Novel left ventricular cardiac synchronization: left ventricular septal pacing or left bundle branch pacing?

It is well recognized that a high burden of right ventricular pacing results in deleterious clinical outcomes over the long term. His bundle pacing can achieve optimal ventricular synchronization; however, relatively high pacing thresholds, low R-wave amplitudes, and the long-term performance have been concerns. Recently, left ventricular (LV) septal endocardium pacing (LVSP) has demonstrated improved acute haemodynamics. Another novel technique of intraseptal left bundle branch pacing (LBBP) via transvenous approach has been adopted rapidly and has demonstrated its feasibility and effectivene

Role of concealed conduction and allied phenomena in the genesis, maintenance, and termination of cardiac arrhythmias.

Concealed conduction is a common electrocardiographic phenomenon whereby a series of events may occur as a result of incomplete propagation of an impulse. The occurrence, maintenance, and resolution (termination) of several events such as functional block and cardiac arrhythmias are linked to concealed conduction. This phenomenon should be suspected on the surface electrocardiogram whenever an arrhythmic event occurs unexpectedly. Several electrophysiological concepts such as transseptal conduction and linking phenomenon are close allies of concealed conduction. His-bundle electrocardiography

The QRS Complex: Normal Activation of the Ventricles.

The ventricular conduction system starts below the His bundle, where it bifurcates into the right and left bundle branches that taper out to the subendocardial Purkinje network, which activates the ventricular myocardium. This system is responsible for the synchronized and almost simultaneous activation of both ventricles. On the surface electrocardiogram, the ventricular conduction system lies in the terminal portion of the PR interval, whereas the QRS complex composed of the electrical currents originating from ventricular depolarization. This article reviews the main electroanatomic feature

[An elderly case of supernormal conduction in the posterior division of the left bundle branch].

An electrophysiologic study was performed in a 95-year-old man with bifascicular block (right bundle branch block and left anterior hemiblock). During sinus rhythm (AA interval = 980 ms), every sinus beat was conducted to the ventricle. The AH interval was 130 ms and HV interval was 50 ms. A programmed premature atrial stimulation was performed after 8 paced beats at a slightly shorter cycle length than the sinus cycle length (900 ms). As the atrial coupling interval was shortened, the H1H2 interval also shortened. At an H1H2 interval of 680 ms the premature atrial beat was blocked distal to t

[Topographic diagnosis of atrio-ventricular conduction defects].

Atrioventricular (A-V) conduction was evaluated in 51 patients who underwent electro-physiological study. Eighteen patients had nodal conduction delay (A-H greater than 150 ms), six of them were congenital, in two others it was associated to a His-Purkinje (H-V) delay. None of them had bundle branch block in the surface ECG. The conduction delay was located within the his bundle in 15 patients (29.4%). In three of them, a split His bundle electrogram was recorded; in the other 12 (80%), His bundle stimulation normalized QRS morphology; in all of these patients H-V interval was longer than 70 m

Impacts of Left Bundle/Peri-Left Bundle Pacing on Left Ventricular Contraction.

BACKGROUND: His-bundle pacing is an emerging routine technique that avoids pacing-dependent side effects. However, the success rate of His-bundle pacing is not 100%. METHODS AND RESULTS: Left bundle pacing or peri-left bundle pacing (LBP/peri-LBP) are recently developed techniques that directly capture the left bundle or ventricular tissue near the left bundle. We evaluated the success rate of LBP/peri-LBP in patients whose treatment with His-bundle pacing failed. In addition, we evaluated left ventricular contraction and desynchrony after LBP/peri-LBP. CONCLUSIONS: LBP/peri-LBP is an altern

Atypical Wenckebach AV Block in the Infra-Hisian Region: Clinical Implications and Management.

BACKGROUND Second-degree atrioventricular (AV) block is a frequently encountered conduction abnormality on surface electrocardiogram (ECG). However, it does not always imply a block at the AV nodal level. In rare cases, this block can occur below the bundle of His, within the infra-Hisian region of the His-Purkinje system. While the incidence of infra-Hisian block is generally low in the general population, it becomes more common in specific high-risk groups, such as older adults and individuals with structural heart disease. Infra-Hisian block carries a significant risk of progressing to comp…

Tailored electrocardiographic-based criteria for different pacing locations within the left bundle branch.

