Low-cost, easy-to-build noninvasive pressure support ventilator for under-resourced regions: open source hardware description, performance and feasibility testing.

AIM: Current pricing of commercial mechanical ventilators in low-/middle-income countries (LMICs) markedly restricts their availability, and consequently a considerable number of patients with acute/chronic respiratory failure cannot be adequately treated. Our aim was to design and test an affordable and easy-to-build noninvasive bilevel pressure ventilator to allow a reduction in the serious shortage of ventilators in LMICs. METHODS: The ventilator was built using off-the-shelf materials available via e-commerce and was based on a high-pressure blower, two pressure transducers and an Arduino

Antral control of gallbladder cyclic motor activity in the fasting state.

The hypothesis that gastric antrum controls the phasic contractions of gallbladder cyclic motor activity in the fasting state was tested. Gallbladder, gastric, and small bowel motor and myoelectric activity was recorded by strain gauge transducers and bipolar electrodes. Gallbladder pressure was measured manometrically by a surgically implanted intraluminal catheter. After control recordings for 4 to 6 weeks, antrectomy and gastroduodenostomy were performed. Six weeks later, bilateral truncal vagotomy was performed in each dog. Recordings were made after each surgical procedure. In the control

Ambulatory blood pressure is associated with polymorphic variation in P2X receptor genes.

The P2X receptor gene family encodes a series of proteins that function as ATP-gated nonselective ion channels. P2X receptor channels are involved in transducing aldosterone-mediated signaling in the distal renal tubule and are potential candidate genes for blood pressure regulation. We performed a family based quantitative genetic association study in 248 families ascertained through a proband with hypertension to investigate the relationship between common genetic variation in the P2X4, P2X6, and P2X7 genes and ambulatory blood pressure. We genotyped 28 single nucleotide polymorphisms, which

Structure, innervation, mechanical properties and reflex activation of a striated sphincter in the vestibule of the cat vagina.

Vaginal constriction might be important for reproduction in mammals, but existing information is both limited and controversial. This paper shows the structure, mechanical properties, innervation and reflex response of a striated sphincter in the vestibule of the cat vagina. A Foley catheter coupled to a pressure transducer detected in the lumen of the vestibule a pressure wave that was induced by stimulation of the external branch of the motor pudendal nerve. The peak pressure of the wave induced by bilateral stimulation (30.6 cm H(2)O) was about double of the peak pressure wave induced unila

Basal joint arthroplasty and carpal tunnel release through a single incision: an in vitro study.

PURPOSE: Carpal tunnel syndrome coexists with basal joint arthritis in a large percentage of patients. These 2 conditions are often treated surgically through separate incisions. The purpose of this cadaver study is to show the effect of trapeziectomy and transverse carpal ligament release from the scaphoid tubercle on carpal canal pressures. This technique may permit both problems to be addressed through the same Wagner incision. METHODS: In 4 fresh frozen cadaver limbs pressures in the carpal canal were elevated to 30 mm Hg through a percutaneously placed balloon. Pressures were measured us

A simple and reliable technique to monitor intracranial pressure in the rat: technical note.

A technique for intracranial pressure (ICP) monitoring in the rat that uses a permanent cisterna magna cannula is described. The cannula is placed into the subarachnoid space through the atlanto-occipital membrane with the operating microscope and is secured with cement. The distal end is connected to a pressure transducer and a polygraph recorder. To study the consistency of this technique, 12 anesthetized adult rats were subjected to baseline ICP measurements 2 days after placement of the cannula. Baseline pressures ranged between 1.0 and 10.0 cm H2O, with a mean of 5.6 cm H2O. Respiratory v

Respiratory rhythmically regulated electrical stimulation of paralyzed laryngeal muscles.

Electrical stimulation of the posterior cricoarytenoid muscle, synchronized with inspiration, was achieved in dogs, utilizing a radio frequency stimulus triggered by a chest wall expansion transducer. This system brings about the abduction of the paralyzed vocal cord for the entire duration of inspiration, which allows a normal flow of air through the larynx. The stimulation system could be tested successfully in actual experiments in dogs with artificial paralysis of the recurrent laryngeal nerve. Its effectiveness could be proved through observation of the vocal cord movements (photographic

Is prostaglandin F2 alpha involved in the increased myometrial contractility of primary dysmenorrhoea?

The concentrations of prostaglandin F2 alpha (PGF2 alpha) and E2 (PGE2) in menstrual fluid collected daily from 13 women with primary dysmenorrhoea and 11 matched controls, were compared with the pattern of uterine contractility during the hour following the menstrual fluid collection. The intra-uterine pressure (IUP) was measured using a micro-transducer catheter and the tracings analysed. On Day 2 the concentration of PGF2 alpha correlated with the peak area, but not with amplitude, duration or rate of contraction. These findings add additional support to the hypothesis that increased produc

Modeling the upper airway: A precursor to personalized surgical interventions for the treatment of sleep apnea.

