BACKGROUND: The spectrum of injuries to the tibial plateau is so great that no single method of treatment has been proven to be uniformly successful. The purposes of this study were to evaluate the clinical results, to identify the advantages and disadvantages and to take out useful conclusions of the application of the internal and hybrid external fixation in the treatment of these fractures.
METHOD: Sixty tibial condylar fractures, of all types, according to Schatzker's classification were treated with open reduction and internal fixation (30 patients) or with hybrid external fixation (30 p

Neglected posterior dislocation of the elbow is a common problem in developing countries. One acceptable form of treatment is open reduction and transarticular Steinmann pin fixation. However, this leads to a stable but stiff elbow. We used an improvised external fixator after open reduction in two cases, which allowed for early mobilisation with adequate stability. This option can be considered when modern implants are not available.

BACKGROUND: Ilizarov technique is useful in the management of infected nonunions of the tibia. Its main drawback is the long duration of external fixation (EF) with marked patient discomfort. Several techniques have been described for early removal of EF to avoid this problem.
METHODS: Between September 2000 and October 2001, a prospective study at a tertiary trauma center was performed. Thirty-three patients with infected nonunion of the tibial shaft were included. The eradication of infection was by debridement, followed by limb reconstruction using segment transport technique with Ilizarov

UNLABELLED: Ankle equinus can result from congenital, traumatic, neurological, and pathological etiologies. Corrective methods have been described in the literature using a combination of soft tissue releases or osseous procedures with reported complications. We present a case report of a patient with a post-traumatic fixed equinus deformity of 28 degrees at maximum dorsiflexion, treated successfully through gradual correction using a modified constrained external fixator. The patient maintained a rectus foot and was able to perform all daily activities at the final follow-up. Fixed equinus de

The optimal management of circumferential extremity burns remains a challenge. Elevation of the extremity to decrease wound swelling and avoid pressure at the point of contact on new skin grafts is a standard objective but can be frequently difficult to accomplish. Although various forms of elevation have been proposed, our purpose was to evaluate the safety and efficacy of the uniplanar external fixation (UEF) device in providing extremity elevation and stabilization while optimizing skin grafting. Intraoperative application of either upper or lower extremity UEF was performed at the time of

In this study the authors evaluate the results of internal synthesis of femoral fractures in polytraumatised patients initially treated by external fixation (EF). From January 2002 to December 2005, 39 femurs in 37 polytraumatised patients (average age 34.2 years, range 18-44) with closed fractures and an ISS>20 were initially treated with EF. There were three groups: Group A, 13 cases when conversion to internal osteosynthesis occurred after 4-7 days (average 5.6 days); Group B, 11 cases with a 4-6-month interval before internal osteosynthesis, and after investigation using MRI and scintigrap

A 50-year-old man presented with severe back pain and tenderness throughout the lumbar area after falling from a ladder. He had an unstable type-B burst fracture, with a spinal canal narrowing of 36% and an anterior height loss of 65%. His lower-limb neurological function was intact. An Ilizarov external spinal fixator was used; the pedicular half pins were inserted into the bilateral T11, T12, L2, and L3 pedicles; bilateral pedicular half pins were fixed at each level with external plates and rods. Postoperatively, the patient had a lordosis of 2 degrees and was able to walk 7 days later. The

BACKGROUND: External fixators have been noted to have a place in the orthopaedic management of problems involving the ankle and foot. We here report a case of ankle and foot contracture managed by soft tissue release and slow, gradual external fixator distraction.
METHOD: A case report of a patient with acquired ankle and foot contracture and discussion of relevant literature.
RESULTS: A 9-year old female presented to our out-patient clinic with features of right ankle and foot contracture following treatment by traditional bone setters 6 years earlier. The contracture was fixed at 30 degree

The results of limb lengthening, correction of limb deformities and solving of bone loss, by the use of Mitkovic external fixation system and minimally invasive technique on the series of 96 operations in 89 patients are shown. Only lower extremities were included in this series. The advantages of this external fixation system in comparison to other systems are pointed out. The main advantages are the optimal biomechanical conditions including unilateral but 3-dimensional frame, which provides conditions very similar to natural biomechanical features of the human long bones, simplicity of appl

The authors retrospectively reviewed their experience with circular wire external fixation in the treatment of salvage ankle arthrodesis during the past 9 years. The results of 43 cases in a difficult patient population are presented with an average follow-up of 27.0 months. Thirty-three patients (80.5%) went on to achieve a solid fusion or stable pseudarthrosis. A minimum of a 4-ring frame construct was applied for an average of 96.1 days. The major complication rate was 51.2%, including 3 below-knee amputations (7.3%), 7 unstable nonunions (17.1%), 7 cases of osteomyelitis and/or deep-space

BACKGROUND: The selection of patients with low back pain for fusion is especially difficult when previous surgical interventions failed and/or invasive tests (discograms, facet blocks) do not allow or do not clearly identify the painful motion segment. Test fixation with an external fixator may mimic a definite internal fixation, such as a fusion, and may help select patients for a more favorable result. The purpose of the study was to clarify if temporary back pain relief by external fixation is predictive for back pain relief after final internal fixation and fusion.
METHODS: A retrospectiv