BACKGROUND: Electrocardiographic (ECG)-based criteria are used to confirm left bundle branch (LBB) pacing (LBBP), but current cutoff values have never been validated for different pacing locations. OBJECTIVE: The purpose of this study was to describe diagnostic performance of V6-R wave peak time (RWPT), V6-V1 interpeak interval, and aVL-RWPT for different pacing sites within the LBB and to determine 100% specific values for each criterion at each pacing location. METHODS: Consecutive patients with confirmed LBBP were selected. Population was divided into subgroups based on the site of pacing

Miniseries 2-Septal and paraseptal accessory pathways-Part I: The anatomic basis for the understanding of para-Hisian accessory atrioventricular pathways.

AIMS: Although the anatomy of the atrioventricular conduction axis was well described over a century ago, the precise arrangement in the regions surrounding its transition from the atrioventricular node to the so-called bundle of His remain uncertain. We aimed to clarify these relationships. METHODS AND RESULTS: We have used our various datasets to examine the development and anatomical arrangement of the atrioventricular conduction axis, paying particular attention to the regions surrounding the point of penetration of the bundle of His. It is the areas directly adjacent to the transition of…

A very unusual wide QRS tachycardia: VT or SVT?

INTRODUCTION: A 13 old girl presented with recurrent wide QRS tachycardia since she was 4. METHODS: An electrophysiologic study was performed. RESULTS: The electrophysiologic study showed that QRS complexes identical to those of the tachycardia could be elicited with premature atrial extrastimuli but with a shorter atrioventricular (AV) delay when the QRS was wide compared with narrow QRS complexes. The tachycardia was ablated at 9 o'clock on the tricuspid annulus demonstrating the presence of an atriofascicular fiber. CONCLUSION: We believe that this atypical behavior can be explained by A

Recording an isoelectric interval as an endpoint of left bundle branch pacing with continuous paced intracardiac electrogram monitoring.

BACKGROUND: The present study aimed to evaluate the feasibility and safety of the novel left bundle branch pacing (LBBP) procedure that uses isoelectric interval as an endpoint for lead implantation. METHODS: A total of 41 patients with indications for pacing were enrolled. All patients underwent a novel LBBP procedure guided by recording an isoelectric interval as an endpoint for lead implantation. The procedural details and electrophysiological characteristics were then analyzed. RESULTS: A total of 38/41 (92.7%) cases were confirmed of left bundle branch (LBB) capture. An isoelectric inte

Group beating bradycardia due to two level AV junctional conduction block including combined Wenckebach and Mobitz 2 fascicular block.

A case of a complex advanced two level AV junctional conduction block is described with electrocardiographic features consistent with the combination of incompletely concealed Wenckebach in the left anterior fascicle of the left bundle branch, and most probably in the right bundle branch and with Mobitz type II in the left posterior fascicle of the left bundle branch, resulting in intermittent trifascicular block.

Evaluation of the Criteria to Distinguish Left Bundle Branch Pacing From Left Ventricular Septal Pacing.

OBJECTIVES: This study sought to assess the predictive value of the proposed electrocardiogram and intracardiac electrogram characteristics for confirmation of left bundle branch (LBB) capture. BACKGROUND: Previously proposed criteria to distinguish left bundle branch pacing (LBBP) and left ventricular septum (LVS) pacing (LVSP) have not been fully validated. METHODS: A His bundle pacing lead, an LBBP lead, and a multielectrode catheter at the LVS were placed. Direct LBB capture was defined as demonstration of retrograde His potential on the His bundle pacing lead and/or anterograde left con

Left bundle branch pacing: A comprehensive review.

Cardiac pacing is the only effective therapy for patients with symptomatic bradyarrhythmia. Traditional right ventricular apical pacing causes electrical and mechanical dyssynchrony resulting in left ventricular dysfunction, recurrent heart failure, and atrial arrhythmias. Physiological pacing activates the normal cardiac conduction, thereby providing synchronized contraction of ventricles. Though His bundle pacing (HBP) acts as an ideal physiological pacing modality, it is technically challenging and associated with troubleshooting issues during follow-up. Left bundle branch pacing (LBBP) has
