Small bowel obstruction secondary to barbed sutures in bariatric surgery: a cautionary tale.

BACKGROUND: Laparoscopic bariatric surgery relies on technically challenging intracorporeal suturing for critical parts of the operation. Barbed sutures have been developed to provide an alternative to suturing for certain manoeuvres within a procedure. Barbed sutures theoretically negate the need for knot tying and allow for continuous application of tension; however the barbs can unintentionally adhere to surrounding tissues. We describe a case series of three patients who developed V-Loc™ (barbed) suture related small bowel obstruction (SBO) to promote awareness of this unusual but preventa…

Use of Knotless Barbed Sutures in Laparoscopic Inguinal Hernioplasty in Horses: 40 Cases.

Inguinal hernias (IHs) and ruptures are a relatively common condition in horses, occurring in foals (congenital) and adult (acquired) animals. A retrospective observational analysis was conducted on 40 cases that underwent laparoscopic surgery to close the VRs using barbed sutures alone or combined with other techniques. Signalment, clinical presentation, surgery, and follow-up data were obtained. In total, fifty-nine VRs were closed using barbed sutures (alone or in combination with other methods), with six cases performed prophylactically and forty-four due to acquired IH. Of the forty-four …

Successful myomectomy using barbed sutures at 15 weeks of gestation: A case report.

To date, there are no reports on the use of barbed sutures in myomectomy during pregnancy. Herein, successful laparotomic myomectomy at 15 weeks of gestation using barbed sutures is described. A 38-year-old pregnant woman with a large myoma (16 × 11 cm) underwent myomectomy at 15 weeks and 4 days of gestation because of strong pain at the myoma site, a high level of inflammatory markers with degeneration of uterine fibroids, and severe anemia caused by denaturation of myoma. The uterine wall was reconstructed using monofilament absorbable 0 barbed sutures. Unlike conventional sutures, barbed s…

Novel invagination procedure for pancreaticojejunostomy using double purse string sutures: A technical note.

BACKGROUND: Postoperative pancreatic fistula (POPF) is one of the most serious complications after pancreaticoduodenectomy (PD), and the choice of pancreaticojejunostomy (PJ) is considered a key factor affecting the occurrence of POPF. Numerous anastomotic methods and their modifications have been proposed, and there is no method that can completely avoid the occurrence of POPF. Based on our team's experience in pancreatic surgery and a review of relevant literature, we describe a novel invagination procedure for PJ using double purse string sutures, which has resulted in favourable outcomes. …

The potential of cell-assembled extracellular matrix for biological sutures: A promising innovation.

Most surgical procedures require using suture materials that are mechanically efficient and accepted by the patient's body. These sutures are essentially composed of synthetic polymers. However, once implanted in patients, they are recognized as foreign bodies and generate chronic inflammation. Thereafter, the patient's immune system will degrade, encapsulate, or even expel the materials. Our innovation, the Cell-Assembled extracellular Matrix (CAM), synthesized from mesenchymal cells, replicates native tissue environments and promotes integration, reducing complications. In a recent study, we…

Biomechanical Study of Modified Massachusetts General Hospital Flexor Tendon Repair Using Looped Sutures.

Background: Massachusetts General Hospital (MGH) repair is one of the widely used 4-strand flexor tendon repair techniques. However, it uses two single strand sutures that are each passed twice across the repair site. This is time consuming and may cause imbalance of the load across the repair. We modified the MGH repair by using a looped suture and call it the looped MGH repair. The aim of this study is to compare the strength of the looped MGH repair performed with three different looped sutures against the strength of original MGH repair. Methods: Forty porcine flexors were used for the stu…

Fundoplication without esophagocrural sutures: Long-term follow-up of a randomized clinical trial.

INTRODUCTION: We previously conducted a randomized trial that showed a lack of need for esophagocrural (EC) sutures during fundoplication when no esophageal dissection was performed. There was no difference in wrap herniation or other complications in the group without EC sutures at a median 1.5 years of follow-up. In this follow-up study, we aim to evaluate long-term symptom control and complication profiles in these patients. METHODS: 106 patients were randomized and participated in the original trial. We were primarily concerned with identification of late complications and persistence of …

Comparison of absorbable and permanent sutures for laparoscopic sacrocervicopexy: A randomized controlled trial.

INTRODUCTION: Pelvic organ prolapse is a common cause of morbidity and decreased quality of life among women and is treatable by laparoscopic sacrocolpopexy. Recent data suggest that absorbable sutures are a feasible and appealing option for mesh attachment given a potential decreased risk of complications related to mesh erosion. The aim of the present study was to demonstrate the non-inferiority of absorbable sutures to permanent sutures for laparoscopic sacrocervicopexy. MATERIAL AND METHODS: We performed a randomized, single-blinded, non-inferiority trial comparing late-absorbable sutures…

Computationally modelling the mechanical behaviour of turtle shell sutures-A natural interlocking structure.

Sutures, the soft collagenous tissue joining interdigitating bony protrusions on the edges of bone plates, play a significant mechanical role in allowing a turtle shell to respond optimally to a range of loading regimes. In this contribution, qualitative and quantitative aspects of the mechanical behaviour of turtle shell suture regions are investigated by means of mathematical modelling. Notable features of the model include: (i) a geometrically realistic three dimensional model for the suture geometry; (ii) taking the hyperelastic, anisotropic and incompressible nature of the suture material…

Absorbable versus non-absorbable sutures for skin closure after carpal tunnel decompression surgery.

BACKGROUND: Carpal tunnel syndrome is a common problem and surgical decompression of the carpal tunnel is the most effective treatment. After surgical decompression, the palmar skin may be closed using either absorbable or non-absorbable sutures. To date, there is conflicting evidence regarding the ideal suture material and this formed the rationale for our review. OBJECTIVES: To assess the effects of absorbable versus non-absorbable sutures for skin closure after elective carpal tunnel decompression surgery in adults on postoperative pain, hand function, scar satisfaction, wound inflammation…
