The left ventricular outflow tract in atrioventricular septal defect revisited: surgical considerations regarding preservation of aortic valve integrity in the perspective of anatomic observations.

OBJECTIVE: The anatomy of the left ventricular outflow tract in hearts with atrioventricular septal defect has been widely investigated, but controversies remain regarding detailed aspects of left ventricular outflow tract anatomy in the perspective of operative techniques to either prevent or relieve outflow tract obstruction. METHODS: We investigated 29 postmortem hearts with an atrioventricular septal defect. Measurements were taken of the circumferences and of the widths of the components that make up the outflow tract, that is, the interventricular septum, the superior bridging leaflet,

Morphometric analysis of unbalanced common atrioventricular canal using two-dimensional echocardiography.

OBJECTIVES: This study was designed to define morphometric echocardiographic variables of unbalanced common atrioventricular canal (CAVC) that could aid in appropriate referral for surgical repair. BACKGROUND: Unbalanced CAVC has a high surgical mortality rate. This may be secondary to inappropriate referral of some patients for two-ventricle repair (closure of septal defects) instead of single-ventricle repair (Norwood palliation and Fontan operation). METHODS: The echocardiograms of 103 patients with CAVC were retrospectively reviewed. In the subcostal left anterior oblique view, the area

[Usefulness of open lung biopsy for corrective surgery on incomplete atrioventricular septal defect associated with severe pulmonary hypertension and right to left shunting].

In congenital heart defects, it is sometimes difficult to decide the operative indication in patients, who has severe pulmonary hypertension (PH) and right to left shunting, from clinical symptoms and cardiac catheterization. We here report a case of incomplete atrioventricular septal defect. The patient is 9 months old with the Down's syndrome. The cardiac catheterization showed severe PH and right to left shunting. Preoperative mean pulmonary artery pressure was 75 mmHg, pulmonary systemic pressure to systemic pressure ratio (PP/PS) was 1.17, pulmonary systemic flow to systemic flow ratio (Q

Congenital pericardial defect associated with atrial and ventricular septal defects.

Congenital pericardial defects are rare anomalies but in recent years improved diagnostic techniques have often led to a pre-operative diagnosis. The present communication describes a case of a partial pericardial defect with herniation of the lung into the pericardium associated with a lung cyst which is extremely rare. The sternum showed some deficiency in its lower portion. Atrial and ventricular septal defects with a small pericardio-peritoneal foramen were also present.

Transcatheter closure of atrial septal defects in adults with the Amplatzer septal occluder.

OBJECTIVE: To assess the efficacy and complications of device occlusion of atrial septal defects in adults, using the Amplatzer septal occluder (ASO). DESIGN: A prospective interventional study. SETTING: Paediatric cardiology departments in two European teaching hospitals. PATIENTS: The first 20 patients accepted for atrial septal defect device occlusion, on the basis of transoesophageal echocardiography. Sixteen patients had larger defects with right heart dilatation, while the primary indication for closure in four was a history of early paradoxical embolism. INTERVENTIONS: Transcatheter

Arrhythmias in Patients with Atrial Defects.

Atrial arrhythmias are common in patients with atrial septal defects. A myriad of factors are responsible for these that include remodeling related to the defect and scar created by the repair or closure. An understanding of potential arrhythmias, along with entrainment and high-density activation mapping can result in accurate diagnosis and successful ablation. Atrial fibrillation is being seen increasingly after patent foramen ovale closure and may be the primary etiology of recurrent stroke in these patients.

Size of atrial septal defect after balloon atrial septostomy does not correlate with immediate improvement in oxygenation.

OBJECTIVE: Transposition of great arteries is a common cyanotic heart defect. Balloon atrial septostomy aims to improve circulatory mixing and oxygenation. Previous studies have combined infants with intact ventricular septum and those with ventricular septal defect. Additionally, the septostomy was performed much later after birth. The objectives were to ascertain any correlation between the atrial septal defect size and oxygenation, before and after septostomy, as well the change in parameters pre-post procedure. METHODS: We performed an audit of the last 10 years of clinical and echocardio

Accuracy of electrical impedance cardiography for measuring cardiac output in children with congenital heart defects.

This study determined whether noninvasive electrical impedance cardiography accurately measures systemic blood flow (cardiac output) in children with congenital heart defects. A total of 37 patients ranging in age from 2 to 171 months underwent complete right- and left-sided heart catheterizations that included simultaneous Fick and impedance measurement of cardiac output. Based on the diagnosis, 4 groups were formed consisting of a control group (n = 11) with no shunts, a group with intracardiac left-to-right shunting and an atrial septal defect (n = 7), another with a ventricular septal defe

[Blade atrial septostomy in an infant with hypoplastic left heart syndrome awaiting heart transplantation].

Blade atrial septostomy was performed in a 36 day-old infant diagnosed with hypoplastic left heart syndrome and restrictive atrial septal defect. The patient was awaiting cardiac transplantation. The defect was enlarged using a 9.4-cm blade. The mean atrial gradient was reduced from 19 to 2mmHg. After septostomy the patient showed marked clinical improvement and aortic oxygen saturation increased from 68% to 88% with no complications. Twenty-five days later orthotopic heart transplantation was successfully performed.

Combined transcatheter closure of atrial septal defect and patent ductus arteriosus: report of two cases.

Atrial septal defect (ASD) and patent ductus arteriosus (PDA) are common congenital heart defects and usually isolate. The combination of these two defects is infrequent. Nowadays, both defects are amenable to transcatheter closure, without surgery. However, both defects by transcather closure was rarely reported. Here we report that two children with ASD and PDA benefited from combined interventional therapy using the Amplatzer septal occluder for the ASD and Gianturco coils for the PDA. Based on our limited experience, closing the PDA before the ASD may be a better option.

Sequence variations in GATA4 and CITED2 gene among patients with cardiac septation defects from Xinjiang, China.

Studies have shown that genetic factors play an important role in CHD's development. The mutations in GATA4 and CITED2 genes result in the failure of the heart to develop normally, thereby leading to septal defects. The present study investigated the underlying molecular aetiology of patients with cardiac septation defects from Xinjiang. We investigated variants of the GATA4 and CITED2 gene coding regions in 172 patients with cardiac septation defects by sequencing. Healthy controls (n = 200) were included. Three heterozygous variations (p.V380M, p.P394T, and p.P407Q) of the GATA4 gene were id

[The most frequent congenital heart defects in the adult population and their echocardiographic diagnosis].

The congential heart disease (CHD) occur inabout 6% of all live newborn infants. About 77% of children with CHD live to adulthood. The number of adult patients with CHD increases owing to thedevelopment of cadiology, improved possibilities of non-invasive diagnostic methods with use of echocardiography and esophageal echocardiography. The clinic for adult patients with congenital heart defects at the Faculty Hospital in Motol registers 1,100 patients. The most frequent anomalites include atrial septal defects (26%), ventircular septal defects (21%), aortic coarctation (15%) and Fallot's tetral
