Laparoscopic pneumoperitoneum in acute peritonitis does not increase bacteremia or aggravate metabolic or hemodynamic disturbances.

The use of laparoscopy in generalized peritonitis has become increasingly frequent in recent years. However, CO2 pneumoperitoneum in association with increased intraperitoneal pressure may have deleterious effects in patients with hemodynamic or metabolic disturbances caused by bacterial peritonitis. The purpose of this study was to investigate the effect of CO2 pneumoperitoneum on bacteremia, mean arterial pressure, and blood gas disturbances in an animal model of bacterial peritonitis. Dogs were anesthetized, orally intubated, and subjected to experimental peritonitis by intraperitoneal inoc…

Generalized peritonitis with pneumoperitoneum caused by the spontaneous perforation of pyometra without malignancy: report of a case.

Spontaneous perforation is a very rare complication of pyometra. We report herein the case of an 88-year-old woman who presented with muscular rigidity and free air on abdominal X-ray films. Perforation of the gastrointestinal tract was diagnosed preoperatively, and an emergency laparotomy was performed. A total hysterectomy with bilateral salpingo-oophorectomy was carried out under the diagnosis of generalized peritonitis caused by the spontaneous perforation of pyometra. The culture of purulent fluid from the abdominal cavity showed only Escherichia coli, with no anaerobic bacteria. Histolog

[A difficult therapeutic dilemma: pneumoperitoneum in severe blunt injuries treated by respiratory or cardiorespiratory resuscitation].

In the particular context of severe blunt trauma treated with cardiorespiratory resuscitation or, more simply, with positive-pressure ventilation, the occurrence of pneumoperitoneum always raises difficult problems of etiological diagnosis, which in turn lead to a difficult dilemma on the treatment to adopt. In fact, while the air released by the alveolar baro-trauma can, once a given pressure is exceeded, migrate into the retroperitoneum and produce a non-surgical pneumascos (for which the other possible causes are far more exceptional, and rather are unpleasant surprises, such as this orogen…

Diagnostic performance of artificial intelligence in radiographs for pneumoperitoneum detection: a systematic review and meta-analysis.

OBJECTIVES: The aim of this systematic review and meta-analysis is to evaluate the diagnostic accuracy of artificial intelligence (AI) based algorithms in detecting pneumoperitoneum on medical imaging. METHODS: Online databases were searched until June 2024. Statistical analyses were conducted using Open Meta-Analyst software and STATA 17.0. The analysis included overall sensitivity, specificity, diagnostic odds ratio (DOR), and area under the curve (AUC). Meta-regression and subgroup analyses were conducted to identify sources of heterogeneity among the included studies. RESULTS: Among the …

Preoperative progressive pneumoperitoneum: insights on implementation in an ambulatory care setting. How I do it?

INTRODUCTION: Incisional hernias with loss of domain (IHLD) are challenging to treat. Preoperative techniques like botulinum toxin injection (BTA) and preoperative progressive pneumoperitoneum (PPP) are highly effective, potentially preventing the need for perioperative component separation in the vast majority of cases. PPP involves preoperative introduction of gas into the abdominal cavity to increase the abdominal wall volume, aiding diaphragmatic prehabilitation and hernia reintegration. This study aimed at explaining our technique in performing PPP in ambulatory setting. DESCRIPTION OF T…

Intracranial pressure estimated non-invasively and postoperative outcomes in surgery in the Trendelenburg position with pneumoperitoneum.

BACKGROUND: Surgery in the Trendelenburg position (TP) with pneumoperitoneum (PP) is beneficial in several aspects but is associated with postoperative complications, such as postoperative nausea and vomiting (PONV). The mechanism behind this is unknown, but an increase in intracranial pressure (ICP) has been suggested. There are several studies of non-invasively estimated ICP during surgery in TP with PP. The association between perioperative estimated ICP and postoperative complications has not yet been reviewed. METHODS: We performed a scoping review of peer-reviewed clinical studies repor…

The Effect of Pneumoperitoneum on the Ankle-brachial Index of the Patients Undergoing Laparoscopic Cholecystectomy: An Observational Study.

BACKGROUND: Improvement in the perioperative care has led to increased use of minimally invasive surgeries. Multiple physiological changes during minimally invasive surgeries are attributed to the creation of pneumoperitoneum. MATERIALS AND METHODS: One hundred and nine patients who underwent laparoscopic cholecystectomy at a tertiary care hospital in north India meeting the inclusion and exclusion criteria were enrolled. RESULTS: Out of the total 109 patients, 13 were males and 96 females (M:F = 1:7.3), the mean basal metabolic rate was 28.95 kg/m2. The mean systolic and diastolic blood pre…

Yield of abdominal radiographs in children with suspected intussusception; rate of pneumoperitoneum and other abdominal pathology.

