Extensor mechanism disruptions are relatively uncommon knee injuries. Within this group, delayed treatment is an infrequent but difficult clinical situation. Fibrous degeneration, muscle contraction and subsequent hiatus present a challenge to the orthopaedic surgeon. We present an unusual case of a successful delayed repair using the Mitek anchors system (Mitek Products Inc., a division of Ethicon Inc., Westwood, Massachusetts).

From January 1995 to July 1996, the authors used Mitek GIV titanium anchors to treat seven patients with acute rupture of the Achilles tendon (four men, three women; average age 52.42 years; range, 33-62 years). All subjects had a total rupture of the most distal aspect of the tendon; none had an avulsion fracture. After dissection down to the paratenon, the reinsertion site was selected and a 2 to 3-cm-long trough carved through the cortex. Three holes were drilled at a distance of 0.5 cm one from the other and 1 cm parallel and distal to the trough. The anchors were then threaded, inserted i

Twenty-six consecutive cadet athletes sustained an acute, initial anterior shoulder dislocation. All dislocations required a manual reduction for initial treatment. Arthroscopy was performed within 10 days in all patients. The Beach chair position and interscalene anesthesia were used in each case. At arthroscopy, 25 patients had an avulsion of the anterior-inferior capsulolabral complex (Bankart lesion) from the glenoid rim. One patient had a lateral detachment of the inferior glenohumeral ligament from the humeral neck. Twenty-three patients had a Hill-Sachs lesion and 3 SLAP tears were note

101 children in Tromsö, Norway, treated with the Frejka pillow for 4.5 months because of neonatal hip instability (NHI) were compared with 307 children in Malmö, Sweden, treated with the von Rosen splint for 3 months. The pelvic radiographs, taken when the treatment was terminated, were assessed by the acetabular index (AI) and the cases of failure were evaluated. The AI showed no difference between the two groups. The Frejka group had 4 patients who received further treatment because of remaining acetabular dysplasia and/or subluxation while the von Rosen group had none. The difference in ris

The system consists of hooked wires, anchoring screws, and bolts, which provide compression to the osteochondral fragments and prevent pin migration. In 10 patients followed for 4 (3-5) years, all the knees had normal, painless function, with complete healing of all but one of the fragments; the exception was a small chondral fragment, which was removed arthroscopically.

In adolescents dynamic-compression-plate osteosynthesis is an excellent modality for treating femoral shaft fractures. It provides anatomical stabilisation as well as short hospitalisation and rehabilitation. In contrast, fractures of the tibial shaft are usually treated with casts or by extension. Primary operation is rarely necessary, for example in cases of an impaired neurovascular system. In third degree open fractures we usually recommend external fixation. In our department clinical and radiological late-term controls are performed over a period of at least two years for tibial and four

The long-term results of spinal arthrodesis were evaluated in fourteen children and adolescents who had severe spondylolisthesis. Twelve patients had an in situ posterior arthrodesis and the other two had, in addition, open reduction. The two patients who had open reduction lost correction when the rods were removed. At long-term follow-up, which averaged 11.9 years, all patients had a solid fusion and their activities were unrestricted. Two patients were dissatisfied with the cosmetic result. No intraoperative or postoperative complications occurred in association with the in situ arthrodeses

Certain patients with cervical spondylotic myelopathy demonstrate instability in the upper cervical vertebral column associated with marked narrowing of the vertebral canal in the neighboring lower cervical segments. This combination of causative mechanisms creates difficulty in using routine surgical procedures, especially in elderly and severely debilitated patients. We present three cases that typify this situation and illustrate marked resolution of debilitating myelopathy through total posterior fusion of the cervical spine usually incorporating the occiput.

Obtaining an arthrodesis of the lumbar spine has always been a problem, especially in the presence of severe disease. The understanding of the segmental character of the lesions and the application of sound orthopedic principles toward low-back spine pathology has led the way to improving arthrodesis with rigid internal fixation. For about eight years, segmental spinal instrumentation has been used in 352 cases to "stabilize" the spine. Many modifications have taken place and knowledge of the biomechanics of abnormal spine has increased through designs of new methods for different disorders. F

As an introduction statements from literature and own experience are used to show and evaluate the present possibilities for the coverage of skin and soft tissue defects. As a routine operation the stemmed cross-leg flap is further on indicated on the lower leg. For the immobilisation of the legs wer use the fixateur externe. This kind of fixation has got advantages compared to plaster casts: optimal position of the two lower legs to each other, a local and general handling of the nursing with only few problems and a subjective relief for the patient. Moreover, flaps and transplant base can be

This report confirms the concept previously established in the literature tha arthrodesis of the wrist is a technically demanding procedure witha significant incidence of postoperative complications. A series of 31 patients is presented in whom wrist arthrodesis was performed using three separate types of operative techniques, with emphasis on postoperative morbidity and complications. Major complications included: pseudarthrosis (six patients), deep wound infection (one patient), neuroma (one patient), fracture of healed fusion (two patients), and impingement of Steinmann pin on metacarpophal

BACKGROUND: Postoperative pain can lead to complications, prolonged hospital stays, and increased healthcare costs. Orthopedic surgeries are particularly associated with severe pain. This study aimed to identify orthopedic procedures that cause severe pain in hospitalized patients within 72 h post-surgery.
METHODS: Pain scores were retrospectively analyzed for patients undergoing inpatient orthopedic surgery between October 15, 2015, and December 31, 2019, at three hospitals. Patient and procedure details and maximum pain scores were collected up to 72 h postoperatively. Procedures were class
