Effects of neuroactive steroids on cochlear hair cell death induced by gentamicin.

As neuroactive steroids, sex steroid hormones have non-reproductive effects. We previously reported that 17β-estradiol (βE2) had protective effects against gentamicin (GM) ototoxicity in the cochlea. In the present study, we examined whether the protective action of βE2 on GM ototoxicity is mediated by the estrogen receptor (ER) and whether other estrogens (17α-estradiol (αE2), estrone (E1), and estriol (E3)) and other neuroactive steroids, dehydroepiandrosterone (DHEA) and progesterone (P), have similar protective effects. The basal turn of the organ of Corti was dissected from Sprague-Dawley

Gentamicin alters nitric oxide production in semicircular canals and otolith organs.

OBJECTIVES/HYPOTHESIS: Intratympanic application of gentamicin is an important therapeutic option to control vertigo spells in Ménière's disease. Low doses eliminate the function of semicircular canal ampullae (SCCA) and saccule in most patients, although utricular function is maintained in many cases. Local alteration in free radical production might be responsible for these differences. Therefore, the gentamicin-induced nitric oxide (NO) production was determined in an animal model using separate organ cultures of SCCA, saccule, and utricle. STUDY DESIGN: Prospective pilot study in male gui

A controlled and sustained local gentamicin delivery system for inner ear applications.

OBJECTIVE: Intratympanic gentamicin injection (ITGI) has gained acceptance worldwide for the treatment of Ménière's disease. Reports assessing the efficacy of ITGI suffer from high variability between patients. This variability may be due to ITGI, which does not permit a sustained diffusion of gentamicin across the round window membrane. The present study investigates the effectiveness of a sustained local hydrogel system on the delivery of gentamicin into the inner ear for the treatment of Ménière's disease. METHODS: A matrix of hydrogel loaded with/without gentamicin was explored in vivo. G

A functional study on gentamicin-related cochleotoxicity in its conventional dose in newborns.

UNLABELLED: The early identification of hearing impairment allows for an intervention still in the 'critical' and ideal period of hearing and language stimulation. Pediatric ototoxicity is a very controversial topic. There have been variable percentages of ototoxicity cases in children with different aminoglycosides antibiotics. The main pediatric groups whom receive aminoglycosides are newborns with severe infections on the neonatal ICU. AIM: to check the functional aspect of the cochlear external hair cells and treatment regimens used to treat infections during the neonatal period. STUDY D

[A priori prediction of gentamicin peak concentrations: Use of a simple and practical tool].

Aminoglycosides are concentration-dependent killing antibiotics, so that it is necessary to obtain elevated peak levels to reach the pharmacokinetic-pharmacodynamic objectives. Because of their nephrotoxicity, this class of antibiotics are frequently underprescribed and giving at an insufficient dosage when prescribed. That is why therapeutical drug monitoring (TDM) of aminoglycosides is recommanded to assess efficacy. Unfortunately, this TDM allows not a dosage adaptation on the first dose of aminoglycosides. The aim of the work was to elaborate a model using simple pharmacokinetics formulae

Vestibular function and vertigo control after intratympanic gentamicin for Ménière's disease.

The aim of this study was to correlate long-term vertigo control with reduction in vestibular function after intratympanic (IT) gentamicin therapy for unilateral Ménière's disease. IT gentamicin injections were given as needed to control vertigo attacks. Vertigo frequency and changes in angular vestibulo-ocular reflex (AVOR) gain (measured using magnetic search coils and manual head thrusts) and caloric weakness were assessed before and after treatment. Better vertigo control after treatment was found with >or=60% reduction in quantitative ipsilateral horizontal semicircular canal AVOR gain fr

Discrepancies between predicted and observed rates of intravenous gentamicin delivery for neonates.

OBJECTIVES: This study aimed to investigate intravenous infusions as used in the neonatal intensive care setting, to determine the effect of gentamicin dose (mg), gentamicin concentrations (mg/ml), flow rate (ml/h) and flush volume (ml) upon the length of infusion time. METHODS: Intravenous infusions were set up to simulate administration of gentamicin to neonates. Dextrose (10%, w/v) was administered as the primary intravenous fluid at 3.8 or 18.7 ml/h. Gentamicin doses (0.5 mg/0.2 ml, 2 mg/0.2 ml, 2.5 mg/1.0 ml, or 10 mg/1.0 ml) were delivered into the intravenous line at a T-connection usi

BACKGROUND AND METHOD: This article describes and discusses a prospective randomized study with gentamicin sulfate in the surgical treatment of hidradenitis suppurativa. The purpose of the study was to investigate whether enclosure of antibiotics after primary excision and closure reduces the number of postoperative infections. Therefore, the hidradenitis lesions were excised and closed with or without enclosure of a gentamicin-collagen sponge (GC). RESULTS: A total of 200 patients were included in the study. Seventy-six patients underwent surgical excision with primary closure (PC), and 124

Measurement of ampicillin, vancomycin, linezolid and gentamicin activity against enterococcal biofilms.

BACKGROUND: Enterococci frequently cause biofilm infections but susceptibility of clinical isolates growing in biofilms has not been investigated. The minimum biofilm eradicating concentration (MBEC) has been suggested as a guide to treatment of biofilm infections. We measured an alternative endpoint, the minimum biofilm inhibitory concentration (MBIC) and compared the results with MIC and MBC. OBJECTIVES: To compare the MIC, MBC and MBIC of ampicillin, vancomycin and linezolid against enterococcal biofilms, to assess the impact of additional gentamicin and correlate findings with clinical ou

[Intratympanic interval therapy of Menière disease with gentamicin with preserving cochlear function].

For the effective treatment of Menière's disease, gentamicin should be administered at most three times: on days 1, 8 and 15. More than 50% ( n=30) of 57 patients seen between 1997 and 2000 only needed a single dose of gentamicin to obtain a sufficient reaction (single shot-therapy). Vertigo attacks were controlled in 95% of our 57 patients and there were no substantial hearing losses. Four patients lost 15 dB. Tinnitus (46%) and ear pressure (69%) could be controlled or improved. The intratympanic injection-technique (single dose=0.3 ml=12 mg) performed under local anaesthesia is simple and s
