Skull deformations affect approximately 45% of newborn babies, the most common ones being: plagiocephaly, brachycephaly and scaphocephaly. The first symptoms can be observed 4 to 8 weeks after birth. The causes of skull deformation in newborns can be divided into congenital ones and those acquired after birth. An increase in the incidence of acquired head deformations can be attributed to the "BACK TO SLEEP" campaign, carried out in 1992 by the American Academy of Pediatrics (AAP), which was aimed to reduce the frequency of sudden infant death syndrome (SIDS) by placing babies to sleep in the

INTRODUCTION: Textbooks commonly recommend using the true anterior-posterior (ap)-view with the patient's arm in a sling and therefore in internal rotation (IR) for radiologic diagnostic assessment of the proximal humerus after trauma. However, IR or external rotation (ER) may affect the projection of the head shaft angle (HSA) and therefore bias the diagnostic conclusion significantly. We hypothesized that neutral rotation (NR) of the arm is mandatory for true ap-view to provide true projection of the HSA.
MATERIALS AND METHODS: A simplified geometrical model of the proximal humerus was used

OBJECTIVES: Mechanical orthoses, such as the hip knee ankle foot orthosis (HKAFO) and the isocentric reciprocating gait orthosis (IRGO), are both used for walking in spinal cord injury (SCI) patients. The aim of this study was to analyze the energy expenditure during walking with these orthoses compared with a powered gait orthosis (PGO) in patients with SCI.
METHODS: Five patients with SCI who were experienced users of HKAFOs participated in this study. Subjects were also fitted with an IRGO and PGO and underwent a specific gait training program. Patients walked along a flat walkway using th

The Ponseti method of clubfoot correction is now widely practiced worldwide. Initial correction rates are nearly 100%, but subsequent relapses may occur in up to one-third of patients. Very little has been written by anyone other than Dr Ponseti about the characterization and treatment of recurrent clubfoot deformity following use of the Ponseti method. This review paper is the first one which draws together the current literature on the topic.

OBJECTIVES: Axillary and forearm crutches are commonly utilized in the treatment of foot and ankle injuries. In order to decrease the energy expenditure during mobilization, to prevent upper extremity complications, and to let the upper extremity free for other usages, we designed a new orthosis. The study is conducted to compare walking energy parameters of this newly designed orthosis with the axillary and forearm crutches.
MATERIALS AND METHODS: 10 healthy young male volunteers walked on treadmill with three different orthoses in randomized order. Oxygen expenditure, oxygen cost, rate of p

This case report describes the use of treadmill training without body weight support to improve walking speed in a child with diplegic cerebral palsy. The child was a six-year-old girl with spastic diplegic cerebral palsy. She walked short distances independently using a posterior support walker but was unable to keep up with her peers walking long distances. Speed of walking 50 feet and speed of walking from her classroom to the cafeteria (458 feet) were timed pretreadmill, posttreadmill, and 3 weeks following treadmill intervention. The GMFM-88 was administered pre- and posttreadmill interve

The objective of this study was to attempt the analysis of the ground-foot reaction forces in hemiplegic patients in routine gait re-education conditions, with and without supporting this process with typical orthopaedic aids. Ninety people with hemiparesis participated. The subjects were divided into three groups, according to their locomotion type (no walking aids, stick and elbow crutch). The ground-foot reaction forces during level gait trial were measured for each subject. Analysis was carried out by means of Kistler lane. The ANOVA results showed a considerable effect on almost all groun

Historically, trunk movement has been thought to be reactionary to lower body motions. The excessive trunk movement in patients with myelomeningocele may be a primary mechanism during ambulation. The purpose of this study was to quantify three-dimensional trunk movement in patients with myelomeningocele. Patients walked with ankle-foot orthoses (AFOs), both with and without crutches. Data were collected in reference to global and pelvic coordinates. Patients who walked with AFOs and crutches showed less trunk dynamic range of motion than patients without crutches. Between the two patient group

OBJECTIVE: To determine gait differences in a subject ambulating with a knee-ankle-foot orthosis (KAFO) with a locked knee joint versus an automatic stance-control knee joint.
DESIGN: Single-subject crossover design.
SETTING: Tertiary rehabilitation facility with a motion analysis laboratory.
PARTICIPANT: A 61-year-old ambulatory male volunteer with postpoliomyelitis walking with a stance-control KAFO.
INTERVENTIONS: Instrumented gait analysis and Physiological Cost Index in the locked knee and stance-control modes.
MAIN OUTCOME MEASURE: Differences in gait parameters.
RESULTS: On the br

BACKGROUND: The capacity of the plantar sole of the foot to convey information about foot position is reduced by conventional smooth boot insoles, compared with barefoot surface contact.
OBJECTIVE: To test the hypothesis that movement discrimination may be restored by inserting textured replacement insoles, achieved by changing footwear conditions and measuring the accuracy of judgments of the extent of ankle inversion movement.
METHODS: An automated testing device, the ankle movement extent discrimination apparatus (AMEDA), developed to assess active ankle function in weight bearing without

We introduce a novel orthosis used with serial manipulations for the treatment of 14 congenital clubfeet (six boys and three girls). The average age at the time of initial treatment was 3.5 months (range, 2-6 months). The average length of treatment was 4.9 months (range, 3-7 months). All patients traveled at least 480 kilometres for their clinic visits. According to the clubfoot classification of Dimeglio et al., 12 clubfeet (85%) were severe and two clubfeet (15%) were moderate. At the end of treatment, all clubfeet were improved by at least one level of grading. Although all clubfeet went o

A 5-year-old male child who had undergone orthopedic surgery for foot deformities three years prior presented to the outpatient clinic with a complaint of flexion restriction in both hands. The radiological evaluation of the patient showed C3-4 spondyloptosis. Halo traction was placed and posterior and anterior spinal fusions were performed respectively to achieve realignment of the spine. The postoperative period was uneventful and the patient has been followed for 24 months. The etiology, diagnostic features and treatment options of childhood cervical spondyloptosis have been briefly reviewe

For the non-operative treatment of flexion contracture of the haemophilic knee we have used serial casting and wedging in 58 patients, and extension/de-subluxation orthoses in 13 patients. On average it was possible to achieve -5 degrees of extension by 4 weeks, with only a little improvement in the following 4 weeks. The short--to medium-term results using either the extension/de-subluxation hinges or serial casting were similar. Both methods have been shown to result in significant improvement in joint contracture.

Current literature disputes Whitman's belief that muscle action plays the primary role in foot stability. Because of this faulty premise, Whitman included an excessively high medial flange in his orthosis. The high medial flange caused many patients a great deal of discomfort. To avoid this pain, the patients would not wear the brace, and as a result, it had little clinical value. For this reason, podiatrists seldom use the Whitman/Robert's orthosis unless it has been modified. Although Whitman constructed his brace under a false premise, it provided current researchers with a great deal of va

Flat feet are common in children. Loss of the medial arch may be due to intrinsic or extrinsic abnormalities of the foot. Physical evaluation usually suffices to identify the various growth stages which modify alignment and joint mobility, thus affecting foot function. Flat feet not caused by intrinsic structural abnormalities are right considered benign since they only exceptionally cause discomfort in adulthood. Shoe inserts provide only improved comfort during walking. Surgery is used in the extremely small proportion of patients with severe arch loss or deformities secondary to another con
