Altered White Matter and Sensory Response to Bodily Sensation in Female-to-Male Transgender Individuals.

While most people take identification with their body for granted, conditions such as phantom limb pain, alien hand syndrome, and xenomelia suggest that the feeling of bodily congruence is constructed and susceptible to alteration. Individuals with xenomelia typically experience one of their limbs as over-present and aversive, leading to a desire to amputate the limb. Similarly, many transgender individuals describe their untreated sexed body parts as incongruent and aversive, and many experience phantom body parts of the sex they identify with (Ramachandran, 2008). This experience may relate …

[Phantom pain after eye enucleation].

BACKGROUND: Phantom pain is a well known and extensively documented complication after limb amputation. Nearly all surgical disciplines have to deal with phantom symptoms related to different anatomical regions and organs but limited data is available about phantom eye pain (PEP) after enucleation. Only one pilot study is present in literature. It was the aim of this study to analyze a group of patients with a standardized surgical procedure of enucleation concerning the incidence of PEP and its relation to further preoperative and postoperative data. PATIENTS AND METHODS: A standardized ques

Dysregulated proprioception in trigeminal pain: A novel perspective on oral dysesthesia, from chaos to balance.

INTRODUCTION: Oral dysesthesia (OD) is a chronic orofacial pain disorder characterized by abnormal sensations - burning, tingling, altered t … ergence. RESULTS: Proprioceptive dysregulation may contribute to OD through processes analogous to phantom limb phenomena. Current management - pharmacologic, behavioral, and neuromodulatory - remains limited by mechanistic non-specificity. Preliminary concepts of a custom-fabricated oral device suggest it may modulate proprioception, provide sensory gating, …

Biomimetic Intraneural Sensory Feedback Enhances Sensation Naturalness, Tactile Sensitivity, and Manual Dexterity in a Bidirectional Prosthesis.

Peripheral intraneural stimulation can provide tactile information to amputees. However, efforts are still necessary to identify encoding st … taining high levels of manual accuracy. They also improved prosthesis embodiment, reducing abnormal phantom limb perceptions ("telescoping effect"). Hybrid strategies are able to provide highly sensitive and natural percepts and should be preferred. VIDEO ABSTRACT.

Functional outcome following traumatic upper limb amputation and prosthetic limb fitting.

Nineteen consecutive patients underwent traumatic upper limb amputation for nonreconstructible or replantible upper limb injury at a Level I trauma center over a 9-year-period. Eleven amputations were at the transradial level, five were transhumeral, and three were shoulder disarticulation. Eighteen patients underwent prosthetic limb fitting. Fifteen of the 18 initially underwent preparatory prosthetic limb fitting within 30 days following amputation with a body-powered, cable-driven prosthesis. Seventeen of the 18 achieved sufficient proficiency with their prostheses to allow them to return t

Alterations in Brain Structural Connectivity After Unilateral Upper-Limb Amputation.

Previous studies have indicated that amputation induces reorganization of functional brain network. However, the influence of amputation on … strength of the contralateral middle temporal gyrus was positively correlated with the magnitude of phantom limb sensation. Our results suggested a degeneration of FA network after dominant-side upper-limb amputation.

"Decreasing Postamputation Pain with the Regenerative Peripheral Nerve Interface (RPNI)".

Over 185,000 limb amputations are performed in the United States annually, many of which are due to the sequelae of peripheral vascular disease. Symptomatic neuromas remain a significant source of postamputation morbidity and contribute to both phantom limb (PLP) and residual limb pain (RLP). While many interventions have been proposed for the treatment of symptomatic neuromas, conventional methods lead to a high incidence of neuroma recurrence. Furthermore, these existing methods do not facilitate an ability to properly interface with myoelectric prosthetic devices. The Regenerative Periphera…

Feasibility of Magnetic Resonance-Guided High-Intensity-Focused Ultrasound (MRgHIFU) Ablation of Stump Neuromas for the Relief of Chronic Postamputation Neuropathic Pain.

Up to 70% of limb amputees develop chronic postamputation neuropathic pain (CPANP) which includes phantom pain and residual limb neuropathic pain due to neuroma formation. CPANP often requires invasive procedures aimed at neuroma ablation. Five amputees received 6 noninvasive magnetic resonance-guided high-intensity-focused ultrasound MRgHIFU treatments ExAblate®, Insightec, Tirat-Carmel, Israel). Although ablative temperature (>65°C) at the neuroma was reached in only 1 patient, pain intensity dropped from 5.7 at baseline to 4.3 and back to 5.6 at 3 and 6 month follow-up. Post-treatment bone

Impact of Lower Limb Amputation on Implant Heating During 1.5 T MRI: Assessment of Active and Passive Device.

PURPOSE: This study investigates the impact of lower limb amputation on radiofrequency (RF)-induced heating of active and passive implantable medical devices (AIMDs and PIMDs) in a 1.5 T MRI environment. METHODS: High-resolution anatomical models representing various body types and levels of amputation (below-knee and above-knee) were explored in the study. Full-wave electromagnetic simulations were used to assess specific absorption rate (SAR) near modular hip prostheses. The RF-induced lead-tip heating for a clinically relevant peripheral nerve stimulation (PNS) device was evaluated followi

Restoration of natural somatic sensations to the amputees: finding the right combination of neurostimulation methods.

UNLABELLED: Limb amputation results in such devastating consequences as loss of motor and sensory functions and phantom limb pain (PLP). Neurostimulation-based approaches have been developed to treat this condition, which provide artificial somatosensory feedback such as peripheral nerve stimulation (PNS), spinal cord stimulation (SCS), and transcutaneous electrical nerve stimulation (TENS). Yet, the effectiveness of different neurostimulation methods has been rarely tested in the same participants. Meanwhile, such tests would help to select the most effective method or a combination of method…

Targeted Muscle Reinnervation to Improve Pain, Prosthetic Tolerance, and Bioprosthetic Outcomes in the Amputee.

Scope and Significance: There are ∼185,000 amputations each year and nearly 2 million amputees currently living in the United States. Approximately 25% of these amputees will experience chronic pain issues secondary to localized neuroma pain and/or phantom limb pain. Problem: The significant discomfort caused by neuroma and phantom limb pain interferes with prosthesis wear, subjecting amputees to the additional physical and psychological morbidity associated with chronic immobility. Although numerous neuroma treatments are described, none of these methods are consistently effective in eliminat…
