BACKGROUND: While whole blood (WB) is increasingly utilized to mitigate trauma-induced coagulopathy, evidence supporting its use in blunt trauma-associated prehospital cardiac arrest (PHCA) remains scarce. We compared the association of WB versus component therapy (CT) with mortality and early resuscitation balance among patients who achieved return of spontaneous circulation (ROSC).
METHODS: This retrospective cohort study used National Trauma Data Bank data from 2020 through 2024. Adults aged 16 years or older with blunt trauma, documented PHCA, a nonzero initial emergency department systol

INTRODUCTION: Thromboelastometry evaluates viscoelastic changes in the coagulation process. It offers a graphic representation of the formation of the coagulum, its stability and the presence of lysis.
OBJECTIVE: This first case of transfusion management guided by thromboelastography in Mexico and we conducted a review of the literature.
METHOD: A metasearch search was performed (PubMed, Scielo, Medigraphic) with the words thromboelastometry, coagulopathy, transfusion medicine and the most influential works were included.
CONCLUSIONS: The rotational thromboelastometry is a diagnostic tool t

The principal result of the development of hemovigilance since 1994 has been the declaration of undesirable effects likely to be due to transfusions of labile blood products. Using the 1,694 cases of undesirable effects registered, it seemed worthwhile to us to analyze the distribution of the signs noticed, their frequency and the types of blood products responsible. This analysis allowed us to observe that the majority of reactions were shivery-feverish (47%) or allergic (24%). Most of them are linked to platelet concentrate transfusions especially simple donor platelets (with a frequency of

The New Guideline for Transfusion Practice (TP) and the New Criteria for Indication of Blood Products for Transfusion (BPfT) were revised in September 2005. Both are based on the New Blood Law approved in July 2003, which defines the principles of TP and determines the responsibility of the related parties. The New Blood Law and the revised guidelines require safe and appropriate TP; namely, avoid mismatched transfusion, determine indication appropriately and supply sufficiently safe blood products to cover the domestic demand. The New Guideline for TP defines the essential points of the hospi

BACKGROUND: Intrauterine transfusion (IUT) is an invasive but critical and potentially life-saving intervention for severe fetal anemia with demonstrated improvement in outcomes. The fetus is vulnerable to hemodynamic alterations and transfusion-related adverse events; therefore, special consideration must be given to blood component selection and modification. There is widespread IUT practice variability, and existing guidance primarily relies on expert opinion and single center experiences.

BACKGROUND AND OBJECTIVES: Despite safety measures to minimize the risk of transfusion-transmitted infections, a residual risk remains. To trace and review some such cases, we ask donors to notify the blood centre if they are diagnosed with an infection after they donate blood.
MATERIALS AND METHODS: We analysed all data on postdonation cases of hepatitis A reported between 2007 and 2012. Archived specimens from these donors were tested for hepatitis A virus (HAV) using anti-HAV IgM/IgG and HAV-PCR as markers. If any of the test results were positive, we reviewed the medical records of the re

BACKGROUND: Fresh frozen plasma (FFP) transfusion is used to manage coagulopathy and bleeding in cardiac surgery patients despite uncertainty about its safety and effectiveness.
METHODS: We performed a propensity score matched analysis of the Australian and New Zealand Society of Cardiac and Thoracic Surgeons National Cardiac Surgery Database including patients from 39 centres from 2005 to 2018. We investigated the association of perioperative FFP transfusion with mortality and other clinical outcomes.
RESULTS: Of 119,138 eligible patients, we successfully matched 13,131 FFP recipients with

The impact of variant Creutzfeldt-Jakob disease (vCJD) on the clinical practice of haemophilia in the UK is coloured by the haemophilia community's experience of hepatitis C virus and human immunodeficiency virus (HIV) transmission via plasma-derived therapies in the 1980s, when the delay in recognizing and acting on the potential risks cost many patients their lives and left others to manage another chronic disease. This crisis prompted organisations such as the United Kingdom Haemophilia Centre Doctors' Organisation to advocate for the introduction of haemophilia therapies that would not be

