Altered Bone Mechanics, Architecture and Composition in the Skeleton of TIMP-3-Deficient Mice.

Tissue inhibitor of metalloproteinases-3 (TIMP-3) maintains a healthy extracellular matrix by regulating matrix metalloproteinases (MMP), disintegrin-metalloproteinases (ADAM), and disintegrin-metalloproteinases with ThromboSpondin-like motifs (ADAMTS) activity. Currently, there is a need for a comprehensive understanding of the effects of TIMP-3 on the bone quality and integrity. In this study, we examined the mechanical, morphological, and compositional properties of TIMP-3 knock out (Timp-3 -/-) mouse bone. We hypothesize that the lack of TIMP-3 plays an important role in maintaining the ov

Histomorphometric, physical, and mechanical effects of spaceflight and insulin-like growth factor-I on rat long bones.

Previous experiments have shown that skeletal unloading resulting from exposure to microgravity induces osteopenia in rats. In maturing rats, this is primarily a function of reduced formation, rather than increased resorption. Insulin-like growth factor-I (IGF-I) stimulates bone formation by increasing collagen synthesis by osteoblasts. The ability of IGF-I to prevent osteopenia otherwise caused by spaceflight was investigated in 12 rats flown for 10 days aboard the Space Shuttle, STS-77. The effect IGF-I had on cortical bone metabolism was generally anabolic. For example, humerus periosteal b

Sclerosing mucoepidermoid carcinoma with eosinophilia of the thyroid: report of two patients, one with distant metastasis, and review of the literature.

Sclerosing mucoepidermoid carcinoma with eosinophilia (SMECE) is a recently recognized malignant neoplasm of the thyroid gland. Two additional cases of this condition which occurred in a 70-year-old woman and a 69-year-old woman are presented. The case of the 70-year-old woman (patient 1) is the first report of distant metastasis, besides lymph node metastasis, for this type of tumor. The patient initially presented with a thyroid mass, and the thyroid gland with surrounding cervical lymph nodes was removed. Because of focal keratin "pearl" formation, the tumor was misinterpreted as a metastat

Effectiveness of Occupational Therapy Interventions for Musculoskeletal Shoulder Conditions: A Systematic Review.

People with musculoskeletal disorders of the shoulder commonly experience pain, decreased strength, and restricted range of motion (ROM) that limit participation in meaningful occupational activities. The purpose of this systematic review was to evaluate the current evidence for interventions within the occupational therapy scope of practice that address pain reduction and increase participation in functional activities. Seventy-six studies were reviewed for this study-67 of Level I evidence, 7 of Level II evidence, and 2 of Level III evidence. Strong evidence was found that ROM, strengthening

Blunt cardiac rupture in a patient with a history of total correction of tetralogy of Fallot.

A 35-year-old man with a history of total correction of tetralogy of Fallot (TOF) fell down while riding a bike and experienced blunt cardiac rupture. His vital signs were stable because the bleeding was limited by an adhesion caused by the previous operation. Chest computed tomography clearly displayed the ruptured points, and an emergency operation was performed. Because a pneumothorax was suspected, a cardiopulmonary bypass was established with a femorofemoral bypass while the patient was conscious before artificial ventilation was initiated. Two ruptured points were detected on the anterio

[A cerebral watershed infarction after general anaesthesia in a patient with increased anti-cardiolipin antibody level].

During the first generalised epileptic attack, a patient suffered a humerus fracture which necessitated an operation. This patient also had a history of spontaneous lung emboli and an elevated anti-cardiolipin plasma level for which coumarin was prescribed but was stopped preoperatively. After induction of general anaesthesia for a total shoulder arthroplasty, the patient became hypotensive and the bispectral index recorded perioperatively dropped to 0. Postoperatively, the patient developed signs of a unilateral borderzone cerebral infarct in the area of the medial cerebral artery. The possib

Localizing the course of the radial nerve based on anatomical landmarks: A cadaveric study.

BACKGROUND: Surgeons treating humeral shaft fractures must be able to reliably locate the radial nerve in the posterior arm to limit iatrogenic injury. This study aims to improve the understanding of radial nerve anatomy by increasing the sample size, comparing bilateral upper extremities, and analyzing the impact of sex on its location. METHODS: Twenty-five pairs (n = 50) of fresh cadaveric upper extremities were studied with the cadaver in the lateral position. The shoulder was abducted and internally rotated and the elbow flexed to 90° and supported, as done intraoperatively. A triceps-spl

Risk factors for instability and dislocation after reverse total shoulder arthroplasty: a systematic review.

BACKGROUND: The number of reverse total shoulder arthroplasties has increased due to demographic changes, leading to more revision surgeries. Postoperative instability or dislocation is a frequent and challenging complication that impairs clinical outcomes. This study aimed to identify risk factors for instability following reverse total shoulder arthroplasty and to evaluate the underlying mechanisms described by Abdelfattah et al. METHODS: A systematic literature review was conducted using PubMed and Google Scholar with MeSH terms and keywords including "shoulder replacement," "shoulder arthr

Traumatic Asymmetric Bilateral Shoulder Dislocation: The First Case Report in Mexico.

INTRODUCTION: Asymmetric bilateral shoulder dislocation is an exceptionally rare pattern of glenohumeral injury characterized by simultaneous dislocation of both shoulders in different directions. Since its first description in 1985, only a limited number of cases have been reported worldwide. Most cases are associated with seizures or electrical injuries, while traumatic mechanisms are uncommon. Reporting additional cases is important to improve recognition and management of this unusual injury pattern. CASE REPORT: We report the case of a 39-year-old Hispanic male who presented to the emerg

Anterior Shoulder Instability Management: Indications, Techniques, and Outcomes.

Anterior shoulder instability is common in young athletes. Male individuals younger than 20 years who are involved in contact sports are at particular risk of injury and recurrence. Essential imaging includes radiography and magnetic resonance imaging in all patients, with 3-dimensional computed tomography being helpful to evaluate glenoid bone loss and Hill-Sachs lesions. Evaluation of the glenoid track is essential to help determine appropriate treatment because off-track scenarios in which the Hill-Sachs width is greater than the glenoid width impart a risk of failure with isolated arthrosc
