Ring external fixators developed by Gavriil Abramovich Ilizarov from Russia are used to treat the difficult cases of infected non-union, shortening of limbs by bone lengthening, and deformity correction in joints and bones in isolation or in combination. Fixation of the involved bone with the ring is commonly achieved using four rings, each having two Ilizarov wires that are passed along the superior surface of the ring, then across the bone, and exiting out on the opposite side touching the surface of the ring. The case report herein reports a patient who kept the ring fixator on his thigh wi

Complex, perilunate fracture-dislocations of the wrist comprise severe and challenging injuries, characterised by a total loss of contact between the lunate bone and the head of the capitate as well as fracture of one, or more bones surrounding lunate bone. The principle of treatment of such injuries involves stable fixation of fractures, anatomical reduction of the displaced carpal bones with temporary arthrodesis wit K-wires to maintain their position. We present a patient who sustained a complex, perilunate fracture-dislocation of the wrist with fractures of the scaphoid, hamate and base of

Tibial tubercle osteotomy provides a safe and reliable means of extensile exposure of the knee. A technique was developed using a long osteoperiosteal segment including the tibial tubercle and upper tibial crest leaving lateral muscular attachments intact to this bone fragment. The bone fragment was reattached to its bed with two cobalt-chromium wires passed through the fragment and through the medial tibial cortex. The procedure was used in 71 knees to expose the joint for total knee arthroplasty, and the follow-up period was one to five years. All healed uneventfully, and no significant comp

PURPOSE: This study aims to introduce the "tripod" technique using cannulated compression screw and two anti-rotational K-wires for treatment of unstable scaphoid waist fracture nonunion, and to compare it with the single cannulated screw fixation technique in term of scaphoid union and surgical outcomes.
METHODS: It was a retrospective study. From January 2014 to March 2020, 103 consecutive patients with scaphoid waist fracture nonunion treated with the tripod fixation and bone grafting (n = 45) or with single cannulated compression screw and bone grafting (n = 58) were included. All the pro

BACKGROUND: Although nonunions of the proximal humerus are rare, they cause significant disability to patients. Surgical reconstruction is challenging, especially with small and excavated head fragments. A promising surgical option is open reduction and stabilization using the Humerusblock device along with tension wires. The aim of this retrospective investigation was to evaluate the clinical and radiological results of this procedure.
MATERIALS AND METHODS: Fifteen patients with symptomatic surgical neck nonunions were treated with open reduction and internal fixation using the Humerusblock

PURPOSE: To investigate the outcomes of vascularized distal radius pedicled bone grafting secured with K-wires for scaphoid nonunions with small avascular proximal fragments.
METHODS: We included patients with scaphoid nonunions and small, avascular proximal fragments that were too small for screw fixation. The mean size of the proximal pole fragments was 21% (range, 9% to 28%) of the entire scaphoid, based on quantitative radiographic measurement. All patients had distal radius bone grafting based on the 1,2-intercompartmental supraretinacular artery pedicle and fixation with K-wires. There

For the prophylaxis and treatment of bony infections antibiotics are locally used. Since several decades antibiotics mixed with bone cement (methylmethacrylate) are successfully used in prosthetic surgery and a gentamicin coated tibial nail is approved in Europe for fracture stabilization. The goal of the present study was to investigate if gentamicin, locally applied from a polymeric coating of intramedullary nails, might interfere with the bone healing process. Female Sprague Dawley rats (n = 72) were used and the tibiae were intramedullary stabilized with Kirschner-wires (k-wires) after ost

BACKGROUND: Extra-articular dorsal closing wedge osteotomy has been used with successful results for Freiberg disease; however, there has been concern regarding the technical difficulty. The purpose of this study was to address technical tips and report on the clinical and radiographic outcome using Kirchner wire-assisted dorsal closing wedge osteotomy.
METHODS: A retrospective review of 13 patients with a mean age of 29.1 years was conducted. Extra-articular dorsal wedge osteotomy was performed after debridement of the joint. Two Kirschner wires were inserted and used as a joystick to manipu

