Electroencephalographic correlates of pain catastrophizing in chronic musculoskeletal pain: a scoping review.

INTRO: Individuals with chronic musculoskeletal pain (CMP) and high pain catastrophizing demonstrate functional and structural brain changes via functional magnetic resonance imaging that may serve as useful neural correlates. Yet, few studies take advantage of electroencephalography (EEG), a more cost effective and accessible tool. Utilizing EEG to further establish clinical characteristics of pain catastrophizing may assist in developing novel interventions that modulate brain activity and expand the depth at which CMP is treated. METHODS: Search strings were entered into PubMed, ProQuest C

Capacity to manufacture key pharmaceuticals in New Zealand after a global catastrophe.

INTRODUCTION: Human civilisation faces global catastrophic risks such as: nuclear war, bioengineered pandemics, major solar storms and a volcanic winter. For some of these catastrophes, island nations may have relative survival potential but any collapse in international trade could also end critical imported goods such as pharmaceuticals. We aimed to explore the latter in New Zealand, a highly trade-dependent island nation. METHODS: We identified the 10 most extensively prescribed pharmaceuticals in New Zealand that can be used for acute treatment (by annual prescription numbers). Based on m

Effect of parental anxiety and catastrophizing on post-tonsillectomy opioid consumption in children.

BACKGROUND: The impact of parental psychological factors on post-operative pain management in children is not well understood, but is important. The aim of this study was to determine the relationship between parental anxiety, pain catastrophizing about child pain, and opioid administration in children undergoing tonsillectomy. METHODS: This was a multicenter prospective cohort study from four different centers between 2022 and 2023. All parents of children undergoing adenotonsillectomy were included. Parental levels of anxiety and pain catastrophizing were assessed using the State-Trait Anxi

Catastrophic cerebral venous sinus thrombosis secondary to fulminant invasive otomastoiditis.

A previously healthy, middle-aged immunocompetent man presented to a regional hospital with a 12-h history of right ear pain and discharge following a mild flu-like illness. He rapidly deteriorated neurologically, developing dilated pupils and seizures requiring intubation. Following urgent transfer to a quaternary ICU, multidisciplinary assessment (ENT and Neurosurgery), neuroimaging, and right ear myringotomy confirmed Otomastoiditis with catastrophic cerebral venous sinus thrombosis. CT venography demonstrated extensive thrombosis involving the right sigmoid, transverse, and superior sagitt

Delayed, catastrophic expansion of acute traumatic subdural hematomas: a case series.

BACKGROUND: Incidences of catastrophic, life-threatening expansion of acute traumatic subdural hematomas (SDH) greater than 72-hours after injury are rare and limited to single case reports in the literature. Here we report the largest series of this rare subset of patients and evaluate the incidence and associated clinical factors. METHODS: We retrospectively reviewed all asymptomatic acute traumatic SDH admissions that were managed conservatively to a single large academic tertiary trauma center over a 5-year period. Inclusion criteria were diagnosis of non-operative acute SDH, Glasgow coma

Missed Gastroesophageal Injuries During Antireflux Surgery: Infrequent but Catastrophic Complications.

Background: Laparoscopic antireflux surgery (LARS) is widely used to treat gastroesophageal reflux disease (GERD). Iatrogenic gastroesophageal injuries, when recognized intraoperatively, can be managed without major consequences, whereas undetected injuries presenting as postoperative leaks are associated with high morbidity and mortality. Despite their complexity, research on post-LARS leaks is scant. We aim to describe the diagnosis and management of such injuries at a tertiary referral center. Methods: We describe a single-center case series of patients referred for gastroesophageal perfora

Catastrophic antiphospholipid syndrome presenting with a stroke as a first presentation: case report.

Catastrophic Antiphospholipid Syndrome (CAPS), an autoimmune disease that causes multi-organ thromboses leading to their failure, is a rapidly developing form of antiphospholipid syndrome (APS). APS may be a primary disease or secondary to an autoimmune condition like Systemic Lupus Erythematosus (SLE). A 31-year-old male patient with unremarkable medical history presented with a sudden onset of left-sided body weakness including upper and lower extremities, frontal headache, and slurred speech. Diagnostic workup revealed diffuse ST elevation with elevated cardiac enzymes, elevated inflammator

Catastrophic Type A Aortic Dissection Temporally Associated With Recreational Ketamine Use.

Aortic dissection is a life-threatening vascular emergency, typically associated with hypertension, connective tissue disease, or stimulant drug use. Ketamine, a dissociative anesthetic with sympathomimetic properties, is rarely implicated in major cardiovascular events. This case report describes a 57-year-old woman with no known medical history who presented with acute altered mental status after reported intranasal ketamine use and subsequent back pain. On arrival, she was obtunded and hypertensive, requiring intubation for airway protection. CT angiography revealed a Type A aortic dissecti

WATCHMAN Gone Astray, Catastrophic Mitral Regurgitation Following Device Dislodgement.

WATCHMAN (Boston Scientific, Plymouth, MN, USA) is the most commonly utilized left atrial appendage occlusion device. Although increased operator experience has led to a significant decline in reported complications, serious and potentially fatal events can still occur. We present a case of WATCHMAN device embolization that was identified using transesophageal echocardiography shortly after device deployment, along with the subsequent management and retrieval of the device.

