Carotid Stenting as Definitive Treatment for Free Floating Thrombus-Review of 7 Cases.

BACKGROUND AND PURPOSE: Free floating thrombus (FFT) is a rare condition. The optimal treatment strategy is yet to be determined although medical management with anticoagulation is the mainstay. This article reports experience of treating FFT with carotid stenting. METHODS: A retrospective analysis of a prospectively maintained database was performed to identify all patients with FFT treated with carotid stenting. For each patient the demographic data, clinical presentation, location of the thrombus, type of stent and use of adjunctive devices, e.g. balloon guide catheters, clinical and radio

Carotid Artery Stenting Versus Endarterectomy for Stroke Prevention: A Meta-Analysis of Clinical Trials.

BACKGROUND: Data conflict regarding the relative effectiveness of carotid artery stenting (CAS) and carotid artery endarterectomy (CEA) for the prevention of stroke due to carotid artery stenosis. OBJECTIVES: The authors performed an updated meta-analysis evaluating the efficacy and safety of CAS versus CEA, given recently published clinical trial data. METHODS: Databases were searched through April 30, 2016. Randomized trials with ≥50 patients, that had exclusive use of embolic-protection devices, and that compared CAS against CEA for the treatment of carotid artery stenosis were selected.

Predictors of carotid occlusion intolerance during proximal protected carotid artery stenting.

OBJECTIVES: The aim of this study was to identify predictors of occlusion intolerance (OI) developing during proximal protected carotid artery stenting (CAS). BACKGROUND: The use of proximal embolic protection devices, such as endovascular occlusion, during CAS has been demonstrated to be particularly safe and effective. However, endovascular occlusion can expose the ipsilateral hemisphere to hypoperfusion and produce transient neurological symptoms (OI). METHODS: From March 2010 to March 2012, 605 consecutive patients underwent proximal protected CAS at our institution. To identify independ…

Carotid stenting using tapered and nontapered stents: associated neurological complications and restenosis rates.

Self-expanding stent design systems for carotid artery stenting (CAS) have morphed from nontapered (NTS) to tapered (TS); however, the impact of this change is unknown. We reviewed the outcomes of CAS with these two broad categories of stents in a single-center retrospective review of 308 CAS procedures from May 2001 to July 2007. Nitinol self-expanding TS or NTS coupled with cerebral embolic protection devices were used to treat extracranial carotid occlusive disease. Data analysis included demographics, procedural records, duplex exams, and conventional arteriography. Mean follow-up was 18 m…

Comparative Safety and Efficacy of Left Atrial Appendage Occlusion Technologies for Prevention of Stroke and Bleeding in Atrial Fibrillation: A Comprehensive Meta-Analysis of Watchman Versus Oral Anticoagulation and Alternative Device Platforms.

Left atrial appendage occlusion (LAAO) offers an alternative to oral anticoagulation for stroke prevention in atrial fibrillation. We performed a systematic review and meta-analysis comparing Watchman (Boston Scientific Corporation, Marlborough, MA) with oral anticoagulants (OACs) and alternative LAAO devices. PubMed, EBSCO, and ClinicalTrials.gov were searched (January 2005 to August 2025) for randomised trials and observational studies comparing Watchman with OACs or other devices in adults with atrial fibrillation (minimum one-month follow-up). Twenty-one studies (eight randomised, 13 obser

Propensity-Matched Comparison of Oral Anticoagulation Versus Antiplatelet Therapy After Left Atrial Appendage Closure With WATCHMAN.

OBJECTIVES: In this propensity-matched analysis of post-left atrial appendage closure antithrombotic therapy, the safety and effectiveness of oral anticoagulation (OAC) and antiplatelet therapy (APT) were compared. BACKGROUND: Left atrial appendage closure with the WATCHMAN device is an alternative to OAC in patients with nonvalvular atrial fibrillation, who are at high bleeding risk. Initial trials included 45 days of post-implantation OAC, but registry data suggest that APT may suffice. Clinical and neurocognitive outcomes after transcatheter aortic valve implantation (TAVI) with cerebral protection: initial experience with a novel dual-filter device in Southeast Asia.

INTRODUCTION: Transcatheter aortic valve implantation (TAVI) is increasingly performed in patients with severe aortic stenosis. A novel dual-filter system to reduce cerebral embolism during TAVI recently became available. We aimed to assess the feasibility, safety, and clinical and neurocognitive outcomes of TAVI with cerebral protection in Asian patients. METHODS: 40 consecutive patients undergoing TAVI with cerebral protection were enrolled. All procedures were performed via femoral access using the self-expanding Evolut R/PRO or Portico, or the balloon-expandable SAPIEN 3 bioprostheses. Ba

Transcervical carotid stenting with flow reversal for neuroprotection: technique, results, advantages, and limitations.

Carotid angioplasty and stenting are progressively earning a role as a less invasive alternative in the treatment of carotid occlusive disease. The most common approach for carotid artery stenting involves transfemoral access and use of a filter or balloon device for neuroprotection. This approach has limitations related to both the site of access and the method of neuroprotection. Specifically, an aortoiliac segment with advanced occlusive or aneurysmal disease or an anatomically unfavorable or atheromatous arch and arch branches can significantly limit the safety of the retrograde transfemor

Percutaneous Mechanical Aspiration: A Game Changer for Infective Endocarditis?

The incidence of right-sided infective endocarditis is increasing with injection drug use, indwelling lines, and device implantation. Although contemporary guidelines emphasize early referral to heart valve centres and multidisciplinary endocarditis teams, many patients who meet surgical criteria are ineligible because of prohibitive risk, clinical instability, or social complexity. Percutaneous mechanical aspiration (PMA) with the AngioVac (AngioDynamics, Latham, NY) system has emerged as a minimally invasive option to debulk infected masses, expedite bacteremia clearance, and mitigate septic

Outcomes and insights from a decade of transcarotid artery revascularization in community practice.

OBJECTIVE: Transcarotid arterial revascularization (TCAR) with flow reversal is a well-established minimally invasive alternative to carotid endarterectomy. Our multicenter, private practice is an early adopter and has performed TCARs in the community since 2013. We report our real-world experience after performing 650 TCARs over 10 years in the community. METHODS: We performed 655 TCAR procedures on 588 patients between 2013 and 2024. A retrospective chart review was performed on these procedures. The cohort included 9 vascular surgeons across 10 hospitals. All procedures used the enroute tr

Seven years of CRAFT: Clinical results of a multicenter cohort of carotid artery relining with the CASPER RX stent.

BACKGROUND AND PURPOSE: Carotid artery stenting (CAS) is an established treatment for carotid stenosis or occlusion. Use of the dual-layer microporous CASPER RX stent has had mixed results in prior reports. The CaRotid Artery Filtering Technique (CRAFT) uses the distal end of the CASPER stent as an embolic protection device during deployment. We present the largest cohort of patients having undergone CAS with the CASPER stent and report our multicentre experience with CRAFT over the last seven years. MATERIALS AND METHODS: All patients undergoing CAS at three tertiary neurointerventional cent
