Do subsidies reach the sick? Implementation challenges and equity implications of pharmaceutical subsidy reform in Iran: a qualitative study.

BACKGROUND: Pharmaceutical subsidy reforms are commonly implemented to improve access to essential medicines, strengthen financial protection, and enhance the sustainability of health systems. In Iran, the Darooyar policy was introduced to shift pharmaceutical subsidies from subsidized foreign exchange allocation toward consumer-oriented insurance coverage. Despite its policy importance, limited evidence exists regarding the implementation challenges of this reform from the perspective of health system stakeholders. METHODS: This qualitative study was conducted using an inductive thematic ana

Therapy deserts: A call to action to address inequitable access to therapy services among pediatric orthopaedic patients.

BACKGROUND: "Food deserts" are areas with limited access to affordable and healthy foods, disproportionately affecting low-income and ethnic-minority communities in the United States. Analogous disparities exist in other disciplines. Our interprofessional team observed "therapy deserts" in orthopaedic and rehabilitation settings, wherein pediatric patients (particularly those with Medicaid-type insurance) appeared to lack equal access to physical therapy (PT), occupational therapy (OT), and speech-language pathology (SLP) services. Inadequate access to therapy services places patients at risk

Current status of continuous glucose monitoring among Korean children and adolescents with type 1 diabetes mellitus.

Type 1 diabetes mellitus (T1DM) requires life-long insulin therapy because of diminished insulin-secretion capability. Glycemic control and glucose monitoring are important to prevent T1DM complications. Continuous glucose monitoring (CGM) measures glucose level, every one to five minutes, in the interstitial fluid from a subcutaneous sensor and facilitates better glycemic control, reduces hypoglycemia, and is safely used in the pediatric population. CGM can be categorized as retrospective, real-time, or intermittently scanned CGM, and all forms are available in Korea. The CGM device has 3 com

Extracorporeal shock wave therapy in orthopedics. Assessment of an emerging health technology.

OBJECTIVE: Extracorporeal shock wave therapy (ESWT) as an emerging technology in orthopedics has been assessed in Austria with the objective to establish a scientific basis for pending and pressing health policy decisions. Despite encouraging results within some indications and the promising prospect of a noninvasive treatment for some orthopedic diseases, it seemed crucial to assess this new field of application in the light of evidence-based standards, without forgetting the reality of healthcare decisions. This article presents the results of the above mentioned assessment in the context of

Variation in coronary revascularisation and mortality after myocardial infarction across three public health insurance schemes in Thailand: an observational analysis from nationwide claims data.

BACKGROUND: Evidence on the impact of diverse healthcare insurance arrangements on healthcare variation is limited in low-income and middle-income countries. In Thailand, the Civil Servant Medical Benefit Scheme (CSMBS), Social Health Insurance (SHI) and Universal Coverage Scheme (UCS) have different provider choice and reimbursement arrangements and cover different populations. We explored to what extent use of revascularisation in patients with ST elevation myocardial infarction (STEMI) varied by insurance scheme. METHODS: We used claims data, including all admissions for patients with STEM

How Is Platelet-rich Plasma Utilized by Outpatient Providers When Covered by Insurance Without Need for Preauthorization?

BACKGROUND: TRICARE is the primary health insurer for US military personnel, military retirees, and their families. Between October 1, 2019 (FY2020) through September 30, 2024 (FY2024), TRICARE authorized private sector reimbursement of outpatient platelet-rich plasma (PRP) injections for knee osteoarthritis and lateral epicondylitis without the need for prior authorization, despite minimal evidence of efficacy for these conditions. We sought to describe utilization and reimbursement patterns for outpatient PRP injections covered under the TRICARE Provisional Coverage Program to illustrate pot

The Challenge of Unit Cost Harmonisation: A French National Reference Unit Cost for Healthcare Services (the MOVIE Project).

OBJECTIVE: The lack of standardised unit cost data is a major barrier to the quality and comparability of health economic evaluations in France. Although such evaluations are often required for reimbursement decisions, no national reference cost framework existed outside the hospital sector. The MOVIE project fills this gap by developing the first French reference unit cost framework for 22 healthcare services, using 2021 data from a representative sample of the National Health Data System. METHODS: Total billed amounts, social health insurance reimbursements, and patient out-of-pocket paymen

Barriers and facilitators of implementing public-private mix approaches for active tuberculosis case finding and health insurance access in at-risk populations in Ghana: a qualitative study.

BACKGROUND: In Ghana, although free tuberculosis (TB) services are provided at public facilities, the TB case detection rate is still lower than anticipated. To enhance TB case detection, private community healthcare providers and the National Health Insurance Scheme (NHIS) have been involved using a Public-Private Mix (PPM) model, which includes active case finding (ACF) and access to insurance. This study examines the facilitators and barriers to implementing a PPM model that aims to expand ACF and provide health insurance to newly diagnosed TB patients among at-risk populations in two Ghana

MiCare study, an evaluation of structured, multidisciplinary and personalised post-ICU care on physical and psychological functioning, and quality of life of former ICU patients: a study protocol of a stepped-wedge cluster randomised controlled trial.

INTRODUCTION: Over 70% of the intensive care unit (ICU) survivors suffer from long-lasting physical, mental and cognitive problems after hospital discharge. Post-ICU care is recommended by international guidelines, but evidence for cost-effectiveness lacks. The aim of this study is to evaluate the clinical effectiveness and cost-effectiveness of structured, multidisciplinary and personalised post-ICU care versus usual care on physical and psychological functioning and health-related quality of life (HRQoL) of ICU survivors, 1- and 2-year post-ICU discharge. METHODS AND ANALYSIS: The MONITOR-I

[EDNOS is an eating disorder of clinical relevance, on a par with anorexia and bulimia nervosa].

BACKGROUND: The category 'eating disorder 'not otherwise specified'' (EDNOS) in DSM-IV is restricted to eating disorders of clinical severity that do not completely fulfil the criteria for anorexia and bulimia nervosa. The EDNOS category is, by definition, often regarded as a a residual category and in principle designed to incorporate a small group of patients with atypical characteristics. Health insurance companies argue that the treatment of patients diagnosed with EDNOS should not be treated in mental health institutions and therefore should not get their treatment costs reimbursed by the

Clinical and Economic Burden of Multiple Double-Stranded DNA Viral Infections after Allogeneic Hematopoietic Cell Transplantation.

Conditioning regimens for allogeneic hematopoietic cell transplantation (allo-HCT) are immunosuppressive and increase the risk for reactivation of and infection with double-stranded DNA (dsDNA) viruses, which contribute to morbidity and mortality after allo-HCT. This retrospective observational study evaluated the association of dsDNA viral infections with clinical outcomes, health resource utilization (HRU), and health care reimbursement after allo-HCT. Patients who underwent allo-HCT between 2012 and 2017 were identified from a US open-source claims database (Decision Resource Group Real-Wor