Getting Crunchy With COVID-19: A Unique Case of Catastrophic Multiorgan Calciphylaxis.

Calciphylaxis, or calcific uremic arteriolopathy, is a rare condition with a poorly understood pathophysiology. It is often associated with diseases that cause abnormalities in calcium metabolism, such as end-stage renal disease (ESRD) and hyperparathyroidism. While it most often affects the skin, involvement of extracutaneous organs, called systemic calciphylaxis, has been reported.Interestingly, other risk factors have now been identified with the development of calciphylaxis including recent infections, corticosteroid or warfarin use as well as diabetes mellitus, autoimmune diseases, and pr

Cardiovascular Catastrophe in Hysteroscopic Surgery: A Case of Diagnostic Dilemma, Arrest, and Recovery.

Hysteroscopic surgery carries a risk of fluid overload and electrolyte imbalance. Although acute cardiovascular collapse due to excessive fluid absorption is rare, it can be life-threatening. A 36-year-old otherwise healthy female underwent hysteroscopic myomectomy under combined spinal-epidural anesthesia. She developed sudden bradycardia, hypotension, and unresponsiveness 50 minutes into the procedure, which responded to adrenaline and brief chest compressions (<1 minute). She was intubated and achieved return of spontaneous circulation (ROSC); however, elevated airway pressures complicated

Primary catastrophic antiphospholipid syndrome in children with midbrain infarction: a case report.

BACKGROUND: Catastrophic antiphospholipid syndrome (CAPS) is a multi-system autoimmune disease characterized by extensive thrombosis. Pediatric CAPS is extremely rare and associated with a high mortality rate, especially when midbrain infarction is involved. Hence, early diagnosis and prompt initiation of appropriate treatment for CAPS complicated by midbrain infarction are of utmost importance in achieving favorable outcomes. CASE PRESENTATION: In this report, we present the case of a 14-year-old girl who presented with neurological symptoms and digestive system infection and was initially d

Nearly catastrophe coronary perforation: Is it second drug-eluting stent effective?

Coronary artery perforation (CAP) is an uncommon yet serious complication. Although severe perforations (Ellis III) have become more frequent, the overall mortality rate associated with perforations (7.5%) has decreased in recent years. Unfortunately, our medical facility cannot always access a cover stent. The aim of this case report was to demonstrate the effectiveness of using a second drug-eluting stent as an alternative and successful treatment approach in a CAP patient. This is the case of a 67-year-old female with stable angina pectoris Canadian Cardiovascular Society classification III

Eculizumab therapy and complement regulation in a case of resistant catastrophic antiphospholipid syndrome.

Catastrophic antiphospholipid syndrome (CAPS) is a life-threatening form of antiphospholipid syndrome characterised by diffuse arterial and venous thrombosis, in the presence of positive antiphospholipid antibodies. The multiple sites of thrombosis in small, medium and large vessels progress to multiorgan failure, accounting for the high mortality rate associated with CAPS. Unregulated complement activation is increasingly recognised as critical to the pathogenesis of CAPS. Early diagnosis is essential to initiate prompt life-saving treatment with the triple therapy of anticoagulation, immunos

Anlotinib synergizes with venetoclax to induce mitotic catastrophe in acute myeloid leukemia.

Venetoclax is a BCL2-targeted drug employed in treating various cancers, particularly hematologic malignancies. Venetoclax combination therapies are increasingly recognized as promising treatment strategies for acute myeloid leukemia (AML). In this study, we conducted an unbiased drug screen and identified anlotinib, a promising multi-targeted receptor tyrosine kinase inhibitor with oral activity currently utilized in the treatment of solid tumor, as a potent enhancer of venetoclax's anticancer activity in AML. Our investigation encompassed AML cell lines, primary cells, and mouse models, demo

Increased Pain Variability in Patients With Chronic Pain: A Role for Pain Catastrophizing.

Pain is an inherently negative perceptual and affective experience that acts as a warning system to protect the body from injury and illness. Pain unfolds over time and is influenced by myriad factors, making it highly dynamic. Despite this, statistical measures often treat any intraindividual variability in pain ratings as noise or error. This is consequential, especially for research on chronic pain, because pain variability is associated with greater pain severity and depression. Yet, differences in pain variability between patients with chronic pain and controls in response to acute pain h

Catastrophic Antiphospholipid Syndrome: A Review of Current Evidence and Future Management Practices.

Antiphospholipid syndrome (APS) is an autoimmune disorder characterized by blood clots and pregnancy complications due to antiphospholipid antibodies. Catastrophic APS (CAPS), a severe variant, leads to multiorgan failure and is often fatal. Pathogenesis involves antiphospholipid antibodies, particularly anti-beta-2-glycoprotein I (aβ2GPI), which trigger endothelial cell (EC) activation, cytokine release, and a prothrombotic state. Infections, surgeries, and other triggers can precipitate CAPS, leading to widespread microthromboses and systemic inflammatory responses. CAPS predominantly affect
