A Review of Non-Cardiac Complications of General Anesthesia: The Current State of Knowledge.

General anesthesia, despite the constant development of anesthesiology, still carries certain risks. To provide safe anesthesia, it is crucial to properly qualify patients and to react in an appropriate manner when problems occur. It is therefore essential to have knowledge of risk factors, pathophysiology, symptoms, and management patterns regarding complications. This review comprehensively describes respiratory complications such as airway spasm, conditions leading to intraoperative hypoxemia, postoperative pulmonary complications (PPC), and complications of cross airway compromise, from as

Current Perspectives on Complications of Surgical Treatments for Male Stress Urinary Incontinence.

INTRODUCTION: Treatments for male stress urinary incontinence (SUI) include behavioral modifications, pelvic floor strengthening exercises, bulking agents, and surgical management. The most common surgical therapies for male stress incontinence include male slings and artificial urinary sphincters. Complications of these treatments are discussed in this review. AIM: To review the current literature on SUI diagnosis and the management of common complications that occur after surgical treatments of male SUI. METHODS: A literature search was performed using PubMed and Ovid to identify leading a

Advantages and limitations of small gauge vitrectomy.

Small gauge vitrectomy utilizing 23- and 25-gauge instrumentation has definite advantages, but also limitations, due to the physics of smaller instruments and sutureless surgery. Higher infusion and aspiration pressures are needed to remove the vitreous using 23- and 25-gauge probes. The advantages include decreased surgical times, less tissue manipulation, reduced inflammation and pain postoperatively with more rapid visual recovery. A disadvantage is greater instrument flexion than 20-gauge probes, making small gauge vitrectomy more appropriate for indications such as vitreous opacities, epi

Use of multiple clips for tubal occlusion in interval laparoscopic sterilization: circumstances and consequences.

The use of multiple clips for the occlusion of the Fallopian tubes has been reported in interval laparoscopic sterilization, but the circumstances leading to the performance of the multiple-clip procedure and its effects on safety and efficacy have not been carefully studied. A data set from international multi-center clinical trials of Filshie clips and Wolf (Hulka) clips was used to examine the possible reasons for performing this procedure for 102 women. Their complications, complaints, and surgical and post-surgical events before discharge and during one month of follow-up were compared wi

Acute interventions with intraocular lenses implantation.

Complications associated with the intraocular lenses (IOL) implantation can be classified into 3 groups: intraoperative, early and late complications. Some of the complications have to be treated at once, otherwise the condition may deteriorate and lead to the occurrence of various secondary complications. One of the intraoperative complications during the IOL implantation can be the impairment of the corneal endothelium which occurs due to the inadequate size of the incision. A part of the IOL loop can remain incarcerated in the corneo-scleral wound during surgery. A strong choroidal hemorrha…

Chronological age was not an independent predictor of visual recovery following cataract surgery in adults aged ≥ 75 years: a retrospective tertiary referral centre audit.

PURPOSE: To evaluate whether chronological age independently influences surgical success, intraoperative challenges, complications and visua … upils (52.9%), hazy corneas (20.6%), floppy iris syndrome (14.7%) and patient restlessness (11.8%). Intraoperative complications occurred in 18.5% of eyes. At six weeks, 76.7% achieved BCVA ≥ 6/12 and 89.9% showed visual improvement. Independent predictors of failure to achieve BCVA ≥ 6/12 were severe preoperative visual impairment ( adjusted odds ratio [aOR] 0.13, p < 0.001) …

Does residual ADP reactivity within the therapeutic range predict intraoperative thromboembolic events during endovascular treatment of intracranial aneurysms?

PURPOSE: Thromboembolic events remain one of the major complications during endovascular treatment of intracranial aneurysms. Platelet function testing is widely used to identify clopidogrel resistance, yet its role in predicting intraoperative thromboembolic complications among pharmacologically responsive patients remains unclear. This study aimed to evaluate the association between preoperative ADP-induced platelet reactivity and intraoperative thromboembolic events in patients undergoing endovascular aneurysm treatment. METHODS: We retrospectively analyzed 196 patients treated for unruptu

Intraoperative Hypotension in Patients Having Major Noncardiac Surgery Under General Anesthesia: A Systematic Review of Blood Pressure Optimization Strategies.

INTRODUCTION: Intraoperative hypotension is associated with increased risks of postoperative complications. Consequently, a variety of blood pressure optimization strategies have been tested to prevent or promptly treat intraoperative hypotension. We performed a systematic review to summarize randomized controlled trials that evaluated the efficacy of blood pressure optimization interventions in either mitigating exposure to intraoperative hypotension or reducing risks of postoperative complications. METHODS: Medline, Embase, PubMed, and Cochrane Controlled Register of Trials were searched fr

A comparison of right and left lobectomies for living donor liver transplantation: an anesthesiologist's point of view.

Because of the shortage of cadaveric donor organs, living donor liver transplantation (LDLT) has become an established therapy modality for end-stage liver disease. Based on recipient size, both right and left liver lobe grafts have been used successfully in LDLT. The aim of this study was to compare the risk of intraoperative complications and transfusion requirements between right and left lobe donors. We reviewed the charts of 54 right lobe (Group RL), 29 left lobe (Group LL), and 31 left lateral segment (Group LLS) donors who underwent lobectomy from January 2003 through January 2007. We r…

Iatrogenic and noniatrogenic extrahepatic biliary tract injuries: a multi-institutional review.

Traumatic and iatrogenic extrahepatic biliary tract injuries are rare but may lead to exceedingly morbid complications. Traumatic extrahepatic biliary tract injuries represent less than 1 per cent of all traumatic injuries. Iatrogenic injuries result in 0.2 to 1 per cent of laparoscopic or open cholecystectomies. The objective of this study was to review the incidence of biliary tract injuries--iatrogenic as well as traumatic--and their subsequent management. A multi-institutional chart review was done including Louisiana State University Health Sciences Center (LSUHSC)-Shreveport, LSUHSC-Monr

Intraoperative Use of Vasoactive Agents in Non-Cardiac Surgery: Protocol for a Post Hoc Cohort Study.

BACKGROUND: Intraoperative hypotension is a well-known clinical condition and often treated with vasoactive agents. Previous cohort studies have indicated that large numbers of patients receive this treatment, but they have been limited to specific surgical populations or single agents. Furthermore, it is unknown which agents are the most frequently used and whether certain patient factors make intraoperative vasoactive treatment more likely. With this study, we aim to describe the proportion of patients receiving intraoperative vasoactive agents and the choice of agent; to highlight patient c

Predictive vs. Flow-Derived Haemodynamic Monitoring in Major Abdominal Surgery: Associations with Intraoperative Hypotension and Postoperative Outcomes.

Introduction: Intraoperative hypotension (IOH) is a frequent manifestation of haemodynamic instability during general anaesthesia. Advances in arterial waveform analysis have led to two distinct monitoring strategies: flow-derived platforms and predictive algorithms designed to anticipate hypotension. However, prospective comparisons and their associations with IOH and postoperative outcomes remain limited. The objective was to compare predictive haemodynamic monitoring using the Hypotension Prediction Index (HPI) with flow-derived monitoring using the Vigileo/FloTrac system and to evaluate th
