Konno aortoventriculoplasty following arterial switch operation requires pulmonary valve disruption and right ventricular outflow tract reconstruction.

In the presence of a small aortic annulus, aortic valve replacement can be challenging. Several annular enlarging techniques have been described to allow placement of an adequately sized prosthesis. In patients who have undergone prior cardiac surgery, particularly for congenital heart defects, standard techniques may require modification. This case describes the challenges of enlarging the aortic annulus after previous arterial switch operation for transposition of the great arteries. Specifically, the Konno aortoventriculoplasty following arterial switch operation requires pulmonary valve di

[Percutaneous transcatheter valve therapy].

Aortic stenosis (AS) and mitral regurgitation (MR) are currently the commonest valvular pathologies in developed society. The current standard of treatment of severe AS and severe MR has been surgical aortic valve replacement (AVR) and mitral valve (MV) surgery. In 2002, the first human percutaneous AVR (PAVR) was performed and in 2005, results of percutaneous MV repair were also reported. The field of percutaneous transcatheter valve therapy is evolving very rapidly. There are 2 aortic percutaneous heart valves in clinical trials and several others with first-in-man data or in pre-clinical te

Case report: Buddy wire technique to facilitate atrial septal crossing during transcatheter transseptal mitral valve implantation.

BACKGROUND: Transcatheter mitral valve-in-valve implantation (MVIV) has emerged as a viable treatment option in patients at high risk for surgery. Occasionally, despite appropriate puncture location and adequate dilation, difficulty is encountered in advancing the transcatheter heart valve across interatrial septum. CASE SUMMARY: We describe a case of a 79-year-old woman with severe chronic obstructive pulmonary disease (COPD), prior surgical bioprosthetic aortic and mitral valve replacement implanted in 2007, atrial fibrillation, and Group II pulmonary hypertension who presented with progres

Four novel suture-sealing techniques to repair varying size paravalvular leaks.

BACKGROUND: Paravalvular leak (PVL) is a well-known complication of aortic valve replacement. Currently, there is no consensus on optimal intraoperative management of patients with mild-to-moderate or moderate PVL. TECHNIQUE: In this paper we present four novel surgical techniques, each combining suturing with sealing technique, to repair aortic PVLs, nullifying the need to replace the prosthesis. CONCLUSION: These techniques will offer cardiac surgeons new alternatives to the repair techniques currently used.

Aortic Root Replacement for Destructive Endocarditis - Clinic and Microbiology.

INTRODUCTION: Destructive aortic root endocarditis is associated with high mortality rates. The objective of this article was to characterize the clinical and microbiological profiles of these patients, especially concerning an already implanted aortic valve prosthesis. We also focused on prognostic factors. METHODS: Eighty patients underwent aortic root replacement due to destructive endocarditis from 1999 to 2018 in our institution. We analyzed their pre, intra, and postoperative data, outcomes, and predictors of mortality. RESULTS: Thirty-one patients had native valve endocarditis (NVE),

Melody Valve Fracture Causing Mitral Stenosis: Novel Solution for Transapical Valve-in-Valve.

The use of Melody valves in the mitral position has been introduced in clinical practice. Stent fracture is a recognized complication of Melody valve implantation in the pulmonary position; however, reports in the mitral position are rare. We present the case of an 8-year-old boy in whom complete fracture of the proximal stent struts occurred, causing acute severe mitral stenosis, and in whom urgent hybrid transapical Melody valve implantation in the fractured Melody valve was performed successfully using a novel modified technique. This modification simplified the implantation, led to reduced

Comparison of two spike areas in bileaflet mechanical valve closing sound.

We previously reported our development of a wavelet analysis system which demonstrates that in vivo bileaflet mechanical valve sound splits into two spikes at higher frequency levels and, based on this system, proposed criteria for detecting malfunctioning bileaflet valves (MBVs). However, the results of that study were only tentative due to the small number of patients with MBVs enrolled in the study. Here, we discuss the possibility of new criteria based on the scalographic properties of two spikes of bileaflet valve sound. The study cohort comprised 12 patients who each received a Carbomedi

[Surgical treatment of high-risk valvar endocarditis].

The authors have studied the immediate and long term outcome of surgical treatment of acute phase infective endocarditis in a group of 33 high risk patients with valvular heart disease. The age varied from 2 to 68 years, 18 patients were male and 22 were of the white race. The aortic valve was the most frequently involved (18/54.5%), followed by mitral valve (13/39.3%), and tricuspid valve (2/6.0%). Twenty-four patients (72.7%) were in functional class III and seven (21.0%) in functional class IV. The noninvasive studies in those patients revealed 13 cases (39.4%) with normal cardiac size. The

Pathophysiology, diagnosis, and management of hemolysis in patients with prosthetic valve related paravalvular leaks.

Paravalvular leak (PVL) is a recognized complication after prosthetic valve implantation, occurring in up to 17% of cases. While small leaks are often clinically silent, moderate or severe PVLs may result in significant hemolysis or heart failure. Hemolysis is driven by turbulent flow through the defect, leading to red blood cell fragmentation. Laboratory features include anemia, elevated lactate dehydrogenase, low haptoglobin, indirect hyperbilirubinemia, and schistocytes on peripheral smear. Diagnosis relies primarily on echocardiography, with transesophageal imaging providing superior sensi

Phialophora mutabilis endocarditis.

