adverse events, including acquired invasive infection and extravasation injury. OBJECTIVES: To deter…

Safety and Efficacy of High-Flow Nasal Cannula Therapy in Preterm Infants: A Meta-analysis.

BACKGROUND AND OBJECTIVE: High-flow therapy is the most recent, and popular, mode of respiratory support in neonates. However, the evidence supporting its efficacy and safety has not yet been established. We conducted a systematic review and meta-analysis of clinical trials comparing efficacy and safety of high-flow therapy compared with other modes of noninvasive ventilation (NIV) in preterm infants. METHODS: Articles were indexed by using Medline, Embase, Scopus, OpenSIGLE, Health Management Information Consortium, and Cochrane Central Register of Controlled Trials. Randomized or quasi-rand

High-flow nasal cannula and extubation success in the premature infant: a comparison of two modalities.

OBJECTIVE: To compare the effectiveness of Fisher and Paykel (FP) and Vapotherm (VT) high-flow nasal cannula (HFNC) in preventing reintubation either within 72 h or <7 days after extubation of premature infants. The primary outcome was the rate of extubation failure defined as reintubation within 72 h. Secondary outcomes included reintubation after ≤7 days. STUDY DESIGN: This was a prospective, randomized pilot study comparing the extubation success of 40 infants born between 26 and 29 weeks of gestation. RESULT: The rate of extubation failure at 72 h was 18% for FP and 9% for VT. The failur…

Percutaneous central venous catheters versus peripheral cannulae for delivery of parenteral nutrition in neonates.

BACKGROUND: Parenteral nutrition for neonates may be delivered via a short peripheral cannula or a central venous catheter. The latter may either be inserted via the umbilicus or percutaneously. Because of the complications associated with umbilical venous catheter use, many neonatal units prefer to use percutaneously inserted catheters following the initial stabilisation period. The method of parenteral nutrition delivery may affect nutrient input and consequently growth and development. Although potentially more difficult to place, percutaneous central venous catheters may be more stable tha…

Thrombosis of the arterial cannula during extracorporeal membrane oxygenation in a full-term newborn infant.

The authors report a rare case of cannula thrombosis during extracorporeal membrane oxygenation (ECMO). A full-term newborn infant was successively placed on single-cannula veno-venous extracorporeal lung support and then on veno-arterial ECMO, because of persistent pulmonary hypertension. At 140 hours of ECMO, the infant displayed general cyanosis except in the right arm. Since asymmetric hypoxemia during ECMO may be related either to cannula malposition or to a tip thrombosis, a chest x-ray after contrast injection into the arterial line of the circuit was performed. It showed an opacificati…

High-flow nasal cannula oxygen during breaks from noninvasive ventilation after extubation: an observational study.

PURPOSE: Clinical practice guidelines recommend prophylactic noninvasive ventilation (NIV) after extubation in high-risk patients. The effects of high-flow nasal cannula (HFNC) oxygen during breaks from NIV after extubation have been poorly explored. Our objective was to assess whether HFNC rather than standard oxygen during breaks from NIV may prevent extubation failure. METHODS: Observational study based on two multicenter clinical trials including patients at high-risk of extubation failure (> 65 years or with underlying cardiac or respiratory disease). We included the 1077 patients who re…

High-Flow Nasal Cannula in Hypercapnic Respiratory Failure: An Updated Systematic Review and Meta-Analysis.

BACKGROUND: High-flow nasal cannula (HFNC) and bi-level positive airway pressure (BiPAP) are both employed in the management of acute hypercapnic respiratory failure, yet their comparative benefits remain uncertain. We conducted an updated systematic review and meta-analysis of randomized controlled trials (RCTs) to compare efficacy, safety, and tolerability of HFNC versus BiPAP in adults with hypercapnic respiratory failure. METHODS: We searched PubMed, Web of Science, the Cochrane Central Register of Controlled Trials (CENTRAL), CNKI, and Wanfang Data through June 2025 for RCTs enrolling pa…

Management of hypercapnic acute respiratory failure with high-flow nasal cannula therapy: A narrative review.

Hypercapnic acute respiratory failure (ARF) is a life-threatning condition and a leading cause of hospital admission, mainly in acute exacerbation of chronic obstructive pulmonary disease (COPD). Although non-invasive ventilation (NIV) currently represents the first-line ventilatory strategy in patients exhibiting hypercapnic ARF, NIV may fail primarily due to worsening of ARF, patient-ventilator asynchrony, interface intolerance, even cardiovascular instability. Consequently, the operating principles and physiological effects of HFNC could be interesting and useful for applying this technique

High flow nasal cannula in the emergency department's resuscitation room: a prospective monocenter descriptive study.

