Chronic lower-leg edema in patients with venous disorders was studied by means of lymphoscintigraphy. Lower extremity swelling has a broad differential diagnosis, including lymphedema, obesity-induced lymphedema, phlebolymphedema, and lipedema. Although these conditions may appear similar clinically, their pathophysiology and lymphoscintigraphic findings differ. Obesity, venous insufficiency, and abnormal adipose tissue can complicate image interpretation. We present four representative cases to illustrate characteristic imaging patterns across these conditions. These cases highlight the importance of interpreting lymphoscintigraphy in a clinical context.

We report a female infant with congenital lymphedema, facial anomalies, intestinal lymphangiectasia consistent with a diagnosis of Hennekam syndrome. At birth the patient presented with severe respiratory distress due to nonimmune hydrops fetalis, a congenital chylothorax (CC), and pulmonary lymphangiectasia. Hydrops fetalis may be present in newborns with the Hennekam syndrome. Lymphoscintigraphy can be useful in explaining pleural-pulmonary involvement of this generalized lymph vessel malformation syndrome.

Stemmer's sign is useful in diagnosing lymphedema as it is present in 92% of the cases. Indocyanine green (ICG) lymphography is also useful for diagnosing lymphedema, and it allows them to see all the anatomy of the lymphatic vessels, leaking pumping capacity, and dermal reflux. The diagnostic ability of ICG lymphography and its evaluation capability for disease severity is similar to lymphoscintigraphy which is the gold-standard examination for extremity lymphoedema but with less invasiveness and a lower cost.

We reported 99m-Tc phytate accumulation in the pyelocalyceal system of the kidney in the lymphoscintigraphic images of a 3.5 months-old male infant with chylous ascites, which was mistaken with the site of lymph leakage. SPECT/CT localized activity in the para-aortic region to the renal pelvis and on delayed images, this was disappeared. Our case illustrates the added value of SPECT/CT for the differentiation of possible false-positive findings in lymphoscintigraphy.

Lymphoscintigraphy in breast cancer usually shows lymphatic drainage to the ipsilateral axilla. Drainage to extraaxillary or contralateral axillary regions is rare and there is still controversy about its management. Due to the significant clinical impact of an accurate staging, a literature research is made based on a case of a patient with recurrence of left breast cancer with contralateral axillary sentinel lymph node detection, without evidence of lymphatic drainage to other locations.

The cutaneous lymphatic drainage of the trunk is complex and its prediction is used to rely on the classic anatomic concepts of Sappey. Lymphoscintigraphy has shown significant discrepancy between the real sentinel nodal basins and the expected sites of drainage based solely on the old lymphatic anatomy. The author presents a case of upper back melanoma with unusual nuchal sentinel node demonstrated by technetium-99m filtered sulfur colloid scintigraphy.

Chyle leak is a potentially serious complication resulting from violation of the thoracic duct, right lymphatic duct, or one of their terminal branches. It may impair nutrition and immunity, compromise and delay wound healing, prolong hospitalization, and delay adjuvant therapy. After surgery for breast carcinoma, this is a rare occurrence. Here, the author describes a novel application of lymphoscintigraphy showing thoracic duct injury after a wide excision and axillary clearance.