This is the first reported case of fungal endocarditis due to vegetative growth of Phialophora mutabilis on a prosthetic mitral valve. The patient had rheumatic heart disease with mitral and aortic stenosis. Four months after the mitral and aortic valves had been replaced by prostheses the patient developed congestive failure. Because of increased left atrial pressure, the mitral prosthesis was replaced. A large matted obstructive fungal vegetation was found on the prosthesis. Culture of this vegetation grew Phialophora mutabilis. The patient died postoperatively, and at autopsy the fungus was

Safety of low intensity oral anticoagulant therapy in patients with bileaflet mechanical aortic valve prosthesis: A propensity weighted study.

BACKGROUND: Scarce data are available on the long-term outcomes of different regimens of oral anticoagulation in an all comer population of patients undergoing aortic valve replacement with a bileaflet mechanical heart valve. METHODS: Outcomes of 88 patients discharged with a target INR of 2.0 (LOW-INR) were compared to 147 contemporary patients who have been recommended a target INR of 2.5 (CONV). Primary outcome was the composite of any thromboembolic or haemorrhagic events. Secondary outcomes were the individual components of the primary outcome, cardiovascular mortality and stroke. To red

Treating hemolysis due to perivalvular leaks: It is all about modifying micro-jets and not the volume of regurgitation.

Paravalvular leak closure may have a therapeutic effect on the extent of hemolysis that some patients experience after valve surgery. Reduction of hemolysis is most likely in a patient with a mechanical valve and is not necessarily correlated with the volume of regurgitative flow or its subsequent reduction. Hemolysis associated with biological valves are less likely to respond to perivalvular leak closure suggesting that mechanisms other than net volume of regurgitation or micro-jets are present and should be considered.

Surgical management of tricuspid valve endocarditis in the current era: A review.

The incidence of isolated tricuspid valve infective endocarditis is increasing. Medical management is the mainstay of treatment but surgical intervention is required in a subset of patients. Surgical treatment options include valve excision and replacement or valve reconstruction. We searched PubMed and the Cochrane library to identify articles to be included in this review of surgical outcomes. References of selected articles were crosschecked for other relevant studies. Surgical management of tricuspid valve endocarditis can be achieved with satisfactory outcomes. However, the optimal indica

Cardiac computed tomography for prosthetic heart valve assessment. An expert consensus document of the Society of Cardiovascular Computed Tomography (SCCT), the American College of Cardiology (ACC), the European Society of Cardiovascular Radiology (ESCR), the North American Society of Cardiovascular Imaging (NASCI), the Radiological Society of North America (RSNA), the Society for Cardiovascular Angiography & Interventions (SCAI) and Society of Thoracic Surgeons (STS).

Prosthetic heart valve (PHV) dysfunction is increasingly seen due to the increase in the number of PHV that are being implanted worldwide. Cardiac CT imaging has emerged as a valuable tool to assess PHVs and determine the cause of dysfunction. This consensus document first summarizes the available techniques for PHV assessment. Then the use of CT in PHV (dys)function assessment is discussed in detail including consensus statements for correct indications and patient selection for CT assessment of PHVs, image acquisition, reconstruction and measurement protocols and how to interpret and report

The Association Between Ascending Aortic and Left Ventricular Dimensions in Patients After Aortic Valve Replacement.

INTRODUCTION: Aortic valve replacement (AVR) is often recommended for patients with severe aortic stenosis or chronic aortic regurgitation. These conditions result in remodeling of the left ventricle, including increased interstitial fibrosis that may persist even after AVR. These structural changes impact left ventricular (LV) mechanics, causing compromised LV diameter to occur earlier than reduced LV ejection fraction (LVEF). The aim of this study was to examine the effect of left ventricular end-diastolic diameter (LVEDD) and its role in aortic expansion one year after AVR. METHODS: Sixty-

Impact of Konno Procedure on Right Heart Function.

BACKGROUND: The Konno procedure, a left-sided surgical intervention, can lead to right-sided heart failure postoperatively due to pulmonary or tricuspid valve dysfunction. CASE SUMMARY: A 62-year-old woman with prior mechanical aortic and mitral valve replacements underwent a Konno procedure. She later presented with severe right heart failure and was found to have right ventricular enlargement, mitral patient-prosthesis mismatch, an iatrogenic ventricular septal defect, and severe pulmonic and tricuspid regurgitation. DISCUSSION: At even the most experienced centers, Konno is associated wit

Percutaneous tricuspid valve implantation: two-center experience with midterm results.

BACKGROUND: Severe tricuspid valve (TV) dysfunction may lead to surgical TV replacement with a biological valve prosthesis in patients with congenital heart disease. To expand the lifetime of this valve and reduce the number of surgeries, percutaneous TV implantation (PTVI) may be an effective alternative to repeated surgery. We report on our 2-center experience with PTVI. METHODS AND RESULTS: Between 2008 and 2014, 17 percutaneous valves were implanted in 16 patients with TV bioprosthesis dysfunction (9 females) from 2 centers. Median age and weight were 31.3 years (5-77.2) and 65.2 kg (17.7