An accurate benchtop model was developed to mimic the different forms of human upper airway collapse in adult sleep apnea patients. This was done via modeling the airway through digital imaging. Airway representative models were then produced in two steps via a customized pneumatic extrusion 3D printing system. This allowed the pressure of collapse and planes of collapse to be manipulated to accurately represent those seen in sleep apnea patients. The pressure flow relationships of the collapsible airways were then studied by inserting the collapsible airways into a module that allowed the cha

Infratentorial intracranial pressure monitoring in neurosurgical intensive care unit.

Monitoring intracranial pressure (ICP) is an important element of neurosurgical critical care that is used primarily as an indicator of adequate cerebral perfusion. Such monitoring is usually done with intraparenchymal, subdural or intraventricular pressure sensor connected to a pressure transducer system. In the past, multiple studies have shown that there are certain pressure gradients between various intracranial compartments, especially if there is some focal intracranial pathological process. Several clinical and laboratory studies measured ICP inside the posterior fossa by placing the se

Calculation of esophageal variceal wall tension by direct sonographic and manometric measurements.

BACKGROUND: LaPlace's law determines the wall tension of a tubular system by measuring the radius (r), wall thickness (w), and pressure gradient of a tubular structure: wall tension = pressure gradient x r/w. In theory, the determination of wall tension could provide the most accurate method of predicting the likelihood of esophageal variceal rupture. Using high-resolution endoluminal sonography, we measured the variceal wall thickness and radius. Using direct needle puncture, we measured the intravariceal pressure in three patients. We then calculated the wall tension by the LaPlace equation

Overnight shunt pressure monitoring in children.

We audited the value of overnight shunt pressure monitoring in 17 children with symptoms of possible shunt dysfunction but no other supportive evidence. The shunt was accessed by a 25G butterfly connected to a disposable arterial pressure monitor positioned on the ipsilateral shoulder whose output was recorded continuously, while noting posture, sleep state and symptoms. Initially traces were analysed only for the portion spent both asleep and supine or prone. Six showed pressure peaks while asleep: of these four underwent shunt revision with remission of their symptoms and two resolved sponta

Exploring upper airway and laryngeal resistance to noninvasive ventilation in healthy awake adults.

The interaction between anatomical structures, pressure, and airflow impacts the airway resistance. The airflow during noninvasive ventilation (NIV) relies on the upper airway and laryngeal patency. This study aimed to quantify the airflow resistance at these levels during NIV. In this cross-sectional study examining 10 healthy, awake adults, we established a NIV setup incorporating a continuous video-recorded transnasal laryngoscopy and simultaneous airway pressure measurement using a transducer positioned above and below the vocal folds. Airflow and mask pressure were recorded by a pneumotac

A psychophysical study of discomfort produced by repeated filling of the urinary bladder.

Psychophysical studies were performed in 10 healthy, female volunteers using urinary bladder distension (UBD) as a visceral stimulus. Stimulus methodology was similar to that used clinically for obtaining cystometrograms with a fixed-rate (100 cc/min normal saline) filling of the urinary bladder, occasional pauses and simultaneous measure of bladder pressure using a catheter-transducer assembly. During bladder filling, subjects were asked to report sensations by verbal report and by using an electronic, hand-held, visual-analog-scale device. Sensations evoked by UBD were generally localized to

Intra-abdominal pressure measurements in lateral decubitus versus supine position.

OBJECTIVE: Intra-abdominal pressure (IAP) has traditionally been measured in the supine position, however, measuring the pressure in lateral semi-recumbent position has not been studied. DESIGN: A single centre prospective 1-day study. PATIENTS: 10 patients admitted for more then 24 hours who were mechanically ventilated and had an indwelling urinary catheter. METHODS: Inclusion criteria included were age > 18 years, sedated to a RASS score of -5 and mechanically ventilated. The pressures were measured via the bladder with the mid-axillary line as zero reference point. When patients were nu

Pressure on anterior region of palate during thumb-sucking.

Measurement of the pressure applied to the anterior region of the palate and incisor region of the mandible during thumb-sucking was carried out 3 female children. A polyethylene bag embedded with a high-sensitivity small pressure sensor was fixed on the ventral side of the thumb so that the baroreceptor could be interposed between the thumb and palate during thumb-sucking. The children were allowed to perform habitual thumb-sucking, and the resulting pressure signals were detected with a high-response dynamic strainmeter and recorded. Measured peak pressures were about 2-4.5 kgw, with large i
