New strategies for the management of ocular surface disease in glaucoma patients.

PURPOSE OF REVIEW: Glaucoma patients commonly suffer from ocular surface disease (OSD). As treatment strategies, medications and devices for the treatment of OSD as well as glaucoma surgical approaches evolve rapidly, it is important to consider their application to these patients. RECENT FINDINGS: OSD in glaucoma patients may lead to reduced reliability of diagnostic tests, decreased medication compliance, poor surgical outcomes, and overall decreased quality of life. Chronic use of topical glaucoma medications has been linked to the development of limbal stem cell deficiency, and the role o

Xenogeneic acellular conjunctiva matrix as a scaffold of tissue-engineered corneal epithelium.

Amniotic membrane-based tissue-engineered corneal epithelium has been widely used in the reconstruction of the ocular surface. However, it often degrades too early to ensure the success of the transplanted corneal epithelium when treating patients with severe ocular surface disorders. In the present study, we investigated the preparation of xenogeneic acellular conjunctiva matrix (aCM) and evaluated its efficacy and safety as a scaffold of tissue-engineered corneal epithelium. Native porcine conjunctiva was decellularized with 0.1% sodium dodecyl sulfate (SDS) for 12 h at 37°C and sterilized v

Autologous serum-derived cultivated oral epithelial transplants for severe ocular surface disease.

OBJECTIVE: To evaluate the use of autologous serum (AS)-derived cultivated oral epithelial transplants for the treatment of severe ocular surface disease. METHODS: We used AS from 10 patients with severe ocular surface disease and total limbal stem cell deficiency to develop autologous cultivated oral epithelial equivalents. These were compared with epithelial equivalents derived from conventional fetal bovine serum-supplemented medium. Surgery involved removal of the corneal pannus and surrounding diseased tissue and transplantation of the AS-derived epithelial equivalents. The oral equivale

Vernal Keratoconjunctivitis.

Vernal keratoconjunctivitis, a chronic bilateral seasonal allergic inflammatory disease of the eye, is an important cause of visual debilitation and impairment of quality of life in children and young adults in certain parts of the world such as the Mediterranean areas, Central and West Africa, the Middle East, Japan, the Indian subcontinent, and South America. It usually has a self-limiting course; however, in a few cases, the disease is recurrent and leads to long-term visual disabling complications such as keratoconus and limbal stem cell deficiency. The main pathogenic mechanism is immunog

Optical Coherence Tomography in Cultivated Limbal Epithelial Stem Cell Transplantation Surgery.

PURPOSE: Cultivated limbal epithelial stem cell transplantation (CLET) is a surgical approach reserved for severe stem cell deficiency and anterior segment pathology. The recipient cornea typically displays a highly vascularized pannus that must be dissected before graft placement. Extensive pannus dissection, however, can result in corneal thinning and increase the risk of ectasia and ultimately, of perforation. We aimed to examine the value of anterior segment optical coherence tomography (AS-OCT) intraoperatively during pannus resection and long-term postoperative follow-up of CLET surgery.

Current and emerging strategies for the manufacture, implantation, and clinical management of corneal tissue allografts.

Approximately 40,000 Australians have received a donor corneal tissue transplant over the last 40 years, with the primary indications being keratoconus, Fuchs' endothelial dystrophy, bullous keratopathy, and failure of a prior corneal transplant. Although corneal cross-linking and rigid contact lenses have emerged as alternative strategies for the management of keratoconus, the demand for donor corneas is increasing in-line with the ageing population in Australia. Moreover, owing to the lack of tissue banking resources in less-developed countries, the global demand for donor corneas exceeds su

Transcorneal Cryotherapy and Descemet Membrane Endothelial Keratoplasty to Treat Epithelial Downgrowth and Corneal Decompensation After Cataract Surgery.

PURPOSE: Epithelial downgrowth is a vision-threatening complication of intraocular surgery or penetrating ocular trauma, and although various therapeutic interventions have been performed to treat this disease, success has been limited. We present a case of corneal decompensation secondary to epithelial downgrowth after uncomplicated clear corneal cataract surgery in the left eye, treated successfully with sequential transcorneal cryotherapy to destroy epithelial cell nests followed by Descemet membrane endothelial keratoplasty (DMEK) to restore corneal clarity and vision. METHODS: We applied

The Role of Hypoxia in Corneal Extracellular Matrix Deposition and Cell Motility.

The cornea is an excellent model tissue to study how cells adapt to periods of hypoxia as it is naturally exposed to diurnal fluxes in oxygen. It is avascular, transparent, and highly innervated. In certain pathologies, such as diabetes, limbal stem cell deficiency, or trauma, the cornea may be exposed to hypoxia for variable lengths of time. Due to its avascularity, the cornea requires atmospheric oxygen, and a reduction in oxygen availability can impair its physiology and function. We hypothesize that hypoxia alters membrane stiffness and the deposition of matrix proteins, leading to changes

Preferred practice guidelines and narrative review on infectious keratitis in ocular surface diseases.

Ocular surface disease (OSD) encompasses a variety of additional factors in the natural history of infectious keratitis like disruption of the normal tear film, altered ocular microbiome, adnexal inflammation, de-epithelization of the cornea due to anatomical factors like trichiasis, lid margin keratinization, presence of limbal stem cell deficiency, and other lid related problems. These cases need special attention with respect to lower threshold for inpatient admission and care along with examination and careful corneal scraping to avoid any perforation. The preferable practice patterns in t