OBJECTIVES: Ultrasound is the criterion standard imaging modality for the diagnosis of intussusception. However, to our knowledge the utility of abdominal radiographs to concurrently screen for pneumoperitoneum or other abdominal pathology that could have a similar presentation has not been studied. Our institutional protocol requires the performance of AP supine and left lateral decubitus views of the abdomen prior to ultrasound evaluation for intussusception, providing an opportunity to examine the yield of abdominal radiographs in this setting. Our primary objective was to determine the rat…

Ruptured splenic abscess as an extremely rare cause of pneumoperitoneum: A comprehensive review with a case report.

Splenic abscess leading to rupture and pneumoperitoneum is a very rare condition that is limited to a few case reports. In this study, we first introduced a case of an adolescent girl with a medical history of diabetes mellitus who presented with left upper quadrant abdominal pain and fever. Ultrasound revealed an abscess in the spleen and a computed tomography (CT) scan of the abdomen revealed evidence of pneumoperitoneum secondary to the rupture of this abscess. The patient underwent splenectomy without postoperative complications. Secondly, we reviewed and discussed the current literature o…

Pulse Pressure Variation Can Predict the Hemodynamic Response to Pneumoperitoneum in Dogs: A Retrospective Study.

Pneumoperitoneum may induce important hemodynamic alterations in healthy subjects. Pulse pressure variation (PPV) is a hemodynamic parameter able to discriminate preload dependent subjects. Anesthesia records of dogs undergoing laparoscopy were retrospectively evaluated. The anesthetic protocol included acepromazine, methadone, propofol and isoflurane administered with oxygen under mechanical ventilation. The hemodynamic parameters were considered five minutes before (BASE) and ten minutes after (P10) the pneumoperitoneum. Based on the cardiac index (CI) variation, at P10, dogs were classified…

The protocol of low-impact laparoscopic cholecystectomy: the combination of mini-laparoscopy and low-pressure pneumoperitoneum.

Low-impact laparoscopic (LIL) cholecystectomy is an innovative surgical protocol that combines the use of mini-laparoscopic instruments (3-mm ports) under a low- and stable-pressure pneumoperitoneum (8 mmHg), with the aim of minimizing the surgical invasiveness and the risks related to CO2 insufflation on the peritoneal environment. In day-surgery settings, LIL may contribute to increase the surgical success due to several potential benefits in terms of postoperative pain intensity and time to full recovery. In 14 consecutive patients requiring cholecystectomy for uncomplicated cholelithiasis,…

Spontaneous perforation of pyometra presenting as acute abdomen and pneumoperitoneum mimicking those of gastrointestinal origin.

Gastrointestinal (GI) perforation accounts for over 90% of acute abdomen and pneumoperitoneum. The presence of pneumoperitoneum secondary to spontaneously perforated pyometra is an interesting yet confusing finding given the absence of gastrointestinal (GI) perforation, because pyometra is more common in postmenopausal women. We report an instructive case of diffuse peritonitis caused by spontaneous perforation of pyometra. A 70-year-old postmenopausal female was admitted to surgical emergency with signs of diffuse peritonitis. After resuscitation, an emergency laparotomy was performed because…

Spontaneous Pneumomediastinum, Subcutaneous Emphysema, and Pneumoperitoneum in RT-PCR-Confirmed Measles: A Pediatric Case Report.

Measles remains a major global public health challenge as declining vaccination coverage fuels outbreaks worldwide. Although pneumonia is the most recognized respiratory complication, spontaneous air leak syndrome-including pneumomediastinum, subcutaneous emphysema, and pneumoperitoneum-is rarely documented. We report the case of a 9-year-old previously healthy girl with no documented measles-rubella vaccination who presented with fever, maculopapular exanthem, Koplik spots, and persistent cough. Measles was confirmed by both immunoglobulin M enzyme-linked immunosorbent assay and real-time rev…

Ultrasound-guided preoperative progressive pneumoperitoneum to facilitate pressurized intraperitoneal aerosol chemotherapy in inoperable peritoneal carcinomatosis.

INTRODUCTION: Approximately 5% of patients with peritoneal carcinomatosis are excluded from pressurized intraperitoneal aerosol chemotherapy (PIPAC) due to a "non-accessible abdomen" caused by extensive adhesions. Preoperative progressive pneumoperitoneum (PPP) is traditionally used in complex hernia repair but has not been applied to facilitate PIPAC. This study evaluates the feasibility and safety of ultrasound-guided PPP to overcome this technical barrier. MATERIALS AND METHODS: A prospective study was conducted in five patients with histologically confirmed inoperable peritoneal carcinoma…

Transumbilical Laparoscopic-Assisted Appendectomy Without Pneumoperitoneum in Children: Feasibility and Outcomes From a Single Center.

BACKGROUND: Transumbilical laparoscopic-assisted appendectomy (TULAA) is a minimally invasive technique for pediatric acute appendicitis. While most TULAA procedures utilize pneumoperitoneum, we adopted a modified technique involving mechanical elevation of the abdominal wall without insufflation. This study aimed to evaluate the safety and efficacy of TULAA without pneumoperitoneum by analyzing clinical outcomes in pediatric patients who underwent this procedure at our institution. METHODS: We retrospectively analyzed 252 pediatric patients who underwent TULAA without pneumoperitoneum betwee…
