The sy-fi study: a randomized prospective trial of synchronized intermittent mandatory ventilation versus a high-frequency flow interrupter in infants less than 1000 g.

OBJECTIVE: Mechanical ventilation has significantly decreased mortality for preterm infants with respiratory distress syndrome. However, the barotrauma associated with mechanical ventilation is responsible for the development of bronchopulmonary dysplasia (BPD) in 20% to 90% of these infants, depending on gestational age. Recent studies suggest that high-frequency ventilation is associated with better gas exchange and less barotrauma. The purpose of this study is to determine if high-frequency ventilation, using the flow interrupter, reduces the incidence of BPD in the ELBW infant. STUDY DESI…

Increase of the oxygenation and decrease of the intrapulmonary peak pressure at constant mean airway pressure using high-frequency jet ventilation in adult rabbits with lavage-induced severe respiratory distress syndrome compared to conventional mechanical ventilation.

Reports are contradictory about the value of high-frequency jet ventilation (HFJV) in the treatment of respiratory distress syndrome (RDS) [10, 11, 18 - 1, 2, 22, 25]. In a preliminary study on rabbits with healthy as well as surfactant deficient lungs, caused by lung lavage, at a constant mean airway pressure (MAP) and 20% inspiration time, the influence of the jet ventilation frequencies of 1, 3, 5, 10, 15 and 20 Hz (cycles per second) on the pressure oscillations along the airways as well as on blood gas and cardiac parameters were investigated. It was presumed that the breathing level, e.g

Non-invasive high-frequency oscillation ventilation versus nasal CPAP as primary respiratory support in preterm neonates ≥ 30 weeks with RDS: a non-inferiority randomized controlled trial.

UNLABELLED: The purpose of this study is to evaluate if non-invasive high-frequency oscillation ventilation (NHFOV) is non-inferior to nasal continuous positive airway pressure (CPAP) as primary non-invasive respiratory support (NRS) in preterm neonates of ≥ 30 weeks' gestation with respiratory distress syndrome (RDS). In this open-label randomized controlled trial (RCT) with a non-inferiority design conducted in a lower middle-income country, 142 preterm neonates were randomized to receive NHFOV (n = 71) or CPAP (n = 71) at equivalent pressures after lung recruitment with nasal mask (NM) inte

Lung protection strategy with high-frequency oscillatory ventilation improves respiratory outcomes at two years in preterm respiratory distress syndrome: a before and after, quality improvement study.

INTRODUCTION: Bronchopulmonary dysplasia (BPD) remains one of the major challenges of extreme prematurity. High-frequency oscillatory ventilation (HFOV) with volume guarantee (HFOV-VG) can be used as an early-rescue ventilation to protect developing lungs. However, the studies exploring the impact of this ventilatory strategy on neonatal respiratory morbidity are very limited. This study aimed at documenting the improvement in respiratory outcomes in mechanically ventilated preterm newborns, after the implementation of a new mechanical ventilation respiratory bundle. METHODS: A prospective, q

Outcome of Preterm Neonates with a Birth Weight <1,500 g with Severe Hypoxemic Respiratory Failure Rescued by Inhaled Nitric Oxide Therapy and High-Frequency Oscillatory Ventilation.

BACKGROUND: Despite being an experimental therapy in preterm neonates, inhaled nitric oxide (iNO) is used as a rescue therapy when high-frequency oscillatory ventilation (HFOV) and other conventional therapies fail. OBJECTIVE: We aimed to determine the outcomes of very-low-birth-weight (VLBW) neonates with hypoxemic respiratory failure (HRF) who had received iNO after maximal conventional therapies. METHODS: We retrospectively reviewed preterm neonates (<33 weeks of gestation with a birth weight <1,500 g) who had all received HFOV and then iNO from March 1, 2009 to April 1, 2014 at the Royal

Non-invasive high-frequency ventilation versus bi-phasic continuous positive airway pressure (BP-CPAP) following CPAP failure in infants <1250 g: a pilot randomized controlled trial.

OBJECTIVE: Non-invasive high-frequency ventilation (NIHFV), a relatively new modality, is gaining popularity despite limited data. We sought to evaluate the effectiveness of NIHFV versus bi-phasic continuous positive airway pressure (BP-CPAP) in preterm infants failing CPAP. STUDY DESIGN: Infants with BW<1250 g on CPAP were randomly assigned to NIHFV or BP-CPAP if they met pre-determined criteria for CPAP failure. Infants were eligible for randomization after 72 h age and until 2000 g. Guidelines for adjustment of settings and criteria for failure of assigned mode were implemented. The primar…

The adverse effects of non-invasive high-frequency oscillatory ventilation on the cerebral hemodynamics of premature infants: point-of-care brain ultrasound findings from a case-series study.

OBJECTIVE: In recent years, non-invasive high-frequency oscillatory ventilation (NHFOV) has been widely used in clinical practice for premature infants with respiratory distress. However, until now, the focus of all published studies has been on the effect of NHFOV on patient's respiratory system, and its impact on the neonatal cerebral hemodynamics has not been considered. METHOD: We monitored the hemodynamics of the anterior cerebral arteries of 10 premature infants who received NHFOV treatment because of respiratory distress and compared them with the cerebral hemodynamics of patients who

Unexpected effect of recruitment procedure on lung volume measured by respiratory inductive plethysmography (RIP) during high frequency oscillatory ventilation (HFOV) in preterm neonates with respiratory distress syndrome (RDS).

BACKGROUND: In clinical practice, one of the major problems in optimizing recruitment or lung volume during HFOV in preterm infants is the inability to accurately measure direct changes in lung volume at bedside. OBJECTIVE: To evaluate changes in lung volume during the recruitment phase of elective HFOV in preterm infants with RDS using respiratory inductive plethysmography. MATERIAL AND METHODS: The preliminary results of an observational prospective study were reported. Newborns with GA ≤ 27 weeks requiring elective HFOV for a diagnosis of RDS were studied within the first 6 hours of life

Non-invasive high-frequency oscillatory ventilation versus nasal continuous positive airway pressure in extremely preterm infants with respiratory distress syndrome: study protocol for a multicentre randomised controlled, superiority trial.

INTRODUCTION: Tracheal intubation and invasive mechanical ventilation (IMV) significantly decreased mortality of respiratory distress syndrome (RDS) in extremely preterm infants (28 weeks' gestational age) whereas bronchopulmonary dysplasia increased. Thus, consensus guidelines recommend the use of non-invasive ventilation (NIV), as the preferred first-line approach for these infants. This trial aims to compare the effect of nasal continuous positive airway pressure (NCPAP) and non-invasive high-frequency oscillatory ventilation (NHFOV) as the primary respiratory support in extremely preterm i