Transfusion related acute lung injury (TRALI) is a rare but potentially severe complication of blood transfusion, manifested by pulmonary oedema, fever and hypotension. The signs and symptoms are often attributed to other clinical aspects of a patient's condition, and therefore, TRALI may go unrecognised. It has been estimated to be the third cause of transfusion related mortality, so it should be better diagnosed. Cases are related to multiple blood units, such as white blood cells, red blood cells, fresh frozen plasma, platelets or intravenous immunoglobulins. Physiopathology of TRALI is poo

For 30 years it has been known that immunodeficient patients can have a fatal reaction to transfused blood components containing viable lymphocytes. This risk of a transfusion-associated graft-versus-host reaction can be totally eliminated by irradiation of the blood prior to transfusion. Immunocompetent individuals can have this potentially fatal reaction if the blood donor is homozygous for one of the HLA-haplotypes of the recipient. The probability of this coincidence is low when donor and recipient are unrelated, but considerably higher when they are genetically related. When HLA-matched b

Background/Objectives: Neonates with hypoxic-ischemic encephalopathy (HIE) undergoing therapeutic hypothermia (TH) frequently develop coagulation abnormalities and bleeding complications, yet data on transfusion burden and its clinical correlates in this population remain limited. Materials and Methods: We performed a secondary analysis of a retrospective single-center cohort of neonates with HIE treated with TH between 2014 and 2022. Transfusion burden was defined as receipt of at least one blood component and coagulation/plasma products during hospitalization and the total number of transfus

It appears that the vast majority of review articles on transfusion deal with complications rather than benefits of this supportive therapy. However, deduced from reports compiled by hemovigilance systems, the majority of transfusion procedures do not lead to complications; rather, the majority of prescriptions of blood components generate measurable benefits in the transfused patient. This short review attempts to analyze the causes for such a discrepancy between benefits and complications in the literature and in the everyday life in wards and introduces the role of Patient Blood Management

Febrile non-hemolytic transfusion reactions (FNHTRs) remain one of the most common adverse events associated with packed red blood cell (PRBC) transfusions. Therefore, it is of interest to compare the efficacy of leukoreduction in the prevention of febrile transfusion reactions in PRBC-transfused patients using the TACE 2 automatic component extractor and Terumo Penpol blood bags. 600 PRBC transfusions were included in the analysis, of which 300 were leukoreduced and 300 were not leukoreduced. The standard operating procedures for leukoreduction were adopted with the help of the TACE 2 device

BACKGROUND: The transfusion medical record is an important tool for providing safe and appropriate blood. However, many patients seek care at more than one hospital and this record is usually not portable. We posited that a centralized transfusion service database (CTS-D) offers benefits through tracking blood types, transfusion requirements, and detecting wrong blood in tube (WBIT).
STUDY DESIGN AND METHODS: Records held in the CTS-D from 1997 to 2010 were queried to enumerate those seen at more than one hospital versus one hospital only. Transfusion-related attributes were collected includi

The deficiency of fibrinogen, prothrombin, factor V (FV), FVII, FVIII, FIX, FX, FXI, and FXIII, called rare coagulation disorders (RCDs), may result in coagulopathies leading to spontaneous or posttrauma and postsurgery hemorrhages. RCDs are characterized by a wide variety of symptoms, from mild to severe, which can vary significantly from 1 disease to another and from 1 patient to another. The most typical symptoms of all RCDs are mucosal bleedings and bleeding at the time of invasive procedures, whereas other life-threatening symptoms such as central nervous system bleeding and hemarthroses

A retrospective study was carried out to ascertain the blood bank provision required to support a liver transplant programme and to assess the effect of intraoperative aprotinin on blood product requirements in liver transplant recipients with cirrhosis. Sixty patients with end-stage liver disease underwent 62 consecutive orthotopic liver transplants between October 1988 and January 1991. The total and intraoperative requirements of red cells, platelets and fresh frozen plasma (FFP) were analysed for three groups of liver transplant recipients, those without cirrhosis (n = 15), those with cirr

Many adverse events associated with the transfusion of allogeneic blood products have been shown to be related to the presence of allogeneic leukocytes in the blood product transfused. Until recently little attention has been paid to the leukocytes present in various blood components, however, over the past two decades it has been shown that the removal of such "passenger" leukocytes is associated with improved clinical outcomes. These include: the reduction in the incidence and severity of febrile transfusion reactions; reducing the CMV transfusion transmission risk; reducing the risk of allo
