Overdose Risk and Acquiring Opioids for Nonmedical Use Exclusively from Physicians in Vancouver, Canada.

A primary response to the alarming rise in overdose and mortality due to nonmedical prescription opioid (PO) use has been to restrict opioid prescribing; however, little is known about the relationship between obtaining opioids from a physician and overdose risk among people who use POs nonmedically and illicit street drugs. Objectives: Investigate the relationship between non-fatal overdose and acquiring POs exclusively from physicians for the purposes of engaging in nonmedical PO use. Methods: Data were collected between 2013 and 2016 among participants in two harmonized prospective cohort s

A Comparison of Maternal Interconception Care Practices in Pediatric and Family Medicine Offices.

Maternal interconception care is essential to improve birth outcomes by targeting maternal risk factors. The Healthy Mom, Healthy Family (HMHF) program expanded the Interventions to Minimize Preterm and Low Birth Weight Infants through Continuous Improvement Techniques (IMPLICIT) Network's scope by incorporating interconception care into pediatric and family medicine well-child visits (WCVs). We compared screening and referral/counseling rates for maternal behaviors between pediatricians and family physicians. Physicians used a standardized data tool to collect information on maternal behavior

International survey on the management of colorectal trauma and alignment with current guidelines.

Colorectal trauma management remains variable worldwide. This study evaluated contemporary practice patterns using an anatomically stratified international survey. An anonymous, electronic survey assessing contemporary colorectal trauma management was distributed internationally through social media platforms and professional surgical societies to capture current practice patterns and variations in clinical decision-making. A total of 280 surgeons from 59 countries responded. For hemodynamically stable intraperitoneal colorectal injuries, 196/280 (70.00%) selected resection, when indicated, wi

Real-world Utilization of Mineralocorticoid Receptor Antagonists in India and the Benefits of GDMT in Heart Failure.

Early initiation of guideline-directed medical therapies (GDMTs) in heart failure (HF) and their uptitration to the target dose confer mortality benefits and reduce the risk of readmission. GDMT nonuse is a significant predictor of mortality in HF patients. However, GDMT prescription and adherence in India are low. Of the GDMTs, mineralocorticoid receptor antagonists (MRAs) are the least prescribed. There are multilevel gaps [healthcare professional (HCP)-related, patient-related] in the adoption and use of MRAs in HF. There is an unmet need to identify these gaps and formulate mitigation stra

Headache and migraine clinical practice guidelines: a systematic review and assessment of complementary and alternative medicine recommendations.

BACKGROUND: Globally, 3 billion people suffer from either migraine or tension-type headache disorder over their lifetime. Approximately 50% of American adults suffering from headache or migraine have used complementary and alternative medicine (CAM), however, the quality and quantity of recommendations associated with such therapies across clinical practice guidelines (CPGs) for the treatment and/or management of these conditions are unknown. The purpose of this study was to identify the quantity and assess the quality of such CAM recommendations. METHODS: MEDLINE, EMBASE and CINAHL were syst

Insights in Public Health: Ambulatory Blood Pressure Monitoring: Underuse in Clinical Practice in Hawai'i.

Hypertension is one of the leading causes of death and disability worldwide. Blood pressure reduction and control are associated with reduced risk of stroke and cardiovascular disease. To achieve optimal reduction and control, reliable and valid methods for blood pressure measurement are needed. Office based measurements can result in 'white coat' hypertension, which is when a patient's blood pressure in a clinical setting is higher than in other settings, or 'masked' hypertension, which occurs when a patient's blood pressure is normal in a clinical setting, but elevated outside the clinical s

Giardiasis in North West England: faecal specimen requesting rates by GP practice.

Many cases of giardiasis in the UK are undiagnosed and among other things, diagnosis is dependent upon the readiness of GPs to request a specimen. The aim of this study is to assess the rate of specimens requested per GP practice in Central Lancashire, to examine the differences between GP practices and to estimate the pattern of unexplained spatial variation in the practice rate of specimens after adjustment for deprivation. To achieve this, we fitted a set of binomial and Poisson regression models, with random effects for GP practice. Our analysis suggests that there were differences in the

[Diagnosis and treatment of bone metastasis].

The improved prognosis of cancer patients has led to an increased incidence of both bone metastases and (impending) pathological fractures. A solitary bone lesion seen on radiography should never be assumed to be a metastasis. Preoperative biopsy is necessary in patients with a known malignancy and a solitary lytic bone lesion as well as in patients in whom the primary tumor is unknown, in order to prevent an incorrect operation (also known as 'whoops surgery'). If the patient has an (impending) pathological fracture, normal bone healing is not to be expected, not even after stable fixation. S

[After-cataract following intraocular lens implantation. Part I. Genesis and prevention by optimizing conventional lens implants and surgical techniques].

After-cataract is the most common complication in cataract surgery. Implementing a sharp posterior optic edge has, together with improved cortical aspiration, drastically reduced its rate. The impact of optic design and material has been systematically studied in detail in Vienna in a series of prospectively randomized intra-individual comparison studies using objective evaluation methods. Circular rhexis-optic overlap is essential for the durability of the posterior sharp optic edge barrier effect. Using haptics with a capsular bag design and a slim junction to the optic enhanced it further.
