Potentially inappropriate medications prescribing to elderly patients with advanced chronic kidney by using 2019 American Geriatrics Society Beers Criteria.

BACKGROUND AND AIMS: A potentially inappropriate medication (PIM) is defined as a drug-carrying risks outweighing the expected clinical benefits. Elderly patients with chronic kidney disease (CKD) are particularly at higher risk of drug-related toxicities. In Lebanon, no studies have been conducted regarding the prescribing of PIMs in hospitalized CKD patients. This study aimed to check the prevalence of PIMs using the American Geriatrics Society (AGS) Beers criteria in elderly patients with advanced CKD stages including dialysis and to identify possible risk factors that may be associated wit…

Pain management in the perioperative orthogeriatric setting: Recommendations of the special interest group Orthogeriatrics of the German Geriatrics Society.

Management of perioperative pain in older adults can be challenging. Multimorbidity, functional decline or diminished physiological reserves, the corresponding polypharmacy, age-related pharmacodynamic and pharmacokinetic changes need to be considered. Pain assessment can be complicated by cognitive impairment and communication barriers. Older adults are at high risk of adverse consequences from surgery and undertreated pain as well as pharmacological overtreatment. Effective pain control in the perioperative period needs to be balanced against potential pharmacological harm. The aim of the or

The dominant role of geriatrics vulnerabilities and comorbidities in readmissions after colorectal surgery: Shifting from "nonmodifiable" to "actionable" risk.

BACKGROUND: Unplanned readmission within 30 days following colorectal surgery occurs in up to 20% of older patients (≥65 years), representing a significant clinical and economic burden. Designing effective interventions to reduce readmission depends on whether risk factors are modifiable or actionable. This study identified and categorized the drivers of readmission, with a specific focus on actionable factors. METHODS: We analyzed data from 49,021 elective colectomy and proctectomy cases aged ≥65 years and discharged home in the National Surgical Quality Improvement Program from 2016 to 2020

Integrating geriatrics into clinical training, research training, board certification, and continuing education in infectious diseases: meeting review and commentary.

Although adults aged 65 years and older constitute the most rapidly growing segment of the U.S. population, geriatric issues have not typically been a focus of training in infectious diseases (ID). Underrecognition of the unique aspects of geriatric care, apathy toward this population, and the feeling that "we're all geriatricians" (and thus know geriatric medicine) all contribute to this problem. This article summarizes the recent meeting focused on integrating geriatric principles within ID training at all levels. The ID/geriatric interface as an attractive area for basic and clinical resear

Geriatric assessment in older adults with non-Hodgkin lymphoma: A Young International Society of Geriatric Oncology (YSIOG) review paper.

Non-Hodgkin lymphoma (NHL) is a disease of older adults, with a median age at diagnosis of 67 years. Treatment in older adults with NHL is challenging. The aging process is associated with a decline in functional reserve that varies among individuals, and results in an increasing risk of treatment-related toxicity and mortality. Chronological age and performance status fail to capture the multidimensional and heterogeneous nature of the aging process. A geriatric assessment (GA) screens multiple geriatric domains and provides a more accurate assessment of functional reserve. Several abbreviate

Preoperative and Postoperative Cognitive Assessment in Geriatric Patients Following Acute Traumatic Injuries: Towards Improving Geriatric Trauma Outcomes.

INTRODUCTION: A growing percentage of the US population is over the age of 65, and geriatrics account for a large portion of trauma admissions, expected to reach nearly 40% by 2050. Cognitive status is important for operative management, especially in elderly populations. This study aims to investigate preoperative and postoperative cognitive function assessment tools in geriatric patients following acute trauma and associated outcomes, including functional status, postdischarge disposition, mortality, and hospital length of stay (H-LOS). METHODS: A literature search was conducted using Medli…

Non-Vitamin K Oral Anticoagulants in Comparison to Phenprocoumon in Geriatric and Non-Geriatric Patients with Non-Valvular Atrial Fibrillation.

Geriatric characteristics such as high age, multi-morbidity, polypharmacy and frailty are common in patients with atrial fibrillation (AF). In a retrospective study using a German claims database, effectiveness (ischaemic stroke/systemic embolism) and safety (intracerebral, gastrointestinal and major extracranial bleeding) were compared in patients with non-valvular AF starting non-vitamin K oral antagonists (NOACs) (apixaban, dabigatran and rivaroxaban) and phenprocoumon. Cox proportional hazards models were used to calculate adjusted hazard ratios, and interaction terms of the treatment grou

[Anemia in the aged - a geriatric syndrome? : Second position paper on anemia in the aged by the working group anemia of the German Geriatric Society].

Geriatric syndromes are the pathognomonic columns of geriatric medicine. In contrast to many syndromes in younger people, in geriatric patients, the chief complaint does not typically represent the specific pathological condition underlying the change in health status. Geriatric syndromes are usually highly prevalent, multicausal and share a number of common risk factors. In recent years, scientific controversy over anemia in the aged has revealed a high prevalence in geriatric patients, which prompted the "working group on anemia" to publish its first position paper at a European level. Multidimensional geriatric assessment in treatment decision in elderly cancer patients: 6-year experience in an outpatient geriatric oncology service.

This prospective cohort study of consecutive elderly cancer patients was undertaken to evaluate the role of the multidimensional geriatric assessment (MGA) as an aid in treatment decision-making. A total of 571 cancer patients (aged > or =70) were enrolled during 6-year (1999-2005). All underwent MGA as part of the first evaluation. In multivariate analysis, the probability of receiving active, instead of palliative, treatment was negatively associated with increasing age (odds ratio=0.69 every 5 years, p=0.005), living alone (OR=0.54, p=0.031), dependence in activities of daily living (ADL sc

A comparison of depressive mood of older adults in a community, nursing homes, and a geriatric hospital: factor analysis of Geriatric Depression Scale.

The Geriatric Depression Scale (GDS)-15 was used in 607 adults aged 65+ years living in a community, nursing homes, and a general hospital to explore characteristics of depressive mood in different care settings. Factor analysis of GDS-15 extracted 4 factors labeled unhappiness, apathy and anxiety, loss of hope and morale, and energy loss. The scale scores labeled unhappiness, apathy and anxiety, and loss of hope and morale were negatively correlated with the Barthel Index and the Mini-Mental State Examination scores. The results classified the depressive patterns into 2 types, one fitting the

Development of a Predictive Model for the Onset of Loneliness in Older Adults From the National Center for Geriatrics and Gerontology-Study of Geriatric Syndromes.

OBJECTIVES: Loneliness is a rising public health concern among older Japanese adults. We aimed to construct a predictive model for loneliness onset among older adults and evaluate its predictive performance. METHODS: A total of 4050 participants responded to our survey (mean follow-up period: 3.1 [range, 2.8-3.3] years). Of these, 1806 older adults (age ≥ 65 years) who were not lonely at baseline were included. Loneliness was assessed using the UCLA Loneliness Scale (Version 3). A score of ≥ 44 indicated the presence of loneliness at follow-up. Predictive models for the onset of loneliness we