BACKGROUND: High-flow nasal cannula (HFNC) therapy is commonly used in emergency departments (EDs) to treat acute respiratory failure (ARF). However, its utilization patterns in the resuscitation room (RR) are not well documented. The aim of this study was to describe patient characteristics, management, and in-hospital trajectories according to HFNC use. METHODS: We conducted a prospective, monocenter, cohort study of consecutive adults admitted to the RR with ARF requiring ≥ 9 L/min of oxygen. We recorded demographic data, comorbidities, clinical and biological parameters, imaging results, t…

The New Cannula Aspiration Technology to Treat Complete Lens Dislocation: A Prospective Randomized Clinical Trial.

BACKGROUND AND OBJECTIVE: This pilot study aimed to evaluate the outcomes of a novel large-bore cannula aspiration technology developed in our laboratory in comparison to phacofragmentation for addressing complete lens dislocation. PATIENTS AND METHODS: Thirty-seven eyes with complete lens dislocation were randomly assigned to two groups. In both groups, standard 3-port pars plana vitrectomy (PPV) was performed to remove the vitreous. The dislocated lens was removed by phacofragmentation or the large-bore cannula aspiration technology, based on the random grouping. The "cannula" aspiration in…

Clinical assessment of cannula performance during adult minimally invasive valve surgery using predictive mathematical models.

OBJECTIVES: Peripheral cannula selection in minimally invasive cardiac surgery (MICS) is crucial, as venous drainage limitations during cardiopulmonary bypass (CPB) can impair end-organ perfusion and overall outcomes. This study evaluates the in vivo venous drainage efficiency of different peripheral venous cannulas for adult MICS by applying dynamic similarity (DS) prediction formulas. METHODS: This observational study included 75 adult MICS patients with use of single peripheral venous cannulation from January 2017 to June 2023. The analysis included cannulas Bio-Medicus Multi XL Life Suppo…

Respiratory Therapists' Attitudes and Beliefs Regarding High Flow Nasal Cannula Use at End-of-Life.

CONTEXT: Respiratory therapists (RTs) are frequently involved in managing high-flow nasal cannula (HFNC) at end-of-life (EOL). However, few studies have examined RTs' attitudes and beliefs about EOL HFNC management, which have important clinical, logistical, and ethical implications for providing goal-concordant care. OBJECTIVES: To determine whether RTs agree patients' goals for comfort-focused care affects their management of HFNC at EOL. METHODS: We conducted an anonymous survey of RTs at our institution between October and November 2024. This 32-question survey consisted of four sections…

The Effect of Simulated Mouth Opening on CO2 Removal During High Flow Nasal Cannula Therapy.

BACKGROUND: High-flow nasal cannula (HFNC) therapy was originally considered only for its targeted O2 delivery but has been shown to also assist in the removal of end-expiratory CO2. Variables that influence the capability of this modality to reduce end-expiratory CO2 are being proposed and investigated. Two types of HFNC therapy were precisely modeled at numerous mouth openings to investigate the effect that mouth open percent (MOP) had on airway CO2 reduction (flush) and upper airway pressure generation for each therapy. METHODS: Computational fluid dynamics models were run at MOPs of 0%, 2…

Iatrogenic Aortic Regurgitation Caused by Arterial Cannula Tip Rotation During Cardiopulmonary Bypass: A Case Report.

Iatrogenic aortic regurgitation (AR) after ventricular septal defect (VSD) closure occurs because of the distortion of the aortic valve or leaflet perforation. We presented a case of a 1-month-old child who had iatrogenic AR after VSD closure caused by the rotation of the arterial cannula tip toward the aortic valve during cardiopulmonary bypass. Transesophageal echocardiography (TEE) detected a retrograde jet flow from the arterial cannula to the aortic valve, causing eccentric AR. Our case shows a new differential diagnosis of iatrogenic AR after VSD closure and TEE's usefulness with color D…

Inserting a VA-ECMO cannula through an inferior vena cava filter during extracorporeal cardiopulmonary resuscitation.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) has been utilized to treat massive pulmonary embolism (PE) accompanied by cardiac arrest or refractory cardiogenic shock. Our team opted for a femoral-femoral approach for vascular cannulation, using drainage and return cannulas in the common femoral vein and artery, respectively. However, femoral venous cannulation can be limited or challenging due to the presence of thrombus in the inferior vena cava (IVC), making the insertion of the drainage cannula via the femoral vein difficult. We present the case of a patient with massive PE wh…