Osseous hemangiomas are mainly seen in the skull or spine and rarely occur in other bones. They are small lesions usually localized in the metaphyseal and diaphyseal regions. This article describes a rare case of a giant intraosseous hemangioma occupying nearly the whole humerus for 28 years. Initially, the patient, a 36-year-old woman, had been misdiagnosed with fibrous hyperplasia of the left humerus. We intended to curette the lesions and transplant fibula to fill the bone defect, but the initial surgical attempt could not be completed due to massive intraoperative bleeding. In the second s

Reconstruction of nasal contour where skeletal support is deficient or absent has usually been achieved using autogenous bone. Membranous bone taken from the cranium is clearly superior to rib or iliac crest when used as autografting material to the craniofacial skeleton. Conventionally, the bone graft is rigidly fixated to the recipient nasal bone with either metal plate-screw systems or Kirschner wires. Reported here are the results of a single biodegradable screw fixation of the split calvarial graft that is used for nasal reconstruction. Ten patients with moderate to severe saddle nose def

INTRODUCTION: Direct, rigid indirect, and nonrigid indirect absolute anchorages using temporary anchorage devices (TADs, mini-implants/miniscrews) can provide promising opportunities for challenging, yet common, orthodontic tooth movements such as molar protraction. Rigid rectangular wire and ligature wire are the most common methods of attaching a tooth to a miniscrew in indirect anchorages. We aimed to provide a comparison of the rigidity of the connecting wire in terms of stress on the miniscrew, the anchorage loss, and the risk of root resorption using finite element analysis (FEA).

Management of multiple hand fractures in a mangling injury requires an individualized approach that balances biomechanical stability with soft tissue preservation and surgical efficiency. While plate-and-screw constructs offer strength, less rigid methods like Kirschner wires may achieve comparable outcomes, particularly when soft tissues are intact. Evidence showed that rigid fixation does not consistently give better outcomes, and early mobilization remains key to preventing stiffness. A tailored strategy using both absolute and relative stability principles can optimize function while minim

The use of cannulated screws and titanium plates to reinforce the sternal closure or to treat sternal dehiscence after median sternotomy has already been suggested in several articles. The system proposed here has some important advantages over those already described. Moreover, thanks to its characteristics, this system can also be used to treat pathologies affecting the entire rib cage. The system consists of a first threaded cannulated screw that is inserted in the bone or chondral cartilage and accommodates a cap screw that is tightened into the first screw and fixes a plate according to t

The incidence of pertrochanteric hip fractures continues to rise as the aging population expands. Despite advancements in techniques and implants, failure rates remain up to 16% in the literature. The recognition of challenging fracture patterns and the use of meticulous preoperative planning can help to avoid common pitfalls. Understanding fluoroscopic views for reduction assessment and implant placement is instrumental in preventing fixation failure. In addition, adjunctive techniques including the use of a bone hook, cerclage wire, and colinear clamps can facilitate reduction efforts. It is

BACKGROUND: Sternal pins are adjuncts used to enhance sternal stability after sternotomy, but only a few complications associated with them have been previously reported.
MATERIAL, METHODS AND RESULTS: A 42-year-old woman underwent pulmonary valve replacement through re-sternotomy for pulmonary valve regurgitation after repaired tetralogy of Fallot. Because of severe sternal deformity, the sternum was refixed using a sternal pin in addition to sternal wires. However, postoperative computed tomography unexpectedly revealed migration of the sternal pin towards the right coronary artery, necessi

CASE: A Salter-Harris II fracture of the small finger middle phalanx with complete extrusion of the epiphyseal fragment in a 6-year-old boy is described. Closed reduction was unsuccessful. Open reduction was performed, the epiphysis was reduced, and a single Kirschner wire was placed obliquely across the fracture and joint.
CONCLUSION: This is an extremely rare injury. Open reduction and pinning was successful in preserving functional range of motion. Despite eventual growth arrest, there was no clinically significant angular deformity, and the joint was preserved. Surgical technique and earl
