The Impact of Water, Sanitation and Hygiene Interventions to Control Cholera: A Systematic Review.

BACKGROUND AND METHODS: Cholera remains a significant threat to global public health with an estimated 100,000 deaths per year. Water, sanitation and hygiene (WASH) interventions are frequently employed to control outbreaks though evidence regarding their effectiveness is often missing. This paper presents a systematic literature review investigating the function, use and impact of WASH interventions implemented to control cholera. RESULTS: The review yielded eighteen studies and of the five studies reporting on health impact, four reported outcomes associated with water treatment at the poin

Using adenosine triphosphate bioluminescence level monitoring to identify bacterial reservoirs during two consecutive Enterococcus faecium and Staphylococcus capitis nosocomial infection outbreaks at a neonatal intensive care unit.

INTRODUCTION: This study aimed to assess the role of adenosine triphosphate (ATP) bioluminescence level monitoring for identifying reservoirs of the outbreak pathogen during two consecutive outbreaks caused by Enterococcus faecium and Staphylococcus capitis at a neonatal intensive care unit (NICU). The secondary aim was to evaluate the long-term sustainability of the infection control measures employed one year after the final intervention measures. METHODS: Two outbreaks occurred during a 53-day period in two disconnected subunits, A and B, that share the same attending physicians. ATP biolu

Hand hygiene opportunities and consumption of alcohol-based handrub across nursing homes of canton of Vaud, Switzerland.

BACKGROUND: Optimal hand hygiene (HH) is necessary to prevent healthcare-associated infections among nursing home (NH) residents. We aimed to quantify the number of HH opportunities per resident-day in NHs and assess institutions' alcohol-based hand rub (ABHR) consumption based on the HH needs of their population. METHODS: Cross-sectional study between 2022 and 2024 with the participation of 117 NHs (95%) and a random sample of 1633 residents. For each resident we accessed the daily care schedule, attributed 2-4 HH opportunities per nursing activity and collected the care dependency score. We

Longitudinal analysis of the hand microbiome in response to chlorine-based antiseptic use during a military field exercise.

Hand hygiene is essential for infection control, yet the impact of frequent antiseptic use on the skin microbiome, which is crucial for skin barrier function and pathogen exclusion, remains underexplored, especially in field conditions. In high-risk military settings, there is a need for safe, multipurpose antiseptics that avoid the drawbacks of alcohol-based options, including skin irritation, flammability, and unsuitability for wound application. We conducted a longitudinal study during the Norwegian-led military exercise Cold Response 2022 to assess the effects of repeated use of stabilized

Irritant Contact Dermatitis on Hands.

Hand hygiene (HH) is the single most important procedure health care workers (HCWs) can follow to reduce the risk of spreading health care-associated infections, yet compliance with this simple task remains at less than 50%. One of the reasons often cited for noncompliance is irritant contact dermatitis (ICD) resulting from repeated exposure to HH products and procedure. This literature review used the World Health Organization's components of empowerment as a guideline for the search and development of a clinical model to address HCW HH and ICD.

Occupational allergic contact dermatitis caused by coconut fatty acids diethanolamide.

BACKGROUND: Coconut fatty acids diethanolamide [cocamide diethanolamine (cocamide DEA)] is a surface-active derivative of coconut oil that is used in industrial, household and cosmetic products. Cocamide DEA contact allergy has been reported relatively seldom. OBJECTIVES: To describe cocamide DEA-positive patients in an occupational dermatology clinic. METHODS: We retrieved allergic reactions to cocamide DEA from test files, and studied the occupation, exposure, concomitant allergic reactions and diagnoses of the positive patients. RESULTS: Of the 2572 patients tested, 25 (1%) had an allerg

Efficacy of alcohol-based hand sanitizer on hands soiled with dirt and cooking oil.

Handwashing education and promotion are well established as effective strategies to reduce diarrhea and respiratory illness in countries around the world. However, access to reliable water supplies has been identified as an important barrier to regular handwashing in low-income countries. Alcohol-based hand sanitizer (ABHS) is an effective hand hygiene method that does not require water, but its use is not currently recommended when hands are visibly soiled. This study evaluated the efficacy of ABHS on volunteers' hands artificially contaminated with Escherichia coli in the presence of dirt (s

Assessment of alternative hand hygiene regimens to improve skin health among neonatal intensive care unit nurses.

OBJECTIVE: The frequent handwashing and gloving required in high-risk, high-volume patient care areas such as critical care units damages skin of the hands. The purpose of this exploratory study was to compare 2 hand care regimens (traditional antiseptic wash with chlorhexidine-containing detergent versus mild soap wash with subsequent alcohol-based rinse for degerming as necessary) in a neonatal intensive care unit (NICU). DESIGN: Prospective, quasi-experimental, random assignment. SETTING: One NICU (47 beds) in a New York City children's hospital. SUBJECTS: Sixteen full-time NICU nurses.

Impact of infection prevention interventions on neonatal bloodstream infections and mortality in Ethiopia.

INTRODUCTION: Bloodstream infections (BSIs) are a major cause of neonatal morbidity and mortality in low-resource settings, yet most are preventable. However, few studies evaluated the effectiveness of multimodal interventions in preventing healthcare-associated BSIs (HA-BSIs) in neonatal intensive care units (NICUs) in low- and middle-income countries (LMICs). METHODS: We conducted an implementation study in a NICU in Northern Ethiopia from January to June 2024, using a pre-and post-intervention design. Multimodal prevention strategies consisted of infection prevention and control (IPC) trai

Limited handwashing facility and associated factors in sub-Saharan Africa: pooled prevalence and multilevel analysis of 29 sub-Saharan Africa countries from demographic health survey data.

INTRODUCTION: Handwashing is fundamentally an inexpensive means of reducing the spread of communicable diseases. In developing countries, many people die due to infectious diseases that could be prevented by proper hand hygiene. The recent coronavirus (COVID-19) pandemic is a threat to people who are living in resource-limited countries including sub-Saharan Africa (SSA). Effective hand hygiene requires sufficient water from reliable sources, preferably accessible on premises, and access to handwashing facility (water and or soap) that enable hygiene behaviors. Therefore, this study aims to de

WASH Benefits Bangladesh trial: management structure for achieving high coverage in an efficacy trial.

BACKGROUND: Water, sanitation, and hygiene (WASH) efficacy trials deliver interventions to the target population under optimal conditions to estimate their effects on outcomes of interest, to inform subsequent selection for inclusion in routine programs. A systematic and intensive approach to intervention delivery is required to achieve the high-level uptake necessary to measure efficacy. We describe the intervention delivery system adopted in the WASH Benefits Bangladesh study, as part of a three-paper series on WASH Benefits Intervention Delivery and Performance. METHODS: Community Health W

Reduction in intraoperative bacterial contamination of peripheral intravenous tubing through the use of a novel device.

BACKGROUND: Hand hygiene is a vital intervention to reduce health-care associated infections, but compliance remains low. The authors hypothesized that improvements in intraoperative hand hygiene compliance would reduce transmission of bacteria to surgical patients and reduce the incidence of postsurgical healthcare-associated infections. METHODS: The authors performed a controlled before-and-after study over 2 consecutive months. One hundred fourteen operative cases were enrolled. Two predesignated sites on the anesthesia machine were selected, decontaminated, and cultured via aseptic techni
